Comparative studies of normal, 'spontaneously' transformed and malignant human urothelium cells in vitro.

In numerous in vitro studies of cell-mediated cytotoxicity directed against transitional cell carcinomas (TCC) urothelial cells of normal origin have served as control material. In the present work, which includes 13 human bladder cell lines of non-malignant or malignant origin, a comparison was made of morphology, chromosomal mode, growth fraction and tumorigenicity in nude mice. Signs of apparently spontaneous transformation were seen in two cell lines of non-malignant origin. The possibilities for cross-contamination as an explanation of these signs of 'spontaneous' transformation are discussed. During these studies radiation-dependent cytotoxicity of disposable microtissue culture plates was revealed as another important source of error.

Cisobitan in treatment of prostatic cancer. A prospective controlled multicentre study.

Cisobitan, an organosilicon compound with estrogenic and antigonadotropic properties has been evaluated clinically in comparison with an estrogen preparation. In a multicenter study a total of 140 patients with well and moderately well differentiated prostatic cancer were randomly allocated to treatment with Cisobitan or Estradurin/Etivex, 70 to each group. Of 34 patients with poorly differentiated prostatic cancer 18 were given Cisobitan--and 16 were given Estracyt-treatment. Among the patients with well and moderately well differentiated prostatic cancer there were, disregarding mortality, no major differences in subjective, objective or laboratory response to the two kinds of treatment. The pattern of side effects was similar, but oedema requiring diuretics occurred more often in the estrogen treated group. There was a significant difference in mortality at 12 months between the groups, two in the Cisobitan group and ten in the estrogen treated group. Cancer was the cause of death in two patients in the estrogen treated group. All other patients succumbed in cardiovascular diseases. At 24 months the difference in mortality rate was less pronounced: Another ten patients had died in the Cisobitan treated group and seven among the estrogen treated patients. Cancer was responsible for the deaths in seven of the Cisobitan patients compared to four of the estrogen treated patients. Within three years one more patient in both groups had died. Of the 34 patients with poorly differentiated cancer, twelve were alive at the 24 months' follow up, six in the Cisobitan group and six in the Estracyt group.

Serum levels of radioreceptor-assayable somatomedins in children with minor neurodevelopmental disorders.

The somatomedins are a family of multitarget, growth-promoting hormones suggested to play a regulatory role in the central nervous system. Serum concentrations of somatomedins were determined by a radioreceptor assay in 26 patients (19 boys and 7 girls) with minor neurodevelopmental disorders. Five of these 26 patients showed high serum levels (greater than 2 S.D.) of somatomedins. It is concluded that patients with elevated concentrations of somatomedins might represent a pathogenetic subgroup.

Lymphocyte-dose response relationship in spontaneous lymphocyte mediated cytotoxicity (SLMC) measured by microcytotoxicity assay.

Titrations of spontaneous lymphocyte-mediated cytotoxicity (SLMC) in a 44-hour microcytotoxicity assay (MA) by serial dilutions of effector lymphocytes were performed to characterize the lymphocyte-dose response relationship, and to investigate the methodological basis for comparative cytotoxicity studies. The following lymphocyte fractions were used: unseparated, E rosetting, non-E rosetting, EA rosetting, and non-EA rosetting. Assays were performed against target cell lines from normal and malignant urothelium with high or low sensitivity to SLMC in allogeneic and autologous combinations, and against an osteosarcoma cell line. Analysis of SLMC titration curves showed that two different mathematical models could be used to describe the relationship between lymphocyte concentration and SLMC, I: linear relationship (log lymphocytes, CI) with exclusion of both high and low CI values, and II: sigmoid relationship (log lymphocytes, CI) which was transformed to linear relationship (log lymphocytes, log CI/100 - CI), including also high and low values. It is pointed out that perfect quantitative comparisons of SLMC in different combinations were complicated by statistically significant differences between the curve slopes. However, estimations based on experimental results in a range close to 50% cytotoxicity index (CI) may be acceptable, as the differences between slopes were small.

Cardiovascular effects of intravenous administration of tetracycline in cattle.

Tetracycline chloride dissolved in saline was injected intravenously to seven cows. Two doses of tetracycline were used: 5 and 10 mg/kg b.wt, and the injections were given over a period of either 10, 60 or 300 sec. A number of the cows collapsed shortly after the injection was completed, usually when the 10 mg dosage was given in 60 sec. When the same dose was given over a period of 5 min none of the cows collapsed. A more or less pronounced drop in blood pressure could be detected during or shortly after the injection; in those cows which collapsed the blood pressure fell almost to zero. The predominant change in pulse rate in connection with the tetracycline administration was a decrease which could be quite marked, pulse rates falling as low as 10-20 per min. Simultaneously with these changes in blood pressure and pulse rate severe abnormalities in ECG could be observed. Pre-treatment with a normal therapeutic dose of calcium borogluconate intravenously prevented collapse in the cows and diminished the drop in blood pressure associated with an ensuing tetracycline injection. It is concluded that intravenous injection of tetracycline is hazardous, but that collapse can be avoided by giving the injection very slowly over a period of no less than 5 min.

