Management and treatment of central steep islands after excimer laser photorefractive keratectomy.

BACKGROUND: Permanent central steep islands are an undesirable phenomenon that cause distorted images and a significant reduction in visual acuity. We describe treatment of central steep islands with repeat excimer laser photoablation in the central cornea. METHODS: Three patients with preoperative refractions of -7.50 -2.50 x 170 degrees D (right eye), -8.00 -2.25 x 10 degrees D (right eye) and -6.00 -1.50 x 90 degrees (right eye) developed central steep islands which persisted more than 12 months. All patients lost more than two lines of spectacle-corrected visual acuity and complained of visual disturbances. We retreated the central steep islands with a VISX 20/20 excimer laser PRK ablation that matched the size of the central island measured on videokeratography. RESULTS: Symptomatic glare and distortion were significantly reduced and the central steep islands were resolved. Several weeks after reablation, spectacle-corrected visual acuity improved to the preoperative level. CONCLUSION: Central steep island, an infrequent complication of excimer laser photorefractive keratectomy, can be safely removed with a repeat laser ablation that matches the central circular steep area.

Peripheral melt of flap after laser in situ keratomileusis.

BACKGROUND: Laser in situ keratomileusis (LASIK) is an effective procedure to correct myopia. It may have complications related to the flap, such as epithelial ingrowth and stromal melt. METHODS: We report on a patient who developed extensive epithelial ingrowth and partial keratolysis of the flap following LASIK. This complication was treated by lifting the flap and removing the epithelium from within the interface. RESULTS: Progressive keratolysis (stromal melt) can result in irregular astigmatism and loss of vision as well as photophobia and ciliary injection. The pathogenesis is not completely understood although the epithelial ingrowth in the interface is always present, and epithelial-stromal interaction with production of proteases may be involved. CONCLUSION: Epithelial ingrowth may develop in the lamellar interface after LASIK and be associated with melting of the edge of the flap. This undesirable complication can be successfully managed with early surgical removal of the epithelium and proper attachment of the flap.

[Lhermitte-Duclos disease associated with tuberous sclerosis. A case report and review of literature].

INTRODUCTION: Lhermitte-Duclos disease is a rare disorder of the cerebellum of unknown origin in which dysplasic thickening of the cerebellar convolutions is seen. It usually occurs in young adults. Currently it is included in the phacomatosis group of disorders. CLINICAL CASE: A 19 year old woman attended the Emergency Department complaining of progressive orthostatic headache for the previous three months. On examination there were striking facial micronodular lesions suggestive of angiofibromas, a hypo-pigmented macula in the inframammary region and a hyperpigmented 'café-au-lait' macula in the right hypochondrium. On computerized tomography there was tetraventricular hydrocephalia. Cerebral magnetic resonance showed significant descent of the tonsils, hypertensive hydrocephalia and a lesion in the left cerebellum, apparently laminar hyperintensity in DP and T2, with thickening of some folia, not enhanced by intravenous contrast and suggestive of a dysplasic gangliocytoma. Laboratory investigations showed subclinical hypothyroidism. Other investigations were normal. The patient was treated by implanting a ventriculo-peritoneal shunt which has relieved the symptoms to date. CONCLUSIONS: Lhermitte-Duclos disease is probably not a single anatomo-clinical condition, assuming that it may be a cerebellar hamartoma associated with a phacomatosis with few clinical signs, whether it be Cowden's disease, tuberous sclerosis as in this case or an 'overlapping' syndrome. The magnetic resonance findings are necessary and sufficient for the diagnosis of Lhermitte-Duclos disease.

A study of the aeromycoflora of Cádiz: relationship to anthropogenic activity.

We describe a quantitative and qualitative study of the fungal spores found in the air of Cádiz during 1989 using a Cour-type trap. The results of this study can be extrapolated to other coastal cities of southern Europe with a Mediterranean climate. The spores identified have been classified into 25 taxonomic categories. The most abundant were Cladosporium, Chaetomium and Ustilago, and the most frequent, in addition to those mentioned, were Alternaria, Ascophyta and Venturia. The great abundance of Cladosporium is in accordance with the coastal situation of the city. Cladosporium, Alternaria, Curvularia and Stemphylium reached maximum concentrations jointly in October, 1989. They showed mutual cross-reactions. Ustilago and Nigrospora appeared during the period of cereal harvesting and storage.

