[Clinical studies with the new saluretic azosemide].

It is reported on the clinical experiences with the new diuretic azosemid in 22 patients with oedemas of various genesis. Azosemid displays the characteristics of a very effective and well tolerated saluretic. After intensive treatment azosemid exerts similar side effects on the electrolyte (hyponatraemia, hypokalaemia, hypochloraemia) and purine metabolism (hyperuricaemia) as furosemide. Since azosemid is a very effective and well tolerated diuretic with no serious side effects it seems that it is a valuable extension of the contemporary diuretic therapy.

Parathyroid hormone, prolactin, and function of the pituitary-gonadal axis in male patients with acute renal failure.

In 26 patients with acute renal failure (ARF), plasma levels of lutropin (LH), folitropin (FSH), prolactin, testosterone, estradiol, and parathyroid hormone (PTH) were studied at the anuric/oliguric (AOP) and polyuric phase. Significantly elevated levels of LH, FSH, and prolactin were found during the AOP. Administration of luliberin (LH-RH) was followed by normal or even excessive and prolonged increase of plasma LH and FSH levels during the AOP. Basal plasma testosterone was significantly depressed, but estradiol was moderately elevated at the AOP. Administration of LH-RH did not influence significantly either testosterone or estradiol levels in blood plasma. A negative correlation was found between plasma prolactin and testosterone, and a positive one was found between prolactin and estradiol at the AOP of ARF. In contrast to chronic renal failure, PTH was not significantly correlated with either plasma testosterone or prolactin. We conclude that the hyporesponsiveness of Leydig's cell to the LH signal is the cause of depressed testosterone biosynthesis. It seems probable that prolactin, but not PTH, is involved in the pathogenesis of testicular dysfunction in patients with ARF.

[Effect of ultraviolet irradiation on renal osteopathy in patients in the chronic hemodialysis program].

Under influence of the ultraviolet radiation the formerly lowered 25-hydroxycholecalciferol (25-OH-CC) level increases in persons with healthy kidneys. Such investigations were not yet carried out in patients undergoing dialysis. Since the insufficiency of active vitamin-D-metabolites plays an essential role in the development of the renal osteopathy, since 1978 in 13 patients undergoing dialysis a regular ultraviolet radiation has been performed, in order to stimulate the cutaneous vitamin-D-production. The time of observation was 31.7 +/- 14.3 months. Deionized water served for the production of dialysate. The calcium content was about 3.5 +/- 3.8 mval/l. The patients had very rarely bone and joint complaints. Severe clinical complications of renal osteopathy developed only in one female patient in the 3rd year of dialysis with a fracture of the neck of the femur as well as in a 2nd patient after transplantation of a kidney with permanently progressing demineralization and spondylitis. Controls of the courses showed only in 18.2% a progressing loss of the peripheral mineral content of the bones. The alkaline phosphatase was not increased, the alkaline bone phosphatase appeared with low and not provable activities. A suppression of the secondary hyperparathyroidism could not be proved. Parathormone was increased with 2.30 +/- 1.90 micrograms/l. The 25-OH-CC-levels were normal or slightly increased with 39.1 +/- 12.4 micrograms/l, whereas they were essentially lower in a comparative group without ultraviolet radiation. Therefore in connection with the very low rate of complications of the renal osteopathy with normalized 25-OH-CC-levels new therapeutic possibilities were the result in patients undergoing dialysis after ultraviolet radiation. The increasing importance consists also in the fact that 90% of the human vitamin-D-need are supplied via skin and it was proved by animal experiments that also after bilateral nephrectomy the formation of the active vitamin-D-metabolite 1,25-dihydroxycholecalciferol is possible by photosynthesis.

Relation between middle molecules and parathyroid hormone in patients with chronic renal failure.

In 10 moderately and 41 severely uremic patients the relation between serum parathyroid hormone (PTH) and middle molecule fraction 2 (MMS fr. 2-corresponding to MMS peak 7 of Fürst) was investigated. A significant correlation between PTH and MMS fr. 2 levels was found. This correlation was even better between PTH levels and MMS fr. 2/creatinine ratios. We would like to propose that PTH is one of several factors regulating middle molecule levels in uremic patients and that some 'toxic' effects of PTH may be mediated by middle molecules.

Prolactin secretion in patients with acute renal failure.

In 44 patients with acute renal failure serum prolactin levels were estimated using chloropromazine as a stimulant and alfa-bromocriptine as an inhibitory agent. Significantly elevated basal serum prolactin levels were found both in oliguric and polyuric patients with ARF. Intramuscular (i.m.) administration of chloropromazine markedly increased serum prolactin concentrations. In patients with ARF the apparent half life of prolactin, calculated from the prolactin curve obtained after alfa-bromocriptine administration, was moderately longer than in normals. From the results obtained the following conclusions were drawn: 1. Hyperprolactinemia of patients with ARF seems to be due predominantly to hypersecretion of this hormone and, to a lesser degree, to impair renal biodegradation. 2. In patients with ARF prolactin secretion is hyperresponsive to intramuscularly administered chloropromazine.

[Influence of azosemide on renal metildigoxin excretion (author's transl)].

The influence of the diuretic azosemide on metildigoxin blood levels and on renal excretion was assessed in 20 patients with chronic cardiac failure. Two groups, each of 10 patients, were treated for 14 days with 0.1 and 0.2 mg metildigoxin, respectively. All patients received 2 tablets of azosemide, 80 mg each, from day 12 to 14 of treatment. There was no significant influence of the diuretic either on blood levels or renal excretion of metildigoxin.

[Comparison of the lipid-lowering agents clofibrate and bezafibrate in patients with hyperlipoproteinemias IIa and IIb].

The influence of clofibrate and benzafibrate on total serum cholesterol and triglyceride level was investigated in patients with primary hyperlipoproteinaemia type IIa (16 patients) and IIb (13 patients), following a double-blind protocol. A significant decrease of the triglyceride level in the blood serum was stated in patients with hyperlipoproteinaemia type IIb, treated by benzafibrate. From the results presented in this paper it seems that benzafibrate is especially suitable for the treatment of hyperlipoproteinaemias, characterized by hypertriglyceridaemia.
