Cutaneous lesions in 67 cyclosporin-treated renal transplant recipients.

Sixty-seven out of 100 kidney transplant recipients treated with cyclosporin and methylprednisolone were evaluated for the presence of dermatological manifestations. Only 2 patients had no dermatological lesions; 80% had iatrogenic lesions, 38% infectious, 13% miscellaneous, 3% cancerous lesions, while 28% had cutaneous manifestations related to previous uremic state. Most of the lesions concerned the pilosebaceous unit: hypertrichosis (60%), epidermal cysts (28%), pilar keratosis (21%), acne (15%), folliculitis (12%) and sebaceous hyperplasia (10%). Among infectious manifestations, viral lesions were the most frequent and were very severe in the first month after transplantation. Two patients developed a squamous-cell epithelioma and a probable cutaneous lymphoma, respectively.

Unusual case of granulomatous dermohypodermitis with giant cells and elastophagocytosis.

A 27-year-old man presented with a reddish hard-elastic plaque on his back near the right armpit, which had slowly developed during 1 year. A granulomatous infiltration with many giant cells which were phagocytizing lymphocytes and elastic fibers was seen at histological examination. The lesion was removed and no new lesions developed after surgery. This case may represent a localized form of Convit's 'Progressive atrophying chronic granulomatous dermohypodermitis' or a rare form of 'Maladie de Besnier-Boeck-Schaumann chalazodermique', but it is also possible that it is a new entity never described before.

Nevus sebaceus. A clinicopathologic study of its evolution.

We studied 130 specimens of nevus sebaceus of Jadassohn. All structures derived from the surface ectoderm were found to be affected in varying degree. Several different histologic patterns were discerned. The nosological position of nevus sebaceus is discussed, and it is concluded that nevus sebaceus is a hamartoma that involves both epithelial and non-epithelial elements of the skin.

Liposomes induce chromosome aberrations in human cultured cells.

The genotoxic effect of multilamellar lipid vesicles (MLV) was analysed on cultured heteroploid and diploid human cells. Dose-dependent reduction of cell survival and mitotic rate as well as induction of chromosome aberrations were observed. Chromatid and chromosome breaks and chromatid exchanges were found in 24-h culture after liposome treatment, whereas chromosome rearrangements were prevalent at 48 h. Neutral (PC/Chol) and positive (PC/SA) MLV showed a greater damage than negative (PS/PC; PS) MLV. Fibroblasts were the most sensitive cell type. In the case of PC/Chol MLV vesicles, control experiments with PC and Chol of controlled purity ruled out the possibility that the observed chromosome aberrations were caused by toxic oxidation products present in commercial preparations.

Gingival hyperplasia by nifedipine. Report of a case.

We describe a case of gingival hyperplasia in a 36-year-old renal transplantation recipient treated with nifedipine for severe arterial hypertension. The appearance of the gingival disorder was probably related to nifedipine intake. Histologically, in addition to the findings of acanthosis, papillomatosis and connective tissue hyperplasia, there was also an important plasma cell inflammatory infiltrate.

Control of cell division by aphidicolin without adverse effects upon resting cells.

Aphidicolin, a tetracyclic diterpenoid obtained from the culture filtrates of Cephalosporium aphidicola and other fungi, inhibits the growth of eukaryotic cells and of certain animal viruses (SV40, Herpes and Vaccinia viruses) by selectively inhibiting the cellular replicative DNA polymerase alpha or the viral-induced DNA polymerases. The arrest of cellular or viral growth is thus due to inhibition of cellular or viral replicative DNA synthesis without interference with mitochondrial DNA synthesis, RNA, protein and nucleic acid precursors synthesis or other major metabolic pathways. The inhibition of all sensitive eukaryotic DNA polymerases by aphidicolin is competitive with respect to dCTP. Aphidicolin has thus proved extremely useful in elucidating the functional role of DNA polymerase alpha in nuclear DNA replication, of DNA polymerase gamma in mitochondrial DNA synthesis and both DNA polymerases beta and alpha in DNA repair synthesis. An important laboratory application of aphidicolin is the synchronization of the cell cycle of eukaryotic cells both in culture and in vivo. The properties of aphidicolin have recently aroused considerable interest for its possible exploitation in al practice. The mechanism of action of this drug suggests in fact that it may be useful for controlling excessive cell proliferation in patients with cancer, psoriasis or other dermatitis with little or no adverse effect upon non-multiplying cells. Interestingly, when administered to mice, the highest levels of aphidicolin are found in those tissues most actively proliferating with little or no aphidicolin present in neurons or myocardial cells.
