Serum lipid pattern in urban and rural Thai population.

Serum cholesterol, triglyceride, phospholipid, total lipid and weight to height ratio were studied in 172 individuals from Bangkok, mainly university staff, and in 247 individuals from rural areas of northeast Thailand. Total lipid and phospholipid levels were statistically significant higher and triglyceride lower in the Bangkok group compared with the group from the rural areas. A cholesterol level did not differ significantly between the groups. The results are discussed in respect of a possible difference in the nutritional status between the individuals from the urban and rural area.

Serum proteinase inhibitors and other serum proteins in protein-energy malnutrition.

1. The concentrations of serum protein albumin, prealbumin and transferrin were determined in twenty-eight cases of protein-energy malnutrition (PEM) with infection, together with the levels of serum proteinase inhibitors (PI), alpha1-antitrypsin (AT), alpha1-antichymotrypsin (Ach), alpha2-macroglobulin (alpha2M) and inter-alpha-trypsin inhibitor (IalphaI). 2. Albumin, prealbumin and transferrin concentrations, as well as the levels of PI, IalphaI and alpha2M were found to be lower in cases of PEM associated with infection than the corresponding values for a group of healthy Thai preschool children and a group of newborn Thai children, but despite starvation AT and Ach values generally were increased. 3. The results provide support for the hypothesis that PI, especially AT and Ach might limit the synthesis of albumin, prealbumin and transferrin in PEM associated with infection, via the inhibition of the mobilization of body's own protein.

Serum uric acid level of Thai individuals in comparison with the nutritional status and some other physical and biochemical parameters.

Serum uric acid levels were measured in a total of 287 adults of Thai origin in Bangkok and the rural area. Mean values and range were found to exceed slightly serum uric acid concentration as reported from western countries. There was no significant difference between the Bangkok group and the group from the rural area but males and females differed significantly in their serum uric acid levels. A significant positive correlation to the ratio between weight to height as an index of the nutritional status was found.

Alterations of human serum proteins and other biochemical parameters after five to ten days of untreated acute falciparum malaria.

Untreated malaria for more than 4 days in eleven patients decreased significantly prealbumin, transferrin levels and increased SGOT activity when compared with a control group and a group of 10 malaria patients who were admitted to the hospital at an earlier stage of the infection. Total protein was significantly lower in the group of patients admitted after five to ten days to hospital compared with the control group. In all malaria patients independent of the duration of the acute infection the 1st post albumin peak in polyacrylamide gel electrophoresis (consisting mainly of Gc-globulin, alpha-1-antichymotrypsin and alpha-1 B-glycoprotein) and creatinine were found to be significantly higher compared with the control group.

The urinary sulfur/creatinine ratio in the assessment of protein nutritional status.

The urinary sulfur/creatinine ratio and urea nitrogen/creatinine ratio of Thai soldiers and schoolchildren from Nakhon Nayok, 100 km northeast of Bangkok, the Lam-takong resettlement area and the Kohn Kaen resettlement and irrigation area northeast of Thailand were determined. As reference group healthy children from the University school in Khon Kaen were selected. The urinary urea N/creatinine ratio was significantly lower for the children from Nakhon Nayok, the Lam-takong resettlement and the Khon Kaen resettlement and irrigation area when compared with the reference group. The ratio values of the soldiers was significantly lower than the ratio of every group of children. The urinary sulfur/creatinine ratio for the children in Nakhon Nayok was lower and for the children in the Lam-takong and Khon Kaen resettlements were significantly lower than the ratio of the reference group. For the reference group the normal range for urea N/creatinine ratio was between 4.8 and 14.5 and for sulfur/creatinine ratio between 0.40 and 1.40.

Serum protein fractions from children of differing nutritional status analysed by polyacrylamide gel electrophoresis and electroimmunoassay.

