Bile acid profile as early indicator of allograft function during orthotopic liver transplantation.

Orthotopic liver transplantation was performed in 20 pigs. Serum total bile acids (STBA) were determined and their profile compared with standard early function parameters: total bilirubin, aspartate aminotransferase (AST), alanine aminotransferase (ALT) and lactic acid. In phase I, the STBA level was 32.89 +/- 1.29 mumol/l. In phase II, STBA accumulated to 84.46 +/- 15.25 mumol/l (p less than 0.01), followed by hepatic clearance in phase III (63.61 +/- 9.71 mumol/1; NS). Between phase III and 6- and 12-hour samples, STBA decreased progressively, reaching values of 33.63 +/- 7.05 mumol/l at 24 h. AST was elevated in phases I, II, III, and at 6, 12 and 24 h (p less than 0.001), as was ALT (but with insignificant differences). Thus, STBA and their profile appear to be earlier and more specific indicators of early graft function than conventional parameters.

[Intestinal parasitoses in the Galician population].

To evaluate the relative importance of the different intestinal parasites in Galicia, we report the results of the investigation of 6,554 samples of feces in the Microbiology Service of the Hospital General de Galicia (Santiago) from 1983 to 1987. The results indicate that the yearly frequency of parasitism has been reduced from 27.8% of the investigated samples en 1983 to 12.1% in 1987. The mean value for the study period was 16.8% (15.6% simple parasitism and 1.2% multiple parasitism). There were no differences in incidence depending on sex. The enteric parasites most commonly found were G. lamblia, T. trichiura, A. lumbricoides and E. coli.

[Surgical evaluation of a liver transplant program. Experiences with the first 45 cases].

The cumulative experience in liver transplantation since the introduction of cyclosporine A has confirmed its efficacy in the treatment of diverse liver diseases in children and adults. In the present study we review the surgical aspects of 45 transplants performed in 39 patients (17 children and 22 adults). Immunosuppressive treatment consisted of cyclosporine A, steroids and monoclonal antibodies (OKT3) in rejection unresponsive to steroids. The actuarial survival rate one year after transplantation was 74%. In the course of development of the program, the elective use of veno-venous bypass during the anhepatic phase in adults was confirmed as useful. Among pediatric transplantations there were numerous arterial malformations (31.5%) in donor organs, which, with the malformations associated in receptors, made allograft reconstruction difficult. Our results confirm those reported by other groups, as well as the importance of specialized training for this type of intervention.

Pregnancy outcome associated with natural family planning (NFP): scientific basis and experimental design for an international cohort study.

Although natural family planning (NFP) is a form of contraception without ostensible maternal risks (other than pregnancy), potential fetal risks could exist if aging gametes are involved in inadvertent fertilization. In the following report, we first review animal studies firmly establishing that aging sperm and aging oocytes (delayed fertilization) cause chromosomal abnormalities in mammals and other species. We next review human studies associating decreased coital frequency with trisomy and studies of NFP populations that generally show no increased frequency of anomalous offspring or spontaneous abortions. Our rationale for initiating an international cohort study is presented, along with the experimental design selected. Preliminary findings indicate that the experimental design chosen will indeed provide information allowing NFP safety to be assessed definitively.

Effectiveness of enteric-coated preparations on nutritional parameters in cystic fibrosis. A long-term study.

To evaluate the effectiveness of enteric-coated pancreatic enzyme supplements in comparison to conventional preparations of ingested enzyme on growth and nutritional parameters of patients with cystic fibrosis, we conducted a long-term study involving 40 patients. The data reproduced here were recorded after 6 months of therapy with powder-containing capsules or with enteric-coated products. Fat absorption was estimated by measurement of steatorrhoea with the steatocrit method. All parameters studied improved after enteric-coated pancreatic enzyme therapy, with a statistically significant increase in weight, cholesterol and haemoglobin values. Furthermore, the number of patients with positive steatocrit test was lower after therapy with enteric-coated enzyme supplementation. These findings suggest that the enteric-coated product not only reduces steatorrhoea, but above all improves the nutritional parameters and growth of patients affected by cystic fibrosis.

Special operative events in the first case of liver grafting after heart transplantation.

Combined heart-liver transplantation is the only therapeutic possibility for patients with familial hypercholesterolemia IIa. A 12-year-old boy with this metabolopathy underwent a double transplant in two steps (different donors). This original alternative was chosen to assure a safer procedure in this critical situation. Details of intraoperative events are described. Cholesterol and LDL decreased to physiologic levels after liver transplantation. The child's condition is optimal 1.5 years after surgery.

