[Surgical evaluation of a liver transplant program. Experiences with the first 45 cases].

The cumulative experience in liver transplantation since the introduction of cyclosporine A has confirmed its efficacy in the treatment of diverse liver diseases in children and adults. In the present study we review the surgical aspects of 45 transplants performed in 39 patients (17 children and 22 adults). Immunosuppressive treatment consisted of cyclosporine A, steroids and monoclonal antibodies (OKT3) in rejection unresponsive to steroids. The actuarial survival rate one year after transplantation was 74%. In the course of development of the program, the elective use of veno-venous bypass during the anhepatic phase in adults was confirmed as useful. Among pediatric transplantations there were numerous arterial malformations (31.5%) in donor organs, which, with the malformations associated in receptors, made allograft reconstruction difficult. Our results confirm those reported by other groups, as well as the importance of specialized training for this type of intervention.

An abundant 56 kD protein with low affinity androgen binding: another member of the steroid/thyroid receptor family?

We have found in genital skin fibroblasts an abundant 56 kD protein which appears related to the androgen receptor. The protein is detected in the soluble fraction by two-dimensional polyacrylamide gel electrophoresis as two spots with isoelectric points of 6.7 and 6.5 respectively. Photoaffinity labelling of GSF with 8 nM or 50 nM [3H]-methyltrienolone selectively labels the two protein spots but with an apparently lower affinity than is known for the androgen receptor. The two protein spots stem from one protein as judged from peptide patterns of partial proteolytic digests and the equal labelling of both spots with methyltrienolone. Cells from subjects with mutant androgen receptors generally lack the 56 kD protein and labelling with methyltrienolone fails, but the protein is not the androgen receptor itself. We propose the hypothesis that the 56 kD protein is synthesized from the same gene as the androgen receptor and that androgen may not be its natural ligand.

[Low-molecular weight heparins in post-surgical recovery].

Low molecular weight heparins are used in human therapy for preventing venous thrombosis during the postoperative period. The monitoring of this therapy uses a measurement of the factor Xla activity. A value of this activity higher than 0.4 IU/ml, suggests a hemorrhage risk. The use of these low molecular weight heparins for the treatment of venous thrombosis is now under investigation. However, the biological monitoring of this treatment has not yet been defined. The anti IIa and anti Xa activities seem to be interesting. In fact the antithrombotic power would be due to the anti IIa and profibrinolytic activities.

Special operative events in the first case of liver grafting after heart transplantation.

Combined heart-liver transplantation is the only therapeutic possibility for patients with familial hypercholesterolemia IIa. A 12-year-old boy with this metabolopathy underwent a double transplant in two steps (different donors). This original alternative was chosen to assure a safer procedure in this critical situation. Details of intraoperative events are described. Cholesterol and LDL decreased to physiologic levels after liver transplantation. The child's condition is optimal 1.5 years after surgery.

Atypical hepatic tuberculosis presenting as obstructive jaundice.

A 78-yr-old female with a short history of malaise, anorexia, mild pyrexia, and obstructive jaundice is presented. A diagnosis of tuberculosis of the liver and gallbladder was established at laparotomy. In spite of anti-tuberculous therapy, the patient died on the 5th postoperative day. Widespread miliary tuberculosis affecting the lung, kidneys, spleen, and mesenteric lymph nodes was observed in postmortem studies, with strikingly severe involvement of the intrahepatic bile ducts. Saccular dilatation of the bile ducts alternating with areas of stenosis and narrowed intrahepatic bile ducts was seen. A correlation of the radiological and hepatic postmortem findings is discussed. The atypical presentation of tuberculosis in present times and the causes of jaundice in atypical cases of hepatic tuberculosis are discussed. The rarity of gallbladder tuberculosis is pointed out.

Influence of heparin and a low molecular weight heparin fraction of the leukocytes in an experimental venous thrombosis model.

A randomized study was performed to observe the effects induced by heparin and a low molecular weight heparin fraction (at different dosages) on the leukocytes of rats in the presence of experimental venous thrombosis. The experimentation was carried out on two series of animals: the first with a ligature of the inferior vena cava inducing the formation of a thrombus, the second without any ligature. The results show that the induction of thrombosis involves: in the blood, an increase of the number leukocytes, principally polymorphonuclear cells; in the thrombi, a significant rise in the total count of leukocytes, here mononuclear cells; the latter number increases with the dosages of the administered drugs.

[Bowenoid basocellular carcinomas (basaliomas with cellular monstrosities].

Marked cellular atypia was detected in 4 out of 2,000 consecutive specimens of basal cell carcinoma. The lesions occurred on the exposed skin of the head of 4 male patients with ages between 49 and 71 years. Besides the general pattern of basal cell carcinoma and cellular atypia there were remarkable differences from lesion to lesion resulting in variable difficulties of differential diagnosis, not only with Bowen's disease (3 cases) but with Darier's metatypical epithelioma as well, either of the mixed or intermediate type (1 case each). As with the other cases reported in the literature, the presence of cell atypia did not correlate with a more aggressive clinical behaviour of the lesions.
