Treatment of senile dementia and cerebellar disorders with phthalazinol. Cyclic AMP-increasing agent, phthalazinol, in therapeutic trials in hitherto incurable morbid conditions (I).

Three patients suffering from presenile dementia and two patients suffering from senile dementia were treated with phthalazinol. A limited but definite improvement in attentiveness, vocabulary reception and psychologic attainment and also a limited but definite improvement in range of motion, gait and endurance were noted shortly after the treatment with phthalazinol and the progression of their dementia seemed to be temporarily retarded by phthalazinol. Seven patients suffering from parenchymatous cerebellar degeneration (late cortico-cerebellar atrophy) and four patients suffering from olivopontocerebellar atrophy were treated with phthalazinol. A relatively rapid, limited and sustained improvement of cerebellar functions, including those of speaking, writing and walking, was noted in almost all patients. Also, rigidity, akinetic tendency and abnormal posture seen in olivopontocerebellar atrophy fairly well responded to the treatment. Dysphagia, disturbance of micturition, and hypersecretion of saliva have also rapidly and completely disappeared in all cases. The cases with a long history responded relatively poorly, but the cases with a relatively short history responded quite strikingly to the therapy. No side effects were noted.

Importance of fluorescein angiographic study in evaluating early retinal changes in Takayasu disease.

PURPOSE: To determine the usefulness of fluorescein angiography in studying Takayasu disease. METHODS: We examined 31 eyes in 16 patients with Takayasu disease using indirect ophthalmoscopy, color photography, and fluorescein angiography. Ophthalmoscopic and fluorescein angiographic findings were compared. RESULTS: Fluorescein angiography revealed no additional retinal changes in 10 eyes that had no retinal vein dilatation as seen by indirect ophthalmoscopy. Seven (33%) of 21 eyes that had dilated retinal veins also had additional abnormal findings, such as microaneurysms, arteriovenous shunts, retinal neovascularization, and avascular areas. Some differences in grading the stages of retinopathy were noted with these newly found retinal changes, as compared with the classifications determined by ophthalmoscopy alone. CONCLUSIONS: In Takayasu disease, studying the fundus of patients with fluorescein angiography is particularly important in correctly classifying the stages of retinopathy when the retinal vein appears dilated in ophthalmoscopic observation.

Quantitative assessment of diastolic filling function: a new Doppler echocardiographic method and in vitro validation.

The effects of changes in preload and chamber compliance on the transmitral flow pattern was investigated with a fluid dynamics model. A decrease in compliance led to a steepening of the deceleration slope and a shortening of the pressure half-time in both restricted and nonrestricted valves. An increase in the preload led to a longer pressure half-time but did not affect the slope in the restricted valve. However, increased preload led to a shorter pressure half-time and a steeper slope in the nonrestricted valve. A mathematically derived method to measure the net atrioventricular compliance (-[annular area] va/[pvt [flow deceleration rate]]), where va and vt are flow velocities at the mitral anulus and the mitral tip levels) was then proposed. The values obtained by this method showed a strong correlation with true values (r2 = 0.89). A possibility of noninvasive quantitative assessment of diastolic filling function was implied.
