Humoral immunity to dietary antigens in atopic dermatitis. I. Biotin-avidin amplified ELISA-analysis for serum IgG antibodies.

Enzyme-linked immunosorbent assays (ELISA) employing a biotin-avidin amplification step are described for the quantification of human serum IgG antibodies to the dietary antigens ovalbumin (OA) and beta-lactoglobulin (BLG). The analytical quality of these assays was acceptable. Antibodies were measured in 16 patients with mild or moderate atopic dermatitis (AD), in 31 patients with a history of AD, and in closely matched controls. Levels of serum anti-OA antibodies did not differ in patients and controls, whereas anti-BLG antibodies tended to be higher in patients with mild or moderate AD than in controls (P less than 0.05).

Humoral immunity to dietary antigens in atopic dermatitis. II. Analysis of IgE and IgG subclass antibodies.

IgG subclass antibodies to two dietary antigens, ovalbumin (OA) and beta-lactoglobulin (BLG) were measured with quantitative ELISA-techniques in 16 patients with atopic dermatitis (AD) (6-21 years old) and closely matched controls. In addition, IgE-antibodies to OA, BLG and milk were determined with RAST. IgG subclass antibodies were frequently detected in IgG1 and IgG4 for both AD-patients and controls, quantitatively dominated by IgG4. The IgG4 anti-BLG antibody levels were significantly higher (P less than 0.001) in AD-patients (median: 1.1 microgram/ml, range: 0-24.0 microgram/ml) than in controls (median: 0.05 microgram/ml, range: 0-1.05 microgram/ml). No relation was found between IgG4 anti-BLG antibody levels, levels of IgE antibodies to milk or BLG, or severity of disease.

Atopic dermatitis. A genetic-epidemiologic study in a population-based twin sample.

Atopic dermatitis is a multifactorial disease that seems both to rise in frequency and to be dependent on a genetic predisposition. In order to clarify these issues we encircled a representative twin series with atopic dermatitis from a total twin population of 592 like-sexed twin pairs. We found that the cumulative incidence rate (0-7 years) of atopic dermatitis in Denmark has increased significantly from 0.03 for the birth cohort 1960-1964 to 0.10 for the birth cohort 1970-1974, that monozygotic twin pairs are more often concordant for atopic dermatitis than dizygotic twin pairs, that monozygotic twins run a risk of 0.86 of having atopic dermatitis if the twin partner has the disease, whereas the disease risk of 0.21 run by dizygotic partners does not differ from the frequency seen in ordinary brothers and sisters. The results indicate that genetic factors play a decisive role in the development of atopic dermatitis and that widespread environmental factors are operating in genetically susceptible individuals.

Purification of Bordetella pertussis antigens using monoclonal antibodies.

Eleven monoclonal antibodies were produced using whole Bordetella pertussis cells as the immunizing antigen. All monoclonal antibodies reacted with components of Bordetella pertussis, as visualized in immunoblotting of SDS polyacrylamide gels. Selected antibodies were coupled to Sepharose columns and used for isolation of the corresponding antigen. In all cases complete accordance was found between SDS polyacrylamide gel electrophoresis of the eluted antigen and the bands found in immunoblotting of the original extract stained with the respective monoclonal antibody. One major problem in the interpretation of the results was the finding that some of the monoclonal antibodies stained a number of bands in immunoblottings of crude B.pertussis extract. This phenomenon was shown to be caused by proteolytic degradation of the antigens, since prior addition of protease inhibitors to the extract resulted in the staining of only one band. The monoclonal antibodies showed different reactivity patterns with various strains of B.pertussis, B.parapertussis, B.bronchiseptica and less closely related bacteria. Two of the antibodies were strictly specific for B.pertussis.

Purification and partial characterization of filamentous haemagglutinin from Bordetella pertussis using monoclonal antibodies.

Two monoclonal antibodies against filamentous haemagglutinin (F-HA) from Bordetella have been produced. In immunoblotting of a SDS-polyacrylamide gel electrophoresis (SDS-PAGE) of high ionic strength extracts of Bordetella pertussis, the two monoclonal antibodies both stained several high molecular weight bands. The two monoclonal antibodies were used for immuno-affinity column one-step purification of F-HA from high-ionic-strength extracts of Bordetella pertussis. In gradient SDS-PAGE, the eluted F-HA is present as a major double band with a molecular weight of approximately 240,000 daltons and several minor bands with molecular weights down to 90,000 daltons. The monoclonal antibodies were used in a monoclonal antibody catching enzyme linked immunosorbent assay (ELISA) for sensitive and specific detection of F-HA. This ELISA system proved valuable in optimizing the elution conditions for the monoclonal affinity columns.

Ipratropium treatment for rhinorrhoea in patients with perennial rhinitis. An open follow-up study of efficacy and safety.