Urinary protein profiles in patients with urothelial bladder tumours.

Urinary protein profiles were studied in 32 patients with urothelial bladder tumours, using immunonephelometry. In patients without residual or recurrent tumours a glomerular type of proteinuria was demonstrated. In patients with grade III-IV transitional cell carcinoma the glomerular proteinuria was more severe than in patients with grade I-II tumours. Increased excretion of low molecular weight proteins was observed only in patients with grade III-IV tumours. The use of the increased relative clearance of albumin, transferrin, and haptoglobin as a diagnostic test was attempted in detecting residual or recurrent tumours. In patients with grade III-IV tumours the predictive value of a positive or negative test was 92% and 100% respectively, whereas the relative protein clearance was without predictive value in patients with grade I-II tumours.

Extraperitoneal pelvioscopy: a new aid in staging of lower urinary tract tumors. A preliminary report.

A new technique to obtain biopsy specimens from the pelvic lymph nodes is presented. A mediastinoscope introduced through a small incision in the abdominal wall allows the identification of the pelvic nodes. A series of 12 patients with prostatic or bladder carcinoma is presented and the feasibility of the method is documented. There was no morbidity associated with the endoscopic node biopsy. Suggestions for modification of the instrument are made. The method is now under further scrutiny to establish its place among other available methods to study lymphogenous spread of pelvic malignancies.

Renovascular hypertension treated by autotransplantation.

Renal autotransplantation was performed in 16 patients with severe renovascular hypertension. The hypertension was cured in 13 patients, 2 patients were improved and one patient had no benefit. The postoperative function and morphology of the transplanted kidneys remained unchanged. In our view, renal autotransplantation is a valuable method in the surgical treatment of renovascular hypertension.

Hearing in women survivors of myocardial infarction.

Hearing for pure tone air-conduction in a sample of women survivors of myocardial infarction (MI) and a random sample of women living in Copenhagen has been examined. For both groups we found that the hearing decreases with age, but the MI survivors at all frequencies have the poorest hearing compared with the random sample. There was no difference between the two groups as regards smoking habits or blood pressure. The noise exposure in the two groups was also the same. There was a significant difference in the mean values of s-cholesterol between the two groups, but we found no correlation between s-cholesterol and hearing in the random sample. Our study supports the hypothesis that MI survivors are more biologically aged than age-matched controls.

Netilmicin therapy of patients with septicaemia and other severe infections.

25 patients were treated with netilmicin because of suspected or verified septicaemia or other severe infections. Netilmicin was administered intramuscularly in a dose of 2 mg/kg body weight every 8 hours. For patients with elevated plasma creatinine the dosage was reduced according to the degree of elevation. Average length of treatment was 7 days (3-12 days). In 22 patients all symptoms and signs of infection disappeared. In one patient antibiotic treatment had to be changed and in two patients abscesses had to be surgically drained. Kidney function was monitored during and after treatment by plasma creatinine measurements and 51Cr-EDTA clearance. Netilmicin was discontinued in 4 patients because of an increase in plasma creatinine but little or no drug related nephrotoxicity was observed. Audio-vestibular function was monitored during and after treatment. Netilmicin was discontinued in one patient because of slight subjective loss of hearing. Subsequent audio-vestibular examination was normal.

The loss of creatine phosphokinase (CK) from intramuscular injection sites in rabbits. A predictive tool for local toxicity.

The CK activity was measured in muscle tissue taken from the injected area (dorsal longissimus muscle) and the contralateral side of the injection site 72 hours after intramuscular injection into rabbits of 1 ml of different dilutions of propylene glycol or glycerol formal in distilled water or 0.9% saline. The total loss of CK activity from the injection site was calculated as the difference between the CK concentration in the normal muscle tissue and that of the injection site from the same animal. From the results the arbitrary amount of muscle tissue depleted of CK activity was further calculated and compared with the severity of the gross pathological findings. A large necrotic area at the injection site was present in all samples with more than 1 g of muscle tissue depleted of CK activity. Minor and probably acceptable pathological changes were found in samples with less than 1 g of muscle tissue depleted of CK activity. The local damaging effect of drug preparations for intramuscular use can thus be evaluated from the calculated amount of muscle tissue depleted of CK activity.
