A survey of patients acceptability of diabetes education programmes in Italy.

When planning diabetes education programmes it would be useful to know in advance the patients' demands and expectations. To this purpose, a questionnaire was circulated throughout Italy. Eight hundred and forty-five forms were returned from 487 men and 358 women of age 49 +/- 18 (SD) residing in all regions of Italy. Fifty percent were on insulin, 37% on oral agents and 13% on diet only. Forty-seven percent believed they had a satisfactory knowledge of diabetes, 49% thought it fair or poor and 4% judged it non-existent. Such knowledge derived from diabetic clinics, mass media, specialized publications and family doctors. Only 11% had previously attended education programmes. The vast majority (83%) believed that better information would help to improve their diabetes care and were willing to attend education courses. Printed material and group learning were preferred to audiovisual aids and individual education. Doctors were preferred as teachers (83%) to other patients or paramedical staff. Patients were mostly interested in learning about complications (84%), diet (74%), control (63%), intercurrent illnesses (61%) and inheritance (59%). The least requested subjects were CSII (20%), the artificial pancreas (22%), insulin preparations (22%), pancreatic transplant (26%) and oral agents (31%).

The beta-glucuronidase release from macrophages activated by immune complexes of varying antigen/antibody ratio.

When unstimulated rat peritoneal macrophages are exposed in vitro to IC formed with BSA and specific rat anti-BSA IgG antibodies, an exocytosis of the lysosomal beta-G occurs. The maximal release of beta-G into the serum-free medium is induced, without cell lysis, by IC after a 6-h contact with the adherent cell population. This phenomenon is dose-dependent, and the percentage of beta-G in the medium is higher with IC in Ab excess than with other types of IC. In this homologous model (rat macrophages and rat antibodies) the Ag/Ab ratio of IC seems to represent an important factor of macrophage activation.

[Beta2 microglobulin, antibeta2 microglobulin activity and circulating immune complexes in rheumatoid arthritis. Serum and synovial study].

In this work, the authors found significantly raised serum Beta2m microglobulin values in a group of 120 cases of rheumatoid arthritis, without any difference between the serum positive group (55 cases) and the serum negative group (65 cases). Such a rise was found in the synovial fluids with a beta2 serum synovial fluid greater than unity in 81 % of cases of rheumatoid arthritis as against 6 % in a control group. The comparative study and the main immunological disorders permits one to note a correlation between the levels of circulating immune complexes (CIC) fixing the Clq and the serum beta2m in serum negative cases of rheumatoid arthritis (RA) (r = 0.52 p < 0.001) whereas this link does not exist in the serum positive group (r = 0.08). These conclusions led the authors to an anti-serum beta2m activity in the serum positive and negative groups of RA, which were very significantly different from the control groups. Furthermore, a study of complement activation, by titration of C3d showed a significant correlation between the levels of CIC and of C3d in RA -, but not in RA +. The authors discuss the participation of beta2m in the formatiton of CIC during RA and its role in the activation of complement.

Primary cardiac malignancy presenting as left atrial myxoma. Clinical and surgical considerations.

Primary heart neoplasms occur in 0.002-0.3% of autopsies: 30% are myxomas and 20-30% are malignancies, almost always sarcomas. Cardiac metastases are 10 to 40 times more frequent than primary heart cancer. We describe a case of a left atrial sarcoma erroneously diagnosed as myxomas preoperatively. Standard surgical indication for resection of cardiac myxomas is based on echocardiography. Because of the severity of cardiac malignant lesions than can mimic atrial myxomas at echocardiography, through preoperative investigation should be accomplished, best by magnetic resonance imaging. In case of suspected malignancy, total body computed tomography should be performed to avoid unnecessary cardiac operations in case of disseminated cancer. To date the only good medium and long-term results in the therapeutic management of heart sarcomas have been achieved by transplantation: the probable explanation is that criteria of surgical radicality should be those followed for soft tissue tumors located elsewhere in the organism.
