[Multifocal glioma. Apropos of 10 cases].

The authors report 10 cases of multifocal glioma of the brain. There are distinguished in multicentric tumors, when lesions develop independently in the space and time and multiples tumors when they are in connection by spreading process. They analyzed the factors that could permit to distinguish these gliomas. Multicentric tumors are histologically of low grade with gradual evolution, and from different histological nature and topography. In other hand, multiple gliomas are from malignant histology and develop simultaneously. The CT scan and biopsy in stereotaxic condition could permit to recognize these two parts which the prognosis is different.

[Primacy of medical treatment of cerebral abscesses (19 cases)].

Therapy in 19 consecutive cases of cerebral abscess was limited to medical treatment whenever possible, using high doses of a combination of several antibiotics. Long-term follow up by serial computed tomography was performed. Antibiotic treatment alone was effective in 8 patients with multiple hemispheric abscesses of metastatic origin, though one patient seen late with three large abscesses die on the 4th day. In 8 cases with single abscesses, medical treatment alone was effective: 5 had lesions less than 4 cm in diameter; in 2 cases with lesions 4 to 5 cm in diameter, additional abscess puncture was carried out. One patient required early excision of an occipital abscess, and 2 cases of pontocerebellar angle abscesses were also treated surgically. Antibiotic therapy alone was effective in 1 patient with a large brain stem abscess. Treatment of cerebral abscess has largely changed since the availability of computed tomography, with a progressively more important place being accorded to antibiotic therapy alone. The stage of the abscess does not appear to be a decisive factor in the choice of treatment, medical therapy appearing to provide a definitive cure with a minimum of sequelae in the majority of cases. Abscess puncture may be an adjunct to antibiotic therapy in large abscesses with clinical signs.

[Acutely developing glioma and negative CT scan. Apropos of 3 cases].

Three adults presented convulsive fits for the first time in their lives, and examination with a CT scan showed no anomalies. Repetition of the CT scans a few weeks later revealed the presence of evolving lesions. Based on these 3 cases and 11 previously published cases, the problem of "normal" CT scans and rapidly developing glioblastomas is discussed.

Acute Disease↗

[Extradural spinal hematoma without traumatic bone lesion. Apropos of 9 cases].

Extradural spinal hematomas observed in 9 cases over a period of 11 years developed in the absence of blood coagulation disorders in 3 cases, during anticoagulant therapy in 5 cases, and during the course of a megaloblastic anemia in one patient. The presence of marked associated myelomalacia was confirmed at postmortem in one case. Etiopathogenic factors remain obscure, particularly in spontaneously developing forms, and careful histologic examination of all clots is necessary to determine possible vascular malformations at the origin of the hematoma.

Indications for halo immobilization of the cervical spine.

In the United States, cervical spine injuries are routinely immobilized with the halo apparatus. In Europe, this device is seldom used except in the treatment of spinal deviations. Based on studies of five cases the authors discuss the indications for use of the halo device in cervical spine injuries.

[Metastasis of visceral tumors in intracranial tumors. Apropos of a metastasis of a lung cancer in an intracranial meningioma].

In a 52 years old man a backward epilepsy revealed a left fronto parietal tumor. The histopathological study shows that it is a meningial tumor composed around by meningocytic proliferation with a metastasis of lung cancer in the middle. This fact was proved only two months after neurosurgical intervention. Thirty three cases of cancer with metastasis in an intracranial primitive tumor have been collected in the literature. The authors study the clinical aspects and etiopathogenic hypothesis of this surprising anatomopathological fact.
