The blood leukocyte count and its prognostic significance in subarachnoid hemorrhage.

This study reviews the white blood cell count in 75 patients with subarachnoid hemorrhage and correlates it with the clinical grade at admission, the outcome, the presence of angiographic spasm and blood clots in the cisternal spaces. Patients with subarachnoid hemorrhage with normal angiograms usually have slow white cell count and favourable outcome. In patients with subarachnoid hemorrhage due to ruptured aneurysms, their'is a clear correlation between the cell count and the entity of the cisternal hemorrhage and arterial spasm. Whereas the cell count is fairly correlated with the clinical grade at admission, a cell count exceeding 20,000 in associated with a 90% rate of poor outcome, even in patients with low clinical grade at admission. Therefore the count of the white blood cells seems to have high prognostic value in predicting the neurological deterioration due to arterial spasm.

Giant osteoma of the cranial vault with acromegaly and hydrocephalus. A case report.

A case of acromegaly associated to a giant osteoma of the cranial vault and hydrocephalus in a young woman is reported. It can be suggested that acromegaly favoured the marked growth of the osteoma. Hydrocephalus resulted consequent to both the compression of the superior longitudinal sinus by the tumor and alteration of the CSF resorption. The rarity of giant osteomas of the cranial vault associated with intracranial hypertension is pointed out.

[Neurinomas of the acoustic nerve and surgery with the suboccipital approach. A study of 70 cases].

The Authors report 70 cases of medium and large size acoustic neuromas, operated on using the suboccipital approach, the only one which allows their total extirpation. Total extirpation was performed in 77% of the patients. The Authors analyze the clinical picture, the neuroradiological diagnostic procedures and the surgical techniques used in the treatment of these lesions.

Intracranial hemorrhages in metastatic brain tumors.

Ten cases of brain metastases with significant intracranial hemorrhage are examined. The most frequent primary tumors were melanoma and lung carcinoma; bleeding was more commonly intratumoral and intracerebral. The clinical onset was acute in 6 cases and gradual in 4; the average survival time was 2 months. Intracranial bleeding in patients with brain metastases can present as a "stroke syndrome" and it remarkably reduces the survival time.

Hypertrophy of the ligamenta flava of the cervical spine. Clinico-radiological correlations.

Among 1,000 myelographies performed between 1974 and 1983, 22 patients with isolated hypertrophy of the ligamenta flava of the cervical spine have been selected and their neurological troubles correlated with the radiological findings. The degree of neurological involvement resulted in relation with the degree of hypertrophy of the ligaments, the diameter of the spinal canal and the number of spaces involved by compression. The radiological diagnosis and the indications to surgery are discussed.

Acute Disease↗

[Epilepsy caused by vascular insufficiency as a result of a malformation in a child].

The Authors report a case of a 7-year-old boy with focal epileptic seizures; cerebral panangiography revealed a "coiling" of the right internal carotid artery, a scarcely visualized right posterior cerebral artery, and a diffuse hypovascularisation of the right cerebral hemisphere. They emphasize the possible relation between the vascular insufficiency and the epileptic seizures and they point out the possible role of congenital vasal malformations, as coiling and hypoplasia, in the origin of epilepsy in infancy.

[Metastases of intestinal carcinoma simulating a meningioma of the olfactory perforation].

A case of a tumor of the anterior cranial fossa, which had clinical, radiological and surgical features of an olfactory meningioma, but histological findings of a metastasis from intestinal carcinoma, is reported. Differential radiological features between meningiomas and metastases are briefly examined and the role of the dura mater in metastatic spreading is discussed.

Adenocarcinoma↗

[Malignant extradural spinal cord compression in childhood: diagnostic, surgical, and prognostic considerations with respect to 6 cases].

The authors report six cases of malignant extradural spinal tumors in childhood (3 sarcomas, 2 neuroblastomas and one metastasis). They emphasize the different frequency of these tumors in infants and in adults, the clinical diversity between the primitive and metastatic tumors and the necessity of an early surgical treatment followed by the radiotherapy in the primitive tumors, because of the possibility of long survival.

Cerebral hemorrhagic complication in polyarteritis nodosa. Case report and review of the literature.

Polyarteritis nodosa (PN) is frequently cause of both central and peripheral neurological disorders. However, involvement of the central nervous system is rare at the beginning of the disease. In this paper we report a case of a 38-year-old woman hospitalized in our Neurosurgical Service because of left hemiparesis in presence of cutaneous arteritis (PN). At computed tomography (CT) an intracerebral hemorrhage was found. No radiographic evidences of vasculitis of the visible arterial branches, at angiography, were seen. Cerebral arteritis should be suspected as a cause for intracranial hemorrhage in patients without hypertension or other risk factors.

Cerebellar medulloblastoma in adults: report of 15 cases and review of the literature.

From 1974 to 1988, 15 adult patients (aged over 16 years) with cerebellar medulloblastomas were observed in our Neurosurgical Department. All were treated by total (8 patients) or subtotal (7 patients) resection of the tumor, followed by radiation therapy to the posterior cranial fossa, spine and whole brain. A 5-year survival was achieved in 7 patients (46.6%). Local tumor recurrence occurred in 4 patients. The best treatment and the factors that influence the prognosis and survival of adult medulloblastomas are discussed also from the analysis of the pertinent literature.