Screening cornea donors for antibodies against human immunodeficiency virus. Efficacy of ELISA testing of cadaveric sera and aqueous humor.

Coded cadaveric sera from 35 patients with acquired immunodeficiency syndrome (AIDS), from 45 cadavers at high risk of human immunodeficiency virus (HIV) infection, and from 262 cadavers without known signs or risk of AIDS were assessed using three commercially available enzyme-linked immunosorbent assays (ELISA) kits and Western blot analysis. Greater than 94% sensitivity and 99% specificity was achieved with each of the ELISA test kits using cadaveric sera. The Western blot method gave 97.1% sensitivity compared with the autopsy-proven diagnosis of AIDS. Positive results were obtained on sera from AIDS cadavers even if the time of blood draw was delayed 35 hours from death and the time of sera preparation was delayed up to 176 days. False-negative or false-positive ELISA results did not appear to correlate with hemolysis or any parameter of sera preparation. In contrast to the high sensitivity in testing sera, only 16 to 26% of aqueous humor samples from AIDS cadavers were ELISA-positive and 79% were positive by Western blot. These results indicate that three commercially available ELISA test kits are an effective means of screening cadaveric sera for antibodies to HIV, but that aqueous humor cannot be reliably substituted for cadaveric sera to screen potential cornea donors by an ELISA assay.

Sympathetic nervous system mediates urinary kallikrein excretion in conscious rats.

1. The influence of the sympathetic nervous system on urinary kallikrein excretion (UKal) was investigated in conscious rats during stress produced by inserting a silastic catheter through the urethra into the bladder. The effect of this stress on blood glucose (BG), mean arterial pressure (MAP), urinary volume (Uv), urinary sodium (UVNa), and glomerular filtration rate (GFR) was also studied. 2. In intact animals stress of 120 min duration produced a non-significant increase of MAP, a significant increase of BG and a decrease of UNa (P less than 0.001 t-test, 9 d.f.), but it did not affect Uv and GFR. In stressed rats UKal was considerably lower (52 milli Amidasic Units [mAU] per 100 g bodyweight, s.e.m. = 5, n = 6) than in control rats (170 mAU per 100 g, s.e.m. = 21, n = 5). The inhibitory effect on UKal was also observed when kallikrein was measured by the kininogenase method. 3. Adrenal medullectomy, performed 1 week before the experiment, suppressed the stress hyperglycaemia but did not affect the reduction of urinary kallikrein or the anti-natriuresis. 4. Intracerebroventricular (i.c.v.) injection of saline also had no effect in the control or in the stressed rats, while i.c.v. d-l-propranolol decreased MAP, suppressed the stress hyperglycaemia and the anti-natriuresis and stimulated UKal, without changes in Uv and GFR. Non-stressed control rats i.c.v. injected with saline excreted considerably less kallikrein than rats i.c.v. injected with d-l-propranolol (control saline: 162, s.e.m. = 14, n = 6; vs control d-l-propranolol: 559, s.e.m. = 20, n = 6). Even in stressed rats this difference was registered (stressed saline: 56, s.e.m. = 8, n = 6; vs stressed d-l-propranolol: 554 mAU per 100 g, s.e.m. = 33, n = 6). 5. Peripheral sympathectomy with 6-hydroxydopamine (6-OHDA) did not suppress the hyperglycaemic response to stress, but it stimulated UKal. Kallikrein excretion was similar in 6-OHDA stressed (534 mAU per 100 g, s.e.m. = 30, n = 6) than in 6-OHDA control rats (491 mAU per 100 g, s.e.m. = 34, n = 6). No differences were observed on UNa and GFR between control 6-OHDA treated rats and stressed 6-OHDA treated rats. 6. The present results suggest strongly that urinary kallikrein excretion is modulated by sympathetic activity. Results after central beta-adrenergic blockade and peripheral sympathectomy led to the hypothesis that normal sympathetic tone in the kidney inhibits the release of kallikrein into the urine.

Biotyping and phage typing of coagulase-negative staphylococci from blood cultures of neonates.

One hundred coagulase-negative staphylococci isolated from blood cultures of neonates have been biotyped and phage typed. These results have been compared with previous reports. The susceptibility of the strains to antimicrobial drugs has also been examined and a rise in resistance rates towards the end of the study was documented. The possible causes of this finding are discussed.