1. The nutritional status of children showing no clinical signs of malnutrition, from the University School of Khon Kaen, Khon Kaen province, north-east Thailand and from two villages nearby, was tested. The children were grouped according to their body-weight expressed as a percentage of expected weight-for-height (Harvard standards (Stuart & Stevenson, 1959), as given by Jelliffe (1966)). 2. The differing prealbumin concentrations indicated that nutritional status differed between the groups. 3. The urinary urea: creatinine ratio was significantly lower in the village children compared with the children from Khon Kaen, indicative of the higher dietary protein intake of the latter. 4. alpha1-Acid glycoprotein and the first 'post-albumin peak' (obtained by polyacrylamide gel electrophoresis of serum and containing mainly mainly Gc-globulin, alpha1-antichymotrypsin and alpha1-B-glycoprotein) were found to be significantly higher in the village children compared with children from Khon Kaen. 5. The three main proteins of the first 'post-albumin peak' from polyacrylamide gel electrophoresis of serum were tested separately using the electroimmunoassay method. There was no significant difference in Gc-globulin between the children from Khon Kaen and the village children. The concentration of alpha1-B-glycoprotein from those Khon Kaen children whose body-weight was more than 95% expected weight-for-height was significantly lower compared with that of village children, alpha1-Antichymotrypsin concentration was significantly higher in serum from Khon Kaen children than in serum from village children.

The vitamin B1, B2 and B6 status of school children in two resettlement areas in Northeast Thailand.

The vitamin B1, B2 and B6 status of school children were studied in the Khon Kaen resettlement and irrigation area and in the Lam-takong settlement in northease Thailand using the in vitro tests based on the erythrocyte transketolase, glutathione reductase and glutamate-oxaloacetate transaminase activities. Healthy school children from the Khon Kaen University School were selected as a reference group. The results demonstrate that the vitamin B1 status in the resettlement area is sufficient, but about 20% to 35% of all the children show evidence of vitamin B2 and B6 deficiencies. It is noticed that besides a high prevalence of parasitic infections and poor hygienic conditions the protein and vitamin intake is low and improvement of the situation is necessary.

Some human "carbohydrate-rich" serum proteins in protein-energy malnutrition.

The human serum proteins with a carbohydrate portion of more than 10 per cent in the molecule, alpha1-acid glycoprotein, alpha1-antichymotrypsin, alpha1B-glycoprotein and alpha2HS-glycoprotein were studied in thirteen cases of protein-energy malnutrition. The "acute phase reactant" proteins alpha1-acid glycoprotein and alpha1-antichymotrypsin were found to be increased above the controls, whereas the other "glycoproteins" either decreased or did not change significantly. Also albumin, prealbumin and transferrin were found to be low in the cases of protein-energy malnutrition.

Serum proteins and nutritional status of free-living Thai elderly.

Fifty-six Thai males and 146 Thai females aged 60 years and above visiting a special clinic for the elderly were investigated. The serum protein and immunoglobulin of these elderly were assessed. Anthropometric measurements were also taken. From a random sample of the group, the dietary intake of main nutrients was determined by a 24 h recall method with the help of food models. Weight, height and mid-arm muscle circumference of males were greater than those of females. Triceps skinfold thickness was less in the males than in the females. Forty-seven per cent of the individuals investigated had alpha1-antitrypsin levels below 200 mg/dl and 25.4% had IgG levels of 200 mg/dl and 39% IgA levels above 450 mg/dl. The males tend to consume more calories in the form of carbohydrates than the females. The nutritional density of fat was less than 20% in the males. A negative correlation between serum albumin and height and a positive correlation between serum IgG and tricep skinfold thickness were found. Weight was found to have a positive correlation with calories, carbohydrates and fat. Height correlated positively with calories, protein and carbohydrates. Serum prealbumin correlated with fat intake. It is concluded that, except for alpha1-antitrypsin, IgA, serum proteins are found in a range normally detected also in elderlies in western countries. Body composition does not relate to level of protein intake.

A self-administered device for cervical cancer screening in northeast Thailand.

OBJECTIVE: To test a self-scraping device as a mass screening device against a routine scraping method and to evaluate the acceptance of the self-scraping device by a group of rural females from Northeast Thailand. STUDY DESIGN: From a rural area of Northeast Thailand, 552 women were trained and motivated, through primary health care structures, to participate in a cervical cancer screening exercise using a self-scraping device. After one week, the same females were reexamined by gynecologists using the routine scraping method. In both cases the specimens were stained according to Papanicolaou. Through questionnaires the acceptance of the self-scraping device was evaluated. RESULTS: Through the self-scraping method, 13 cases suspicious for malignancy were detected. Specimens obtained through examination by physicians confirmed 11 cases to be suspicious for malignancy. No false negative cases were found. In the detection of inflammation, the self-scraping method was not as accurate as examination by a physician. The device was accepted by the females who participated in the study. CONCLUSION: In the rural areas of developing countries, where physicians and other trained medical personnel are not often available to carry out regular screening tests on a population basis, the self-scraping method can be applied as an integral part of primary health care for mass screening for uterine cervical cancer.