The efficacy and safety of intranasal ipratropium therapy for rhinorrhoea were tested in an open follow-up study of 20 adult patients with perennial rhinitis. The reduction of nasal hypersecretion of 40-45% obtained with 80 micrograms four times daily in a placebo-controlled study, was maintained over the next 4 weeks although the mean daily dose was reduced to 180 micrograms. The spray treatment was well tolerated and rhinoscopy, measurements of the sense of smell and the secretory responsiveness of the nose before and 10-48 months after the start of the trial did not reveal any signs of serious local adverse effects. It is concluded that ipratropium nasal spray is effective in the treatment of rhinorrhoea and appears to be safe for long-term use.

Unusual immunofluorescence findings in pemphigus vegetans.

An 84-year-old man with massive verrucous lesions in the intertriginous areas is reported. The clinical, histological and electron microscopical findings indicated pemphigus vegetans. However, in addition to the intraepidermal findings direct immunofluorescence showed a linear deposit of IgG and C3 at the basement membrane.

Effect of cyclophosphamide on spleen cell suppressor activity and tumour growth in mice.

Following a single injection of cyclophosphamide (Cy) a biphasic spleen reaction occurred. During the first week the spleen size as well as the proliferative activity of the spleen cells were markedly reduced. However, during the next week a compensatory reaction developed with spleen enlargement and cellular proliferation far above normal. In the period of increasing spleen size, days 7-12 after Cy injection, the mitogenic response of the spleen cells was reduced and simultaneously they developed a suppressor activity which was able to inhibit the mitogen response of normal spleen cells. This suppressor activity could be reduced if the mice were reconstituted by an injection of normal spleen cells after Cy injection. Injection of Cy one day before tumour transplantation resulted in reduced tumour growth in syngeneic recipients compared with non-treated controls. However, this inhibition of tumour growth was abolished when normal spleen cells were injected after Cy treatment.

The anti-allergic effect of the beta 2-adrenergic agent fenoterol in the nose.

Beta-adrenergic agents have a direct relaxing effect on bronchial smooth muscle. In vitro investigations of a number of sensitized preparations have demonstrated an inhibition of the antigen-induced histamine release. Grass pollen allergic patients were pretreated in the laboratory, out of the season, with different dosages of intranasal fenoterol before an allergen challenge and hay-fever patients treated with fenoterol during the season in a clinical trial. The laboratory studies demonstrated a moderate and significant (p less than 0.01) inhibitory effect on the immediate allergen-induced symptoms, suggesting an action on mediator release from mast cells. Fenoterol had only a slight, although significant (p less than 0.05), effect on nasal symptoms in the clinical study. In conclusion, fenoterol seems to have an anti-allergic effect, when applied to the airway mucosa, but the effect on allergic rhinitis does not seem sufficient to justify the introduction of fenoterol as a nasal spray.

Subpopulations of lymphocytes and lymphocyte transformation tests in atopic dermatitis: evaluation of a systemic treatment with a new chromone compound and comparison with a normal group.

In 28 adults patients with atopic dermatitis participating in a double-blind controlled clinical trial of systemic treatment with a new chromone compound, immune monitoring were performed with a number of in vitro tests. The parameters were phytohemagglutinin, pokeweed mitogen, concanavalin A, purified protein tuberculin derivative, Escherichia coli, Staphylococcus aureus, streptokinase/streptodornase, and allogene lymphocytes in a one-way mixed lymphocyte culture. No benefit could be proven for the drug in the clinical investigation or any change in the immunological tests during the trial. There was no demonstrable differences in the applied immunoparameters between mild and severe atopic dermatitis. The study group was compared with an age- and sex-matched control group and a general tendency to decreased T-cell function in the PHA, PWM, PPD and MLC tests together with a slightly increased HEAC percentage was found in the patients with atopic dermatitis.

Petechial reaction following patch testing with cobalt.

A total of 132 patch tested patients reacted with petechial reactions to cobalt chloride 1% in petrolatum; 23 were retested with various concentrations of cobalt. In about 60% of those retested the petechial reaction could be reproduced. Histopathological examination showed slight perivascular lymphocytic infiltration, swollen endothelium and extravasation of erythrocytes but no signs of vasculitis. It is suggested that in predisposed patients the petechial reaction following patch testing could be the result of primary irritation.

HLA antigens in atopic dermatitis. A family study.

The HLA antigens in a family with atopic dermatitis were studied. All family members with atopic dermatitis had haplotype A2, Bw35, Cw4. It has been recently demonstrated that HLA type Bw35 is found with an increased frequency in atopic dermatitis. There appears to be an inconsistent and weak correlation between atopic dermatitis and HLA-A, HLA-B and HLA-C loci.
