[Neurological manifestations of fat embolism syndrome].

Fat embolism is a frequent complication of long bone trauma. Although neurological signs are key elements in the diagnosis of this syndrome, little attention to them has been paid in the spanish literature. We present a retrospective study of 13 patients with fat embolism syndrome in which we focused on neurological manifestations and alterations in computerized tomography (CT) of the head. The most frequent symptom was impairment of consciousness of varying degrees. Three patients out of the 13 also presented focal signs (hemiparesis and partial seizures). CT revealed edema (4/7) and multiple low density foci (1/7). All but one patient recovered without sequelae. Our study does not allow us to establish either the physiopathology of fat embolism syndrome or the most effective treatment.

[Idiopathic thrombosis of the renal vein. Report of a new case].

Thrombosis of the renal vein in adult patients is a rare entity, commonly associated to tumoral masses or hypercoagulability states. Contribution of one case of thrombosis of the renal vein in an 83-year old male patient, and review of current literature.

Nail-patella syndrome and pre-eclampsia.

We present here the first described case of Nail-patella syndrome (NPS) and pregnancy. Complications occurred during the pregnancy with the onset of preeclampsia at 22 weeks, leading to intrauterine fetal death at 24 weeks. The nephropathy of the NPS began clinically during the course of gestation. Postpartum, it persisted as isolated proteinuria, which became a nephrotic syndrome 18 months later.

Role of Na+/Ca++ exchange in the relaxant effect of sodium taurocholate on the guinea-pig ileum smooth muscle.

Sodium taurocholate (NaTC), at concentrations below the critical micellar concentration, caused a transient relaxation of isolated guinea-pig ileum smooth muscle strips. The relaxation was not inhibited by previous incubation with either 10 microM ouabain, 0.4 mM d-tubocurarine or 0.5 microM apamin, ruling out the participation of hyperpolarization of the plasma membrane induced by either stimulation of Na+/K+ ATPase or by opening of Ca(++)-dependent K+ channels. In guinea-pig ileum smooth muscle cultured cells, addition of NaTC (1 mM) stimulated Na+ uptake and Ca++ efflux. The relaxation induced by NaTC was inhibited by 3',4'-dichlorobenzamil, a blocker of the Na+/Ca++ exchanger. Preincubation with NaTC, or its addition during the early stage of the tonic response of the ileum to acetylcholine, enhanced that response, whereas a relaxation was observed when NaTC was added at the late stage of the acetylcholine response. In cultured cells, NaTC potentiated the stimulation of Ca2+ influx by acetylcholine. Our results suggest that NaTC acts on the smooth muscle cell membrane causing a stimulation of the Na+/Ca++ exchange mechanism.

Oral administration of insulin in winter-acclimatized carp (Cyprinus carpio) induces hepatic ultrastructural changes.

1. The intestinal absorption of insulin in carps was assessed examining the transepithelial passage of ingested gold-labeled hormone by electron microscopy. Insulin transfer occurred mainly through the intercellular spaces between the enterocytes. 2. When reaching the lamina propria, the gold-labeled hormone gathered predominantly around the granules of the granular cells, and therefore can enter the circulatory system via the blood capillaries which are found in close contact with these cells. 3. Winter-acclimatized carp were also capable of internalizing the hormone when fed with insulin. 4. Furthermore, the absorbed hormone revealed full activity in regard to the observed changes in the ultrastructure of the liver cells of the treated cold-adapted fish. 5. The fish ingesting the hormone underwent the same type of hepatic ultrastructure reprogramming observed when winter-acclimatized carps are injected intraperitoneally with insulin, i.e. conversion to a phenotype corresponding to hepatocytes from summer-adapted carp. 6. The oral absorption of insulin by winter-acclimatized fish and its effect in reversing the cold-adaptive state might be useful for the fish culturing industry.

[Evaluation of prazosin in the treatment of benign hypertrophy of the prostate. A case-control study].

A control-case study conducted in 56 patients, all diagnosed with benign prostate hypertrophy of which 36 were Prazosin-treated patients and the remaining 20 were controls. The patient's mictional signs and symptoms as well as flow parameters were monitored at 3 and 6 months, respectively. We conclude that Prazosin is an alpha-blocker useful in early prostate disorders and when no obstructive urological disease is present.

[Neurologic complications induced by the treatment of the acute renal allograft rejection with the monoclonal antibody OKT3].

