Pseudomonas aeruginosa sepsis presenting as peripheral subcutaneous nodules. Report of 2 cases.

Two patients developed disseminated subcutaneous nodules with febrile illness. In both cases, Pseudomonas aeruginosa was isolated from the lesions; blood cultures yielded the same organism in one case, and were negative in the other. The portal of entry was thought to be a jugular hemodalysis catheter in the first case and a necrotic zoster complicating lymphoma in the second case. Both patients' condition improved with antibiotic therapy and the Pseudomonas nodules resolved without surgical drainage.

Treatment of central nervous system toxoplasmosis with pyrimethamine/sulfadiazine combination in 35 patients with the acquired immunodeficiency syndrome. Efficacy of long-term continuous therapy.

Thirty-five patients with the acquired immunodeficiency syndrome (AIDS) and central nervous system toxoplasmosis, seen over a 30-month period, were treated with the combination pyrimethamine/sulfadiazine. All patients had clinical and computed tomographic scan findings consistent with active neurotoxoplasmosis. Mean duration of total therapy was six months. During the first two months of therapy, four patients died of acute neurotoxoplasmosis and 31 showed improvement. Of the 24 patients evaluable for long-term therapy, 14 (58 percent) achieved complete resolution and 10 had late clinical (n = 7) and/or computed tomographic scan (n = 6) sequelae. Six patients experienced 10 relapses, which occurred within six weeks of treatment discontinuation in seven of 10. Reintroduction of the combination led to complete resolution of the relapse in eight cases. These clinical results were correlated with brain anatomic findings in the 15 autopsied cases. Side effects, noted in 25 of 35, were mainly hematologic toxicity (n = 21) and cutaneous rash (n = 12). However, the combination had to be definitively stopped in only two cases and sulfadiazine alone had to be withdrawn in eight other cases. These data suggest that pyrimethamine/sulfadiazine is highly efficacious in neurotoxoplasmosis and that life-long therapy is needed to prevent relapses in patients with AIDS.

[Neuromeningeal listeriosis in adults, excluding pregnancy. Prognosis and development of neurologic manifestations. Retrospective study of 63 cases].

Patients' constitutional background, treatment and neurological manifestations were analyzed in a retrospective study of 63 cases of neuromeningeal listeriosis in adults. Age over 60 and coma at the onset were of poor prognosis, but immunodepression (present in only 38% of the cases) did not seem to affect the outcome. The ampicillin-aminoglycoside combination did not appear to improve the vital and functional prognoses more than ampicillin alone. The neurological manifestations observed at the end of hospitalization in 16 of the 42 patients who were cured were not necessarily permanent: among the 13 patients who could be followed up for a mean period of 6.5 years, 5 recovered completely, 5 recovered partially and 3 remained unchanged at neurological examination.

Pasteurella multocida bacteremia: report of thirteen cases over twelve years and review of the literature.

13 episodes of bacteremia caused by Pasteurella multocida were seen in a general hospital during a 12-year period. All the patients had an underlying disease (77% had cirrhosis) and 2 were receiving chemotherapy for hematologic malignancy. There was a numerical preponderance of male patients (69%). In 5/13 cases a recent animal-derived trauma could be found. In the other cases the source of the infecting organism was thought to be endogenous (from patients' own pharyngeal commensal flora) or secondary to contact with secretions of a pet animal. The clinical presentation of sepsis caused by this organism was nonspecific. Hypotension was seen in 5 cases. Localized sites of infection were certain in 6 and only clinically suspected in 4 other cases. The overall mortality rate was 31%. The administration of ampicillin seems the appropriate therapy for Pasteurella multocida bacteremia.

[Infected arterial aneurysms. Anatomo-clinical and surgical aspects apropos of 12 cases].

Twelve cases of mycotic aneurysm are reported. These rare lesions (2 to 3% of all aneurysms) may affect all arteries. Primary aneurysm is currently the most frequent type observed in France, with onset being related to a bacteremia or septicemia with development on an atheromatous lesion (8 of the 12 cases in this series). Manifestations are often misleading or diagnosis delayed in relation to the infectious process, apart from in rupture, which is the commonest cause of detection of aneurysm (half of the present series). Ideal surgical treatment is exclusion with extra-anatomic revascularization: but revascularization in situ has been reported by many authors (6 of the 12 cases in this series). Localization of aneurysm determines surgical procedure and also very variable mortality rate: 4 of the 12 patients in this series.

[Pleuropulmonary disease caused by Pasteurella multocida. Study of 9 cases. Review of the literature].

Nine cases of pleuro-pulmonary infection due to Pasteurella multocida were observed over an 11 year-period (1974-1984) occurring in seven men and two women, with a mean age of 65 (range: 47-80 years). There were 4 pneumonias and 5 cases of empyema, occurring on three occasions after septicemia. There was a background of depressed immunity in 7 cases: alcoholic cirrhosis (4 cases), blood dyscrasias (2 cases), breast cancer (1 case); and of a chronic broncho-pulmonary pathology in two cases. Animal inoculation was present in six cases but only one case of pneumonia followed injury by an animal (cat scratch). The clinical, radiological and epidemiological data of these nine cases were similar to those in the literature (forty-five published cases). There was a zero mortality in our (from 30%) in the literature. Pasteurella multocida is an opportunistic organism, noncommensal in man, producing pulmonary infections in subjects with generalised or localised diminished resistance, the portal of entry being airborne (indirect animal contact) or haematogenous. The organism is nearly always sensitive to Penicillin and other B-lactamines. The gravity of infections to Pasteurella multocida relates to the degree of decompensation or severity of the underlying disorder.

