Mass treatment of opisthorchiasis in Northeast Thailand.

In an attempt to control opisthorchiasis, a single dose of 40 mg/kg of Praziquantel was given to 666 people in the three villages of Nam Pong Water Resource Development Project, Khon Kaen, Northeast Thailand. The results showed a cure rate after 1 month was 95.9%. The side effect was minimized by alternation of the time of treatment from after breakfast to after dinner. The use of Praziquantel for the mass treatment in control of opisthorchiasis is possible. However, to achieve the objective of control programme other measures such as environmental sanitation improvement, health education and change in eating habits must be integrated into the programme.

Proteinase inhibitors and other biochemical criteria in infants and primary schoolchildren from urban and rural environments.

1. Preschool children and schoolchildren from a rural area in the northeast of Thailand were compared with children from urban areas for prealbumin, albumin, transferrin, alpha 1 B-glucoprotein, the acute-phase reactants alpha 1-acid glycoprotein, alpha 1-antichymotrypsin, haptoglobin and the proteinase inhibitors alpha 1-protease inhibitor (A1PI) as well as alpha 2-microglobulin (alpha 2M). Urinary urea nitrogen:creatinine (U-C) as well as haemoglobin were also determined. Village preschool children were grouped according to their SD score for weight-for-height based on the (US) National Center for Health Statistics (1976) standard into a normal group with SD scores of greater than or equal to --1.99 and an undernourished group with SD scores of less than or equal to --2.00. 2. There was no significant difference between the normal and the undernourished groups of preschool children for any of the factors measured. 3. Haemoglobin and prealbumin concentrations of preschool children were lower in the rural children than in the urban preschool children from Bangkok. 4. In rural schoolchildren haemoglobin was lower but albumin, transferrin, alpha 1B-glycoprotein and haptoglobin were higher than in urban schoolchildren from the provincial town of Khon Kaen. 5. Serum concentrations of the proteinase inhibitors A1PI and alpha 2M were significantly higher in the rural children than in the urban children. 6. U-C values were lower in rural schoolchildren compared with urban schoolchildren.

Alcohol consumption, liver function tests and nutritional status in Thai males.

31 healthy Thai males, 22 Thai male regular drinkers not suffering from any clinical signs or symptoms of alcoholism, and 52 patients from a neurological hospital in Bangkok suffering from the effects of chronic alcohol consumption were investigated. Alcohol consumption in asymptomatic drinkers ranged from 7 to 134 (median 44) g/d ethanol, and for the patients 22 to 517 (median 197) g/d ethanol, as assessed by questionnaires. The symptomatic alcohol drinkers had consumed alcohol for 2 to 35 years and the hospitalized patients for 5 to 40 years. Only the median levels of serum triglycerides and serum glutamyl transferase (gamma-G) were significantly increased and vitamin B1 deficiency was found with higher frequency in the group of alcohol drinkers without clinical signs compared with the healthy non-alcohol drinkers. Statistically significant correlations were demonstrated in the group of asymptomatic alcohol drinkers only, between alcohol consumption and the Quetelet's index, gamma-G, and alkaline phosphatase levels. Alkaline phosphatase also correlated significantly with gamma-G. In the group of hospitalized patients, compared with healthy males statistically significantly higher median values of systolic and diastolic blood pressure, serum triglyceride, gamma-G, aspartate aminotransferase (GOT), alanine aminotransferase (GPT), alkaline phosphatase, haemoglobin, hematocrit, folate and total protein were found. The median levels of cholesterol, bilirubin, vitamin B2, B6 and B12 in the hospitalized group were lower than, but not significantly different from the other two groups.

alpha-1-Protease inhibitor phenotypes and serum concentrations in Thailand.

alpha-1-Protease inhibitor (A1PI) Pi phenotypes were determined in 429 individuals from Bangkok and 423 from the rural area of the northeast of Thailand by isoelectric focusing on thin-layer polyacrylamide gel. Variation was more marked in the Bangkok population. In the rural area, with an exclusively northeastern Thai population the Pi-alleles M, S and Z were detected. A1PI serum concentrations measured by electroimmunoassay were slightly higher in Thai than the values reported from European countries; children with PiM from rural areas had slightly higher A1PI serum concentrations than PiM children from Bangkok.

alpha 2HS-glycoprotein serum levels in protein-energy malnutrition.

1. alpha 2HS-glycoprotein accumulates in bone and dentine and its plasma levels could vary in conditions in which the rate of bone formation is altered. 2. The plasma concentration of this protein was found to be lower in thirteen children suffering from protein-energy malnutrition compared with age-matched healthy preschool children. 3. This finding might be associated with the phenomenon of stunting in protein-energy malnutrition.

The effect of riboflavin on the hematologic changes in iron supplementation of schoolchildren.

A supplementation trial was carried out in 101 children, 6-12 years of age, in 3 primary schools in a rural area. Their hemoglobin level and PCV (mean +/- SD) were 11.64 +/- 1.21 g/dl and 0.356 +/- 0.028 respectively, 74% of them were anemic and the hemoglobin level were correlated with the MCHC (P < .01) . Fifty-one per cent of them had hookworm infection and all those with hemoglobin levels below 10 g/dl had hookworm infection, but there was no difference in mean hemoglobin level between those with hookworm infection and those without. The children were divided into 3 groups: Group I comprising 39 children who received placebo tablest; Group II of 33, who received ferrous sulphate (60 mg elemental iron); Group III of 29, who received ferrous sulphate (60 mg elemental iron) with riboflavin (6mg). Each child received one tablet after lunch on schooldays and evaluation was carried out after receiving 80 to 90 tablets. The mean hemoglobin change of Group II was 0.60 g/dl larger than that of Group I (P < .005) with 52% of them responding to iron. The mean hemoglobin change of Group III was 0.38 g/dl larger than that of Group II (P < .005) with 86% of them responding to iron and riboflavin. Thus additional riboflavin is beneficial in iron supplementation.

The serum protein pattern in primary hepatoma and amoebic liver abscess.

A study of the serum proteins pattern of 30 patients with primary liver cell carcinoma and 11 with amoebic liver abscess was carried out. When compared with controls significant differences were found for both conditions in the values of pre-albumin, transferrin, albumin, haptoglobin, alpha 2-macroglobulin, and alpha 2HS-glycoprotein. In the differential diagnosis of amoebic liver abscess and primary hepatic carcinoma, the estimation of albumin, alpha 1-acid glycoprotein, haptoglobin ceruloplasmin, alpha 2H2-glycoprotein and transferrin was found helpful.

Are proteinase inhibitors a factor for the derangement of homoeostasis in protein-energy malnutrition?

In protein-energy malnutrition essential amino acids for the synthesis of proteins are derived from the mobilization of endogenous proteins in order to maintain homoeostasis. Data are presented to support the hypothesis that proteinase inhibitors, which increase rapidly at the onset of infection, interrupt this catabolic effect which is the normal adaptive response to protein-energy malnutrition.
