[Upper digestive system endoscopy in Gabon. Contribution of 2 years' experience].

Reporting on 805 endoscopies of upper digestive tract performed in a rural area of Gaboon during a period of 28 months, the authors describe the indications and the results by analysing comparatively their findings with some other African ones. Their experience is deeply in favour of performing much more often fibreoptic endoscopy, even if the percentage of investigations revealing no anomaly is high, because its indisputable medical and economical interest.

[Hemorrhagic rectocolitis or ulcerative colitis in the black Senegalese. Apropos of 14 cases].

Ulcerative colitis is exceptional in Africa South of the Sahara. The first case was described in 1964: up to 1982, 46 cases were reported. We report on 14 cases (6 females and 8 males) observed in black Senegalese for three years and a half. The medium age of patients was 30 years. All of them had a high social standard of living. Four patients showed intestinal amebiasis in their previous medical history. Clinical and evolutive patterns of ulcerative colitis in Africa present few differences in comparison with the classical ones. The diagnosis is established by endoscopy (rectosigmoidoscopy and/or colonoscopy) and histopathology. Cryptic abscesses were found in only 2 out of 14 cases. Rectal dysplasia was noted in 2 cases. The course of the disease developed on a fulminating acute mode in one case, on a continuous chronic mode in another case and on a chronic mode with relapses in 11 cases. Irrespective to the clinical form observed, intestinal amebiasis represents the most pressing differential diagnosis. Treatment appears to be difficult in severe acute forms out of an intensive care department, as well as in advanced extensive chronic forms. For the later in black Africa endoscopic and histopathological surveillance appears to be preferable to a total colectomy, made necessary by the important rectal lesions, because the functional sequelae of an ileostomy.

[A survey on gastric diseases in black Africans in Dakar (Senegal) (author's transl)].

The authors report on a survey about 1.188 Senegaleses affected by gastric disease. The diagnosis was established by either endoscopy or, less often, by a surgical control. The findings were ulcer (43,09 p. 100) with duodenal localisation in most cases (67 p. 100), non malignant tumor (0,84 p. 100), malignant tumor (1,85 p. 100) and most often (53,95 p. 100) non ulcerous and non tumorous gastropathies. Noticed clinical features are not much different from the ones observed in Caucasian patients. Sex ratio gives male preponderance and there is an etiologic role of age: superficial gastritis and duodenal ulcer prevail from 20 to 40, chronic gastritis from 20 to 50 and gastric ulcer after 35. Smoking tobacco, misuse and overdose of aspirin and antiphlogistic drugs seem to be facilitating factors which were recorded in 50 p. 100 of non ulcerous gastropathies, 40 p. 100 of gastric ulcers and 27 p. 100 of duodenal ulcers. Neither ethnic origin, nor blood group, nor diet have shown any etiologic influence. Most patients with an ulcer have 3 periods of pain every year and one among three needs a surgical intervention.

[The C virus in Africa].

C virus (HCV) main responsible of non A - non B hepatitis appears to be rather frequent within the world population African continent seems to possibly be situated in the high prevalence zones of antibodies HCV carrying. Detection methods of these antibodies have been polished but very fragmentary studies on African populations do not permit to conclude with certainety about the prevalence rate of this virus. However, it appears that concentrations of antibodies detected during chronic hepatopathies might be similar in Europe and in Africa. An epidemiological study on representative samples of the African population seems to be necessary to appreciate the importance of this infection and to monitor its transmission.

[Intestinal parasitoses in the south-eastern district of the Dominican Republic].

A prevalence test about parsitism in the South-East district of Dominique Island was carried out from 2 populations samples: some people preparing foods, and out patients in day care Centers. The parasites, the most frequently identified are: ankylostomae, ascaris and whipworms. The prevalence rate of parasitic infestation in this Island appears to be a suitable socio-economical development index.

[Cholelithiasis in homozygous sickle cell anemia. Review of the literature].

Cholelithiasis in homozygous sickle cell disease is a common complication. Mean prevalence rate recorded is 27.4% with a greater frequency in the second part of life. It is a peculiar lithiasis, made of pigmentary stones either purs or duals, caused by hypersaturation of indirect bilirubin. Systematic detection of such a lithiasis by echography is part of monitoring homozygous drepanocytic.

[Perianal cutaneous ulcerations in the tropical zone. Proposal for a diagnostic and therapeutic decision tree. A case report].

Perianal cutaneous ulcerations in tropical countries have multiple causes: bacterial, viral and parasitic. Taking the opportunity of a case observed in Bambari (CRA) of which the etiologic origin was not clearly established, the authors offer a proposal for a diagnostic and therapeutic protocol aimed at easing decision. Etiologies quickly cured, such as amoebiasis and tuberculosis have to be explored in priority.
