[Cumulative effects of lentinan and endocrine therapy on the growth of DMBA-induced mammary tumor in rats].

The cumulative effects of Lentinan and endocrine therapy on the growth of DMBA (7, 12-dimethyl benzanthracene)-induced mammary tumors of rats were studied. Multiple injection of Lentinan alone resulted in a slight degree of regression of the tumor growth, when administered to rats bearing mammary tumors of about 1 cm in size. Ovariectomy-adrenalectomy, ovariectomy, and adrenalectomy, which are performed as surgical endocrine therapy, resulted in a more marked regression of the tumor than that produced by Lentinan treatment alone. Furthermore, multiple injection of Lentinan performed on these mammary tumor-bearing rats which had received surgical endocrine therapy 2 weeks previously evoked a marked regression of tumor growth. However, no changes in growth curves and survival rates, compared with those of saline controls were observed, indicating that Lentinan might be a useful agent when combined with surgical endocrine therapy. Concurrent injection of Lentinan resulted in a slight augmentation, although the histamine sensitivity of tumor bearing rats which had previously received surgical endocrine therapy elevated greatly, compared with that of controls. By contrast administration of Tamoxifen, which is used for medical endocrine therapy, resulted in a lesser degree of regression than that observed following surgical endocrine therapy, and was also not greatly affected by Lentinan injection.

Regional left ventricular wall motion abnormalities in chronic volume overload.

Computer-assisted analysis of left ventricular (LV) wall motion has been performed in 12 patients (pts) with chronic volume overload and in seven normal subjects. All pts were in sinus rhythm, without congestive heart failure and without coronary artery disease at selective coronary angiography; 30 degrees right anterior oblique ventriculograms were employed in each case. Thirty-six radiants were automatically traced and the percent shortening of each radiant was calculated using two different reference methods (fix and floating methods). In valvular heart disease (VHD) pts LV volumes were significantly increased, and ejection fraction and LV eccentricity were significantly reduced. LV eccentricity inversely correlated with LV volumes. VHD pts showed regional LV abnormalities. Inferoapical and anterior wall motion was significantly reduced (two-sample t-test P less than 0.05) using the fix method. With this method a significant inverse correlation between percent shortening of the radiants correspondent to the inferoapical region and end-diastolic and end-systolic volumes was found; a significant direct correlation was found with LV systolic eccentricity. Ejection fraction directly correlated with the percent shortening of radiants correspondent to both the anterior and inferior regions. The role of LV "dynamic geometry" alterations as a possible cause for the observed regional LV wall motion abnormalities is emphasized.

[Experimental and clinical effect of hypertransfusion and OK-432 on granulocyte recovery].

The experimental and clinical studies were carried out to alleviate the bone marrow suppression by antineoplastic drugs. Faster recovery of granulopoiesis was observed by pretreating recipients with RBC hypertransfusion and/or OK-432 (Picibanil, biological products of beta-streptococci.) In experimental mice, higher granulocyte counts could be maintained with hypertransfusion in the peripheral blood, and the recovery of granulopoietic series and CFU-S of bone marrow cells were found to be more rapid after cyclophosphamide administration. OK-432 also resulted in higher peripheral granulocyte, whereas the total nucleated cell counts and CFU-S were decreased in the bone marrow, suggesting sparing bone marrow granulocyte reserve and its migration to the peripheral blood. The mechanism of higher granulocyte count after hypertransfusion was not clearly explained but it was considered that erythroid suppression caused colateral flow of multipotential stem cells to granulopoiesis. The effect of both combinations was unexpectedly less significant in the recovery of granulopoiesis, but it was thought that the optimal time interval should be sought between pretreatment and the administration of anti-neoplastic agents. The clinical use of hyper transfusion and OK-432 also proved the alleviation of granulocytopenia, and rapid granulocyte recovery at the time of consolidation therapy among children with AML.

[Clinical evaluation of transcutaneous oxygen tension measurement for lower limb ischemia].

Transcutaneous oxygen tension (tcPO2) of superficial layer of the foot at 44 degrees C was studied preoperatively in 33 patients (38 legs) with arteriosclerosis (ASO) or thromboangiitis obliterans (TAO). The cases were divided into two groups. The first group consisted of 3 legs, amputated. The second group consisted of 35 legs, not amputated but receiving such treatments as arterial bypass, lumbar sympathectomy or conservative therapy. The tcPO2 index was calculated by dividing the tcPO2 of the chest wall by the tcPO2 of the foot. The value of the tcPO2 of the chest decreased in proportion to the advance of age. There was no significant differences in the tcPO2 indices between ASO and TAO, nor among different sites of peripheral vascular occlusion. On the other hand, as the grade of Fontaine classification increased, the tcPO2 index reduced, with significant difference between grade II and IV. The tcPO2 index of the amputated group was 0.08 +/- 0.04. In contrast, the tcPO2 indices of the lumbar sympathectomy, arterial bypass and conservative therapy groups were 0.74 +/- 0.18, 0.84 +/- 0.12 and 0.74 +/- 0.13, respectively. There was significant difference between the amputated and non-amputated groups. From our results, it is suggested that oxygen tension measured transcutaneously is a useful indicator for the decision of operative indication and surgical procedure.

[Corrected transposition of the great vessels associated with severe pulmonary stenosis in a 59-year-old man].

The case of a 59-year-old man with corrected transposition of the great arteries (CTGA) is reported. This anomaly is rare in adult patients, uncommon in patients over 50. Most of the cases with CTGA are diagnosed in childhood because of associated cardiac abnormalities. When isolated or complicated by other mild anomalies, CTGA can cause no symptoms until adult life or can even go unrecognized. However, most of these patients present with congestive heart failure when they are 40 or 50 years old. Our patient was admitted to the hospital because of heart failure. Cardiac catheterization and selective angiography demonstrated a CTGA associated with severe pulmonary valve stenosis. The case here reported shows that CTGA even when associated with other cardiac abnormalities, can be a relatively benign lesion, and can be revealed in adult life by signs of cardiac failure.

