[Arachnoid cysts of the cerebellopontile angle].

Ten cases of arachnoid cysts of the ponto-cerebellar angle are presented. In most cases, local arachnoiditis is disclosed during surgery, directing the discussion toward acquired pathology, and perhaps toward local infection. The clinical symptoms are dominated by the cochleo-vestibular deficit, but the involvement of the V and the VII cranial nerves is inconstant and discreet. At the present time, C.T. scan allows a rapid diagnosis but the prognosis must be reserved, in account to the possibility of a local post operatory arachnoiditis .

[Value and limitations of isolated ventricular shunt in non-traumatic cerebellar hematomas].

Two cases of cerebellar haemorrhage with hydrocephalus are presented which were successfully treated with isolated ventricular drainage. Both occurred in high risk patients, 77 and 68 years old. The first observation confirms the value of this operative procedure when the volume of the haemorrhage is relatively restricted, whatever may be the importance of the neurologic symptoms and brainstem disfunction. But the second observation also shows the risk of misappreciating the causative lesion (here a single metastasis). The importance of an extended C.T. follow up must be underlined.

[Role of surgical decompression by the anterior approach in myeloradiculopathies caused by cervical arthrosis. Apropos of 850 cases].

The authors report their experience of the different clinical aspects of myeloradiculopathy due to cervical arthrosis. From 850 cases with surgical treatment, they describe the radicular, medullar and radiculomedullar features of this condition. The indications for an anterior decompressive procedure are analyzed according to clinical and paraclinical findings and results of the different methods used are compared.

[Subarachnoid spinal metastases. Apropos of 2 cases].

Two cases of spinal subarachnoidal metastases are reported. From data from the literature, the authors review clinical manifestations and diagnostic difficulties. Etiopathological evidence which may explain this localization is discussed.

[Symptomatologic approach to tumors of the pineal region. Apropos of 72 cases].

The semeiology of 72 tumors of the pineal area seen between 1949 and 1982 is analyzed. Manifestations, which are polymorphic and often misleading at onset, subsequently become manifold. Though a given syndrome may result from the mingling of several factors it is possible to establish a correlation between clinical findings and anatomical lesions. Conversely, histo-clinical correlations, which are requisite to institute specific therapy, are still as difficult to ascertain. Stereotaxic biopsy is therefore needed before planning therapy.

[Results of microneurosurgical treatment of 300 intracranial arterial aneurysms].

The authors present the results obtained with the current neurosurgical treatment in an homogeneous of 292 patients totalling 300 intracranial arterial aneurysms. The cure rate was 74% and the death rate 14%; poor results were obtained in 12% of the cases. Analyzed globally, the main prognostic factors were the patient's age and general condition, the site and mode of rupture of the aneurysm and the timing of surgery.

Arachnoid cysts and head injury.

A number of hypotheses and much discussion surround the subject of intracranial arachnoid cysts (ICACs). Based on the observation of 7 cases of ICACs which were asymptomatic until aggravated by head injuries, the aetiopathological relationship between head injury and ICAC is discussed. Trauma is not the only origin of ICACs, but even a slight head injury is capable of provoking the rapid decompensation of a previously dormant ICAC.

Encephaloarteriosynangiosis in the treatment of chronic cerebral ischemia: preliminary report based on 30 cases.

A new surgical technique for the treatment of cerebral ischemia is reported. A scalp artery is freed and placed in contact with the cerebral cortex after craniotomy and opening of the dura mater. Thirty such operations have been performed over a period of 2 years. An analysis of the results, advantages and disadvantages, and indications for this type of surgery is given.

Brain Ischemia↗

Quadriparesis as an unusual manifestation of chronic subdural haematoma.

Chronic subdural haematomas are frequently seen in neurosurgical practice, but because of the multitude of their clinical presentations, appropriate diagnosis is often not made in the early stages. The cases reported concern a 62-year-old male and a 54-year-old woman, both presenting in an unusual manner, namely a progressive quadriparesis.

Cerebral Angiography↗

An intramedullary ependymal cyst in a 71-year-old woman.

An intraspinal ependymal cyst of the thoracolumbar spinal cord was found in a 71-year-old woman with a history of right side abdominal pain since the age of 18. The late discovery of the cyst is related to the extremely slow evolution of this type of cyst in the absence of spinal trauma. Etiopathogenic problems are discussed after a review of the literature.
