Hypotensive shock associated with bradycardia after intravenous injection of contrast medium.

A case of hypotensive shock associated with bradycardia that occurred after injection of nonionic monomeric contrast medium is described. Whether the pathogenesis of this type of reaction is solely vasovagal or is directly caused by the contrast medium is not known. The number of occurrences after administration of nonionic monomeric versus conventional contrast agents should be compared to solve this problem.

Unique etiology of tubal ectopic pregnancy.

We report a case of hysterosalpingography inadvertently performed during early intra-uterine pregnancy. The radiologic procedure seems to have displaced the amniotic sac from the uterine cavity into the Fallopian tube, and thus has been the etiologic factor underlying a tubal ectopic pregnancy.

Physician visits and prescribed drugs among young children and their mothers.

The extent to which use of medical services by young children could be predicted by mothers' use of medical services and by certain family characteristics has been studied. The study population comprised 591 children two to four years of age in a Swedish rural municipality. Register data on physician visits and prescription drug purchases during 1979 constituted main sources of information. Statistically significant positive associations were found between number of physician visits made by the children and by their mothers and between number of prescription drug purchases made for the children and for the mothers. More physician visits and prescription drug purchases were made for the children of younger mothers than for those of older mothers. Six to eight per cent of the variation in the use of medical services by the young children was explained in multiple regression models including mainly mothers' use of medical care, mothers' age and children's age.

Lunatomalacia in a child.

A case of advanced lunatomalacia in an 8-year-old girl treated by immobilization is described. After 2 years of treatment, the girl had a normal wrist, and radiographically the condition had improved, albeit with some collapse of the radial part of the lunate. Our case suggests that the natural course of lunatomalacia is favorable in children as compared with adults.

Calcaneus fracture in the child.

We reviewed 80 fractures of the calcaneus in 78 children through a 30-year period. The fractures were more equally distributed between the sexes than in adults. In contrast to adults, intraarticular fractures of the calcaneus in children constitute less than half, probably because a lower proportion of children sustain the fracture as a result of a fall. Associated injuries were mainly encountered by children who were traffic accident victims. It is disputable that the calcaneus in children is more resistant to fracture than in adults. The rarity of the fracture in children is more likely the result of less exposure to trauma leading to calcaneal fractures and also that the fracture is often overlooked.

A comparative study of hysterosalpingography and endoscopy/laparotomy in infertile patients.

A comparison of hysterosalpingography (HSG), using Lipiodol ultrafluid as contrast medium, with endoscopy/laparotomy in the evaluation of the tubal function in 124 infertile women is presented. There was agreement between the results of both methods in 73.4% of the patients. The false-negative rate was 2.4% and the false-positive rate was 8.9%. Complications in the form of intravasation were seen in three patients but none of them had any symptoms. The conception rate was 35% in the subgroup of patients with normal HSG. The authors recommend the continuous use of HSG in the overall evaluation of infertile patients. It is recommended to use oil-soluble contrast media because of its high diagnostic accuracy, low frequency of complications and its therapeutic advantages.

Radiologic aspects of lunatomalacia.

A radiological classification of lunatomalacia, based upon the chronological radiologic manifestations in 93 patients is presented. Compression fractures were not seen as the initial event. Fifty-seven per cent of the patients had a short ulna (ulna minus variant). The presence of ulna minus variant was especially observed in the younger patients. In 16 patients suffering from a lunate fracture and in 20 patients with lunate or perilunate luxations, lunatomalacia did not develop. Immobilization as treatment of the disorder resulted in a progressive course in all patients, but one, an 8-year-old girl. The lesion of the lunate was arrested and tended to heal following radial osteotomy in all patients.

Cisplatin as a first-line treatment in T2 and T3 bladder carcinoma.

Twenty-two patients with bladder cancer stage T2 or T3NxM0 received preirradiation treatment with cisplatin. Three complete and seven partial remissions were achieved. Responders received additional cisplatin concomitantly with radiotherapy, for a total of eight complete remissions. Toxicity from cisplatin alone or in combination with radiotherapy was moderate. Seven of the ten responders following preirradiation cisplatin are alive without evidence of disease. All nonresponders have died due to relapse (follow-up time, 46-80 months). Randomized trials with and without preirradiation cisplatin are warranted to establish whether cisplatin improves the prognosis of patients with invasive bladder cancer.

Metabolism of sulfadiazine in neonatal and young pigs. Comparative in vivo and in vitro studies.

Metabolism of sulfadiazine (SDZ) was studied in vivo and in vitro during postnatal development of piglets in order to examine whether in vitro metabolism approaches the in vivo situation. Experiments were performed in 1-day-, 8-day- and 60-day-old piglets. In vivo: 14C-SDZ was injected intravenously and urine and tissue samples collected after 3 hr. Urinary excretion data as well as data from liver and kidney tissue indicated a relatively high capacity for acetylation at birth, while the capacity for oxidation is low during the first week of life. At 60 days of age the acetylation and oxidation of SDZ is equal each accounting for about 20% of the amount excreted in urine. In vitro: Incubation of subcellular fractions of liver and kidney showed that acetylation of SDZ in liver reached maximum within 1 week. Oxidative activity was absent at 1 day, present at a low level at day 8, and at a high level at day 60. Neither acetylation nor oxidation of SDZ took place in kidney. The results show a close correlation between in vivo and in vitro results with respect to the developmental pattern seen in piglets during the postnatal period of life.