Acquisition of graft-versus-host reactivity by immature thymocytes in the coexistence of activated macrophages.

The collaborative role of macrophages in the acquisition of the rat graft-versus-host (GVH) reactivity by parental thymocytes was investigated by the popliteal lymph node enlargement assay. The addition of peritoneal macrophages activated by streptococcal bacteria injection resulted in significantly increased GVH reactivity of parental thymocytes, when assayed both in normal F1 hybrid hosts and in those depleted of effective macrophages and lymphocytes. The thymocytes that acquire that alloantigen reactivity in the presence of activated macrophages were shown to be cortisone sensitive and of high density. Hence, the cortisone-resistant and less dense population of thymocytes did not exhibit further reactivity even in the presence of macrophages; however, this subpopulation showed an enriched activity on its own. Moreover, the simultaneous injection of activated macrophages induced a further augmenting effect on the graft-versus-host reaction (GVHR) of freshly peripheralized thymocytes, but did not affect the reactivity of lymph lymphocytes from parental rats thymectomized 15 months previously. These data suggest that activated macrophages play an essential role in the acquisition of alloantigen reactivity by immature thymocytes, and also that the macrophage-dependent thymocytes in the reaction may reside among the subpopulation that is cortisone sensitive, highly dense, short-lived, and freshly peripheralized. The possible nature of immunological maturation of immature thymocytes in the periphery is discussed in relation to cellular interactions with activated macrophages.

Effect of dextran infusion on left atrial size in normal subjects.

The purpose of this study was to evaluate left atrium dimension during acute volume loading. Low molecular weight dextran solution (1,000 ml) was continuously infused in 10 subjects without cardiac disease. Measurements of pulmonary arterial, capillary wedge, right atrial and systemic blood pressures, cardiac output, heart rate and M-mode left atrial maximum diameter were made in basal conditions and after 250, 500, 750 and 1,000 ml of dextran. In 4 subjects a two-dimensional echocardiogram was also obtained in the short axis and four-chamber views. Dextran produced the following changes (mean values +/- standard deviation) in haemodynamic variables: right atrial and wedge pressures increased from 3.2 +/- 1.55 to 12.3 +/- 3.08 mm Hg (p less than 0.001) and from 6.9 +/- 1.45 to 20.6 +/- 3.44 mm Hg (p less than 0.001), respectively. Stroke volume increased from 70.5 +/- 31.6 to 86.6 +/- 24.6 ml/beat (p less than 0.05). Heart rate increased from 81.0 +/- 15.8 to 95.0 +/- 12.3 beats/min (p less than 0.05). Left atrial maximum diameter showed a slight increase after 500 ml of dextran (from 39.5 +/- 4.1 to 43.6 +/- 5.0 mm, p less than 0.05). It did not further increase by continuing the infusion up to 1,000 ml despite the marked increase in wedge pressure. The results show that left atrial size is partly independent of haemodynamic changes and that the compliance of the left atrium is relatively low. This finding is consistent with the common clinical observation that the left atrium is only slightly enlarged in acute mitral insufficiency.

Systolic time intervals during isometric exercise in supine position should be corrected also for arterial pressure.

In this paper we have studied the relationship between the systolic time intervals (STI) and the heart rate (HR) and systolic arterial pressure (SAP) in a group of 24 healthy subjects at three different effort levels during an isometric exercise in supine position and in basal conditions. We observed a decrease of the non corrected STI values during the exercise. Then we corrected STI using the regression equations obtained by plotting STI values versus the corresponding HR values at rest and during the exercise. We showed an increase of corrected left ventricular ejection time (LVET) during the whole exercise, and an increase of corrected total electromechanical systole (QS2c) at 50% effort level and then a return to the basal values. We also derived the multiple regression equations correlating QS2 and LVET with HR and SAP. Notwithstanding these equations show a significant positive correlation between the two STI considered and SAP only at the maximum effort, we corrected LVET and QS2 values by these equations assuming that the relationship between these parameters exists also in basal conditions and during the remaining stages of the exercise. By this correction we found a decrease of LVETc and QS2c in accordance with hemodynamic data reported by other authors. We may conclude that it seems useful to correct the STI obtained during isometric exercise in supine position both for HR and SAP.

STI usefulness during isometric exercise in labile hypertensive patients.

Measurement of the left ventricular systolic time intervals (STI) was considered a valid method in demonstrating the presence of a state of beta-drenergic hyperstimulation. The authors used this method during isometric exercise to differentiate between a group of 16 normal subjects (N) and 16 random labile hypertensives (LH), and between the N and 16 fixed hypertensives (FH). Exercise resulted in a general shortening (p less than 0.01) of the STI, an increase (p less than 0.01) in the systolic arterial pressure (SAP), and an increase (p less than 0.01) in the heart rate (HR) in all the groups studied. The study also demonstrated that the state of blood pressure (diagnosis) alone does not significantly influence the course of the exercise test, and that the interaction diagnosis-exercise gives quantitatively different results in the three groups, to the extent that it is possible to recognize a specific polygraphic pattern for the LH. In this group there is a greater shortening of the Q--S2 and the non-corrected LVET (p less than 0.01) than for the N. The behaviour for the FH falls between the other groups, but it is not possible to distinguish clearly between this group and its neighbour on either side. We conclude that the measurement of the STI during isometric exercise could be useful in determining a diagnostic pattern for subjects who show a lability in their blood pressure.
