Stereopsis after congenital monocular cataract extraction.

Congenital monocular cataract extraction was performed on a 1-day-old baby girl and a hydrophilic contact lens was fitted. A progressive regimen of part-time occlusive therapy was then instituted. Eight years later, the girl has a visual acuity of 20/25 in the operated-on eye and 50 seconds of arc of stereoacuity that has been confirmed by an independent pediatric ophthalmologist. This patient demonstrates the potential for high-grade stereopsis in congenital monocular aphakia.

Phacoemulsification and modified trabeculectomy for managing combined cataracts and glaucoma.

Seven eyes in four patients who had combined cataract extraction, intraocular lens implantation, and glaucoma filtering surgery were examined retrospectively. A phacoemulsification technique was used with trabeculectomy performed under a modified limbal flap. All patients achieved a final visual acuity of 20/40 or better and intraocular pressure of less than 20 mm Hg without medication. The techniques are described and the management of combined cataracts and glaucoma is discussed.

X-linked pathologic myopia.

We examined a patient with decreased visual acuity and macular changes attributable to pathologic myopia. Thirteen additional family members were examined, and historic information was obtained on their relatives. A pedigree spanning six generations was constructed that demonstrated an X-linked mode of inheritance. This finding has important genetic and therapeutic implications for similarly affected patients.
