From Herophilus, Aristotle in the 3rd century BC, Aselli, Pecquet, Mascagni to Jossifow and Rouviere the lymphatic system was investigated. Kinmonth and Wallace were the first to study it with lymphangiography. Mediastinal lymph nodes were poorly visualized in high contrast radiography before the seventies; subsequently with the high voltage technique, xerography and signs from mediastinal lines they were better identified. However these procedures were replaced by CT, with which even lymph nodes less than 0.5 cm in size, are recognized. The differentiation between normal and pathologic lymph nodes is based on dimensional, morphologic and densitometric criteria. CT is able to identify typical patterns of inflammatory, systemic and metastatic lymph nodes. On lymphography a great number of signs as gross and dense dotting, foaming, chipping and lacunae are identified, which allow the differentiation of inflammatory, systemic and metastatic patterns. On sonography some nodal characteristics are evidenced as the round shape, hypoechogenicity, absence of hyperechoic medullary line. CT has replaced lymphangiography in the study of abdominal lymph nodes.

In 34 patients congestive, hemorrhagic lesions were detected by endoscopy. It was considered that trauma was not responsible for these lesions. In all of the patients except four, histology showed no inflammatory reaction. Most of the patients were hypochlorhydric and one was achlorhydric. Serum gastrin levels appeared within a normal range under fasting and stimulated conditions. Twenty-four patients were followed up two years later. Only 1 patient had developed a duodenal ulcer. The endoscopic picture appeared unmodified in the reendoscopized patients. It is concluded that the hemorrhagic lesions of the duodenum may result from local vascular disorders without any inflammation.
We present here a study of parietal cell mass, stimulated acid secretion and basal gastrinemia both in the course of isolated chronic atrophic gastritis of the body-fundus and in chronic atrophic gastritis of the body-fundus associated with pernicious anemia. Analysis of our results evidences an overlapping cyto-secretory profile characterized by hypoparietalism with hypo-achlorhydria. The higher basal gastrinemia levels in pernicious anemia depend on the histological status of the antral mucosa--which was always normal in the patients with pernicious anemia--rather than any substantial morpho-functional differences of the body-fundus. We thus conclude that the term "atrophic gastritis" should be abolished, and that the term "chronic atrophic gastritis" be used to describe both conditions.

Three normal volunteers and 20 patients with known Crohn's disease were examined with MRI--at 0.5 Tesla and with a superconductive magnet. Coronal T1-weighted GE images were mainly acquired, before and after i.v. Gd-DTPA injection in breath hold (TR 70 ms, TE 13 ms, FA 70 degrees). MR findings were compared with the results of small and large bowel enema. In 6 patients (30%) the abnormal loops were missed. In the other 14 patients (70%) MRI did depict the affected loops in the same sites as depicted by conventional radiography. The bowel wall was thickened (4-10 mm) in all patients. In 10 patients the thickened wall was markedly enhanced after Gd-DTPA injection. In 6 patients MRI demonstrated disease complications--i.e., stenoses, fistulae and abscesses--missed by conventional radiography. In 7 patients MRI showed the bowel to be more involved than demonstrated by conventional studies. Bowel wall thickening appeared to be a constant and reliable sign of disease. Wall enhancement was a less frequent sign but, when present, it was considered as characteristic as wall thickening. In the staging of Crohn's disease, MRI yields more pieces of information than conventional radiography and depicts the involvement of the intestinal wall and of its surrounding spaces.
OBJECTIVE: Evaluation of the performance of different HCV PCR detection systems for HCV RNA: A nested PCR, considered the reference assay, was compared with two single-step methods (ss-PCR): the first is based on the detection of PCR products by liquid hybridization with a 32P end-labelled probe (isotopic ss-PCR), while the second assay is a colorimetric method (colorimetric ss-PCR) using microwell plate hybridization with a specific nucleic acid probe (Amplicor HCV PCR, Roche Diagnostics Systems). METHODS: Sera from 56 patients with suspected hepatitis C infection based on reactive serology or altered liver parameters, and sera from 15 blood donors were tested for HCV RNA: After RNA extraction, the synthesized HCV cDNA was amplified in parallel using isotopic ss-PCR, colorimetric ss-PCR and nested PCR. The products were detected by autoradiography, color development and ethidium bromide fluorescence, respectively. RESULTS: In order to assess the analytical sensitivity of ss-PCR versus that of nested of PCR, experiments included serial dilutions of positive control samples. Results showed that both methods had an extinction signal at the 1:512 dilution. A comparative analysis of 71 clinical sera samples was obtained using the three protocols and the results clearly documented 100% concordance. CONCLUSIONS: Single step PCR methods for HCV RNA have a sensitivity equal to that of nested PCR and appear more suitable for diagnostic applications. Ss-PCR is safer than nested PCR in terms of both specificity and contamination problems. In particular, the Roche Amplicor HCV PCR assay minimizes sample exposure and management problems.