The treatment of the acute renal allograft rejection with the monoclonal antibody orthoclone OKT3 produces both systemic and neurologic alterations. In a series of 21 patients with an acute renal allograft rejection treated with this monoclonal antibody, 20 with a renal allograft transplantation and one with a renal and pancreatic allograft transplantation, 29% referred headache associated with fever and vomiting, and 14.2% presented severe neurological alterations induced by the treatment. We stress the need to know these secondary effects to differentiate them from other central nervous system disorders, particularly those of infectious origin.

Presentation of one case of retrovesical hydatidic cyst with no evidence of hydatidosis in other abdominal organ. Both the case and the medical-surgical procedures performed are described.

[Simultaneous occurrence of adenocarcinoma of the prostate in a father and son].

We describe two cases of adenocarcinoma of the prostate, which deserve being commented because of the unusual features of their occurrence. These were two stage D2 adenocarcinomas discovered within one year in a father and his son. One of the tumors was identified following hemostatic disorders. Thus three aspects of carcinoma of the prostate are studied: their possible occurrence in younger patients, the paraneoplastic disorders of coagulation, and the familial forms.

Reduction of glutathione content by 12-O-tetradecanoylphorbol-13-acetate in confluent, but not in sparse cultures of human diploid fibroblasts.

Treatment of confluent cultures of human diploid fibroblasts with 12-O-tetradecanoylphorbol-13-acetate (TPA) (10(-7) M) resulted in a 70% reduction of the glutathione (GSH) content, compared with untreated controls. The effect, which was dose-dependent, was observed 8 h after the beginning of the treatment could be followed for up to 72 h. On the other hand, GSH reduction was specific for confluent cultures, as the level of glutathione remained unchanged by TPA treatment of sparse cultures. The addition of immobilized plasma membrane proteins to sparsely seeded cells has been shown previously to induce cellular reactions which are characteristic for confluent cultures. It was shown that TPA treatment of sparse cultures grown in the presence of immobilized plasma membrane proteins also resulted in a 70% reduction of glutathione content. These data agree with the postulated involvement of redox reactions in tumor promotion, and point to a central role of cell-cell contacts in the regulation of biochemical events which are critical in tumorigenesis.

Role of glutamine as a direct co-repressor of glutamine synthetase in Rhodobacter capsulatus E1F1.

High performance liquid chromatography (HPLC) has been used to determine the internal levels of amino acids in Rhodobacter capsulatus E1F1 cells, subjected to different treatments and nutritional conditions. Glutamine synthetase activity and enzyme concentration correlated negatively with the level of glutamine, suggesting that glutamine per se acts as a co-repressor in the enzyme synthesis. Moreover, addition of the specific inhibitor L-methionine-D,L-sulfoximine, that produced an increase in enzyme concentration, specifically promoted a depletion of intracellular glutamine.

Genetic heterogeneity of thalassemias in Mexican mestizo patients with hemolytic anemia.

Twenty-eight Hb abnormalities (16 thalassemias, 10 Hb variants and 2 hereditary persistences of fetal hemoglobin) were detected in an etiopathogenic search in 131 patients with hemolytic anemia. The observed thalassemic genotypes [6 beta(0)/beta, 2 (delta beta) (0)/beta(0), 2 beta(0)/beta(+), 2 beta(0)/beta(s), 2 beta(+)/beta(+), 1 beta(+)/beta(s) and 1 (delta beta)(0)/beta] showed heterogeneity in the population studied. A native origin of some of these mutations is considered.

Epirubicin in rectal cancer.

Taking into account previous results with epirubicin in colorectal cancer, in January, 1985, an oriented Phase II trial was started in patients with measurable rectal cancer, previously untreated with chemotherapy. Ten patients were treated with 80 mg/m2 every 3 weeks, and another 10 patients with 100 mg/m2 every 3 weeks. One patient from the 80 mg/m2 group and 3 from the 100 mg/m2 group reached partial remission for 4, 9+, 15 and 72+ weeks. Median survival for all patients was 16 months. Hematological toxicity was not a limiting factor. Anemia was significantly more frequent in the higher dose group; clinical cardiotoxicity was not observed. The incidences of nausea/vomiting and mucositis were low. Considering the low toxicity and the responses observed, additional studies seem to be indicated with an increase in the epirubicin dose.

G-6-PD Jalisco and G-6-PD Morelia: two new Mexican variants.

Two new G-6-PD variants designated G-6-PD Jalisco and G-6-PD Morelia were identified in two unrelated Mexican families. An additional G-6-PD variant was found in each family: G-6-PD Trinacria and G-6-PD A-. In both families compound heterozygotes were identified. G-6-PD Jalisco and G-6-PD Morelia belong to Classes 3 and 4, respectively. G-6-PD Morelia is the first variant from its class with a high Km for NADP and a low Ki for NADPH.
