Upper urinary tract obstruction in children caused by Candida fungus balls.

2 children with upper urinary tract obstruction from Candida fungus balls are reported. A presumptive diagnosis, made on the basis of clinical and radiological findings, was confirmed by microscopic examination of urinary sediment. Medical treatment was successful in both patients. Since Candida infections can occur in patients with altered host resistance, this should alert clinicians to the possibility of fungal involvement when radiolucent filling defects are found in the renal pelvis. Such a presumptive diagnosis can then lead to a correct approach with conservation of renal function.

[Uretero-nephroscopy. Personal experience].

The authors present the personal experience concerning with the use of the rigid ureteropyeloscopes. Two groups of patients have been studied. In the first one (14 subjects) the ureteropyeloscopy was performed either with diagnostic or therapeutic purposes, in the second one, just as upper urinary tract endoscopic control in patients previously submitted to distal ureteral resection and ureteroneocystostomy for urothelial neoplasia of the distal ureter. The authors presenting the personal results and complications, discuss the indications for the upper urinary tract endoscopy with rigid instruments.

Water-path echotomography in differential diagnosis of scrotal masses.

Experience with Octoson water-path echotomography in examining 21 testicular masses is reported. Sonographic results were confirmed by surgical and pathologic findings in all cases. We believe this procedure to be of value in differential diagnosis of intrascrotal swellings.

Five years of experience with a modified technique of ureterocolonic anastomosis.

The thick colonic wall permits the creation of an antirefluxing ureterocolonic anastomosis, which reduces the incidence of pyelonephritis and limits the deterioration of renal function. Since 1977 a simple procedure for ureterocolonic anastomosis performed entirely outside of the colon was used in 63 patients. Obstructive complications were encountered in 6.4 per cent, while ureterocolonic reflux occurred in 5.6 per cent of the renoureteral units. Acute pyelonephritis developed in 7 patients.

Conservative treatment of lower ureteral tumor: modified ureteroneocystostomy for upper urinary tract endoscopic control.

A total of 4 patients with transitional cell carcinoma of the lower ureter underwent distal ureterectomy and ureteroneocystostomy, and were followed by ureterorenoscopy. The ureterovesical anastomosis straightened the remaining ureter, thus, avoiding any curvature or angulation between intravesical and extravesical segments. This result made possible routine endoscopic followup of all patients, since passage of a rigid ureterorenoscope through the ureter was easy without the need for dilation. No reflux was observed after ureterovesical reimplantation and endoscopic control. The results of the modified ureterovesical anastomosis strongly favor a more conservative surgical approach to transitional cell carcinoma of the distal ureter.

Transrectal thin-needle aspiration biopsy of prostate: four years' experience.

Fine-needle aspiration biopsy of the prostate is of great advantage in the diagnosis of prostatic disease. The procedure is quick, safe, and reliable. Several aspirations can be done even in outpatients with minimum trauma. Complications are rare. In this study cytologic diagnosis was obtained in 511 consecutive patients. To evaluate the cytologic accuracy, 195 cytologic diagnoses were compared with histologic findings obtained from transperineal biopsy on the same patients. Correlation was achieved in 96.42 per cent of these cases. The findings in 127 histologically graded prostatic cancers were compared with the cytologic differentiation observed in aspiration smears of the same patients. Cytologic grading of prostatic carcinoma corresponded to the histologic grading in 85.8 per cent of the cases.

The evolution of renal function after surgical treatment of malignant lithiasis with severely compromised renal function.

16 patients with calcic nephrolithiasis and renal failure were admitted to our institute and underwent surgery. 4 patients had bilateral nephrolithiasis and 12 cases had relapsed. Selective nephrotomy was performed in all cases, pyelotomy was done in 8 and partial nephrectomy in 2 patients. In 8 patients renal function improved but no difference was observed in 6 patients. Renal function worsened in 2 patients who had shown significant preoperative proteinuria. According to our experience, the success of this therapy depends on: (1) accurate patient selection; (2) adequate preoperative treatment, and (3) personalization of surgical management.

Prevention of calcium nephrolithiasis with low-dose thiazide, amiloride and allopurinol.

We report 5 years' experience with low-dose hydrochlorothiazide, 50 mg/day and amiloride, 5 mg/day, in 519 patients with recurrent calcium nephrolithiasis. Additional treatment with allopurinol, 100 mg/day was prescribed for approximately 50 percent of the patients. All patients had active stone formation, having 3,464 stones in 3,126 patient-years (6.67 stones per patient, 1.10 stones per year). Hypercalciuria was present in 65 percent of the patients and hyperuricosuria in 24 percent. The administration of low-dose hydrochlorothiazide was effective in reducing urinary calcium excretion in most patients. It is possible that the hypocalciuric effect of hydrochlorothiazide were enhanced by amiloride, an agent which has been shown to cause hypocalciuria when given alone. Significant side effects requiring discontinuation of the drug were observed in only 5 percent of the patients. During 872.8 patient-years of treatment, only 53 new stones were formed (0.10 stones per patient, 0.06 stones per year) in contrast with the 916 predicted ones. The difference (chi-square) is statistically significant (p less than 0.001). These results show that the administration of low-dose hydrochlorothiazide and amiloride, either alone or in association with allopurinol, is clinically effective in reducing the rate of recurrence of calcium nephrolithiasis.

Elastin and collagen in the normal and obstructed urinary bladder.

Histological and chemical studies were carried out on the elastin and collagen content of the normal and the obstructed urinary bladder wall. The observations were made on 3 different age groups: (1) 3-month-old children with vesical outlet obstruction, (2) children aged between 4 and 8 years with partial vesical obstruction, and (3) adults with a chronically obstructed bladder. An increase in elastic tissue was shown only in the newborn and in the adults. The collagen did not vary in any of the cases studied. The elastin showed an increase in polar amino acids in the pathological organs compared with the normal. A similar increase in elastin was observed in normal individuals as a result of the ageing process.

Cystinuria in children.

7 children, 20 months to 11 years old, with cystinuria and renal calculi were studied. Surgical treatment and alpha-mercaptopropionylglycine (MPG) gave satisfactory results in 5 children. The causes of the recurrences in the other 2 children are discussed. MPG therapy is effective but can cause a nephrotic syndrome at a dose of more than 50 mg/kg/day. A cystinuria of less than 200 mg/day cannot always be considered safe in children. The alkalinization and dilution of urine remain extremely important in the treatment of cystinuria.

Ureterocolonic anastomosis: description of a technique.

A simple procedure is presented for ureterocolonic anastomosis performed outside the colon. This technique provides excellent exposure for creation of a long tunnel, avoiding any angulation or constriction, and allows an easy ureterocolonic end-to-side anastomosis.

The elastic tissue of the adult dilated ureter.

Adult dilated ureters have been examined in order to calculate the amount of the elastic tissue. From the histological observations and by the chemical analyses performed on the extracted elastin a decrease of the elastic tissue, as compared to the normal adult ureters has been noted. In fact, in the adult normal ureters, the elastin accounts for 9% of the dry matter, while in the abnormal the elastin represents 6.5%.

The elastic component of normal and dilated ureters in children: chemical and histochemical characterisation.

Using chemical and histochemical techniques, it has been possible to calculate the exact amount and distribution of the elastin in normal and abnormal ureters in children. From the quantitative analyses, it has been calculated that in the dilated ureters the elastin content represents the 3% to 5% of the organic material, while in the normal ureters it is less than 1%.