Clinical evaluation of ofloxacin versus ciprofloxacin in complicated urinary tract infections.

Ofloxacin was administered to 18 patients and ciprofloxacin to 17 patients suffering from acute or chronic complicated urinary tract infections. Patients received doses of 100 mg ofloxacin, b.i.d., or 250 mg ciprofloxacin, b.i.d., for seven days. The clinical and bacteriological response were evaluated after 17 days. Resolution of symptoms were found in approximately 60% of cases for both treatments and bacteriological eradication was 66% for ofloxacin and 50% for ciprofloxacin. Side-effects such as itching and rash were reported in three cases. The results indicate that ofloxacin and ciprofloxacin are safe and effective agents in the treatment of complicated urinary tract infections.

Influence of treatment with diethylstilbestrol for carcinoma of prostate on platelet aggregation and plasma lipoproteins.

Treatment of prostatic carcinoma with estrogens is accompanied by an increased risk for thromboembolic and cardiovascular complications. The underlying mechanisms are still unknown. Patients treated with diethylstilbestrol (DES) were compared with patients given no estrogen treatment regarding factors (platelet aggregation in vitro and plasma lipoproteins) that have been suggested to contribute to increased thrombogenesis and cardiovascular risk. The results do not show any increase in in vitro platelet aggregation in patients treated with DES compared with those given no treatment. This indicates that hyperaggregability does not contribute to the increased incidence in thromboembolic events seen in DES-treated patients. This is in contrast to the increased platelet aggregation previously described in patients treated with polyestradiolphosphate + etinylestradiol. The changes in plasma lipoproteins observed during DES-treatment are generally considered beneficial from an atherogenic point of view and do not appear to cause the elevated incidence of cardiovascular disease in these patients.

Transpubic cystectomy and ileocecal bladder replacement after preoperative radiotherapy for bladder cancer.

Radical cystectomy was modified by leaving the apical prostatic capsule to facilitate anastomosis of the isolated ileocecal segment to the urethra and to preserve erectile potency. The transpubic approach was used to increase the exposure, and to facilitate dissection and anastomosis. A total of 15 patients with stages T1 to T4 bladder tumors underwent the operation: 13 after preoperative radiotherapy with 4,000 rad and 2 had salvage cystectomy after 6,000 rad. One patient died postoperatively. The remaining 14 patients underwent urodynamic evaluation 3 to 6 months postoperatively. The maximum urine flow rates were almost normal and none of the patients had significant residual urine. Daytime urinary continence was satisfactory in 13 patients and 1 was moderately incontinent. All of the patients were incontinent at night, probably owing to peristaltic contractions in the intestinal bladder and relaxation of the pelvic floor muscles. Preoperatively, 8 patients experienced erections and 7 had intercourse. Postoperatively, erectile potency was preserved in 4 patients and 3 had sexual function. No orthopedic disability occurred postoperatively. The median followup was 20 months, with a range of 3 to 30 months. There have been no local recurrences. A year postoperatively 6 of 9 patients had sterile urine. This technique makes it possible to avoid a urinary stoma, to obtain satisfactory voiding and urinary continence in almost all cases, and to preserve sexual function in some patients after cystectomy.

Treatment of renovascular hypertension by unilateral nephrectomy. A follow-up study in patients above 60 years of age.

The long term results of surgical intervention in 26 elderly patients with renovascular hypertension are presented. All patients were above 60 years of age at the day of operation. The majority of the patients had atherosclerotic renovascular disease with only one case of fibromuscular dysplasia. Several patients had severe extrarenal atherosclerotic disease. The diagnosis of renovascular hypertension was based upon the results of isotope renography, renal arteriography and renal vein catheterization. All patients underwent unilateral nephrectomy. Notably, no deaths or complications occurred in relation to surgery. At the follow-up study, blood pressure was lowered and the requirement for antihypertensive drugs reduced in 86% of the patients. We conclude that unilateral nephrectomy in elderly high risk patients with renovascular hypertension is a safe and efficient procedure.

Bladder tumour control by abdominal ultrasound and urine cytology.

A blind study comparing abdominal ultrasound and cystoscopy was carried out in 186 patients. 20 bladder tumours sized from 2 to 5 mm were overlooked. Combination with urine cytology increased the diagnostic sensitivity. In order to reduce costs and patient inconvenience in the bladder tumour control population abdominal ultrasound and urine cytology is advocated as an alternative to cystoscopy. This control modality seems safe in patients with "low-risk" bladder tumour disease.