[Interferon-beta response in multiple sclerosis associated with pre-treatment disability].

AIM: To study pre-treatment clinical features and influence of neutralising antibodies (NABs) in clinical response to interferon-beta (IFNB). PATIENTS AND METHODS: We analysed clinical characteristics and NABs to IFNB in 96 multiple sclerosis patients treated with IFNB. Clinical response was established by clinical criteria: = 1 relapse or an increase = 0.5 or 1 point in the Expanded Disability Status Scale (EDSS) score after one year of treatment compared with the year prior to IFNB therapy. RESULTS: Baseline clinical characteristics were similar for responders and non-responders, except for a significantly higher baseline mean EDSS score in non-responders. Time-to-first-relapse was longer and the number of patients relapse-free was higher for NAB-negative patients, but we were unable to show an association with the disability status, probably due to sample size. CONCLUSIONS: Response to IFNB was significantly associated with pre-treatment disability measured by the EDSS. The presence of NABs to IFNB presented a delayed negative effect for relapses.

[Fibroepithelial polyp of the ureter. Report of a new case].

Fibroepithelial polyps are rare benign tumours presenting in one every hundred cases of ureter primitive tumours. Interest on this type of pathology is founded in its low incidence and on being able to reach a pre-operative diagnosis prior to taking surgical action. Uretero-renoscopy is currently the best method available for the identification and histological diagnosis of this polyp.

[Verrucous carcinoma of the penis. Presentation of 3 new cases].

Penis carcinoma shows an incidence between 0.3-1% of all male neoplasias. The verruciform variety represents 5-10% of total penis carcinoma. Verruciform carcinoma is a variety of squamous carcinoma presenting a usually benign behaviour and related to some virus species (H.P.V.). Three cases are presented with ages ranging between 47-87 years-old, all of which undergoing partial penectomy, their progression being monitored for two years.

[Cavernous angioma of the testis. Presentation of a new case].

We present a fresh case of testicular angioma in a 15-year-old patient. The symptomatology displayed, along with the scrotal echograph, made us suspect the existence of testicular tumoration, which led us to carry out surgical exploration. After orchiectomy, it is Pathological Anatomy that provides us with the final diagnosis. We comment on the different histological patterns and make a review of the literature.

Prolapsed lumbar disk: value of CT in diagnosis.

A group of 40 patients with symptoms of prolapsed lumbar disk, seven of which were recurrent after surgery, was studied by computed tomography. In all cases, the diagnosis was confirmed by myelography and posterior surgery. Computed tomography was performed after the disappearance of myelographic contrast medium. Positive herniated disk was shown by computed tomography in 88%. In postoperative cases, computed tomography after intravenous contrast enhancement favored the recognition of postoperative scar, rather than recurrent herniation. Computed tomography also facilitated the diagnosis of spondylotic lesions, which may accompany herniated disks. Computed tomography should reduce the need for myelography, which is reserved for cases with doubtful computed tomographic findings.

Treatment of acute leukemia during pregnancy: presentation of nine cases.

The hematologic and obstetric problems of nine pregnant patients with acute leukemia are analyzed. Seven patients received chemotherapy during various trimesters of pregnancy, four during the first 10 weeks, and there was no evidence of congenital malformations in any of their offspring. Six of nine babies are alive and healthy with a normal growth 2 months to 15 years after birth, one child was stillborn, and two children died 21 and 90 days after birth. These results were compared with others in the literature and the conclusions are that pregnancy is not a contraindication for treatment of acute leukemia, that cytotoxic drugs may not cause congenital malformations, and that the management of pregnant patients with acute leukemia should be carefully planned.

[Preliminary results of indirect isotopic lymphography with colloidal rhenium marked with Tc 99m in urinary tract pathology].

The authors have carried out a study of a new method for studying the lymphatic system in patients with a neoplastic, urological pathology, by using lymphogammagraphy with radionuclids. It involves the comparative analysis of the lymphatic ganglion chains. This method consists in the subcutaneous injection into the interdigital space of a radionuclid, Colloidal, Rhenium Sulphide, marked with Tc-99m and with external detection by means of a gammacamera and with lymphatic channeling not being necessary.