Reliability of the bronchoscopic protected catheter brush in intubated and ventilated patients.

The reliability of a bronchoscopic protected catheter brush (BPCB) in the diagnosis of lower respiratory tract infection was studied in 17 intubated and ventilated patients, including seven patients free from such infection (group 1) and ten patients with suspected infection (group 2). A first sample was obtained in the lower trachea by aspiration through the fiberoptic bronchoscope and a second in a distal bronchus by the BPCB procedure. In group 1, all BPCB cultures were sterile, although lower tracheal cultures yielded two or more bacterial species, showing that uncontaminated specimens can be obtained by the BPCB procedure. In three patients of group 2, BPCB cultures remained sterile as a nonbacterial pulmonary disease was certified by open lung biopsy. In seven patients from group 2, BPCB cultures yielded all of the organisms isolated simultaneously by reference methods (ie, cultures of blood or pleural fluid, serologic tests, and open lung biopsy). In two of these patients, contamination of the BPCB specimens was ascertained by the reference method bacterial results. In this study the BPCB procedure was able to obtain uncontaminated specimens in intubated and ventilated patients and was mainly accurate in identifying the bacterial etiologic agents of lower respiratory tract infections.

[Antibody-dependent cell-mediated cytotoxicity: a method of predicting the severity of fetomaternal alloimmunization in the Rhesus system? A retrospective study of 32 cases].

This retrospective study has as its object to find out whether A.D.C.C. has advantages over other techniques that are usually used, namely titres or levels of antibodies. There is no correlation between A.D.C.C. and the levels or titres of antibodies in the 32 cases that were studied. When the level of bilirubin at birth is taken as the criterion of the severity of the illness, A.D.C.C. is the only method that gives a significant correlation with the degree of severity of the condition of the infant (p = 0.01). If the need for treatment is taken as a criterion of the severity of the condition (ultra-violet light or replacement-transfusion), A.D.C.C. shows up as being the better technique. But if one only looks at very anaemic infants the significance becomes much greater (p = 0.001).

[Peritoneal amyloidosis in Waldenström's macroglobulinemia. X-ray computed tomographic aspects].

A 74 years old man, know to have Waldenström's macroglobulinemia was hospitalized for intestinal obstruction. Upper gastrointestinal barium study revealed an extrinsic stenosis of duodenum. Ultrasonography and abdominal computed tomography revealed a diffuse mesenteric infiltration with medium level echoes and encasing of mesenteric vessels. Histologic examination of biopsies taken at exploratory laparotomy disclosed amyloidosis of the epiploon and mesentery. The review of amyloidosis in macroglobulinemia shows peritoneal amyloidosis to be a very rare event. Sonographic, computed tomographic and pathologic findings of this unusual case are discussed.

[Aphthous hepatic lesions and Behçet syndrome].

The case of a twenty-one-year-old man who was hospitalized for unexpected bilateral phlebitis of the lower limbs is reported. Clinical features suggested a systemic disease, especially Behçet disease although the classical triad was lacking. Laparoscopy showed two lesions identical to canker sores on the hepatic capsule. There were no biological hepatic disorders and histological examination of a needle biopsy specimen was normal. This clinical report is additional evidence in support of the idea that Behçet is syndrome is a systemic disease.

[Simultaneous study of the placental, myometrial and cervical circulations (indium 113 and anemometric thermometry). 2. Effect of xanthinol nicotinate].

The authors have studied the action of nicotinate of xantinol on the placental, myometrial and cervical circulations using an isotope technique (Indium 113) and anemometric thermometry. They show that this product in every case increases the flow through the cervix and has little action on the flow through the placenta, except that in rare cases it lessens it. The action on the myometrial circulation varies from woman to woman. There is a significant negative statistical correlation between variation in the circulation through the cervix and the placenta.

[The Nadis cohort: 6236 HIV-infected patients followed up in French university hospitals].

OBJECTIVE: The Nadis electronic medical patient record allows real time constitution of a database including the clinical, therapeutic, biological, and epidemiological features of HIV-positive patients. METHODS: Data concerning HIV-infected patients followed-up in 6 French University Hospitals was collected. Data quality was assessed on a regular basis in each center. RESULTS: The 6 first University hospitals using Nadis agreed to group their data on March 15, 2004, concerning 6236 patients having consulted at least once in the previous year. Among these, 29% were female patients, 80% were under treatment on March 15, 2004, 9% were off treatment, 29% were co-infected by hepatitis B or C virus, 57% had an undetectable viral load, 15% of the treated patients were in a worrying immunovirological situation, 358 were diagnosed HIV-positive in 2003. 35% of these "new patients" were women, the mode of infection was sexual in 80%, 45% were under treatment on March 15, 04. This recent data allowed us to have an accurate assessment of this population's management in 2004.
