The authors evaluated the costs involved when alcoholic patients were admitted to a hospital medical department. Costs which were identical for all patients, such as basic services and hospitalization fees, were discounted, and only those for individual investigations and treatment were calculated. These costs were then compared with the average daily costs for non-alcoholic patients in the same department over the same period. The results are discussed in relation to other similar studies.
Bitterfeld and Wolfen are towns with extremely polluted air, due to important plants of the chemical industry. In two different actions we examined 3634 children of four different groups of age. As a result we found 7,8% of them (281 children) suffering from chronic or recurrent nonspecific bronchopulmonary diseases (7,0% suffering from recurrent or chronic bronchitis, and 0,8% from bronchial asthma). The rate of morbidity was the same in both towns. It was significantly higher than in the other districts examined by us (Halle, Merseburg, Quedlinburg). In contrast to the other districts we found in Bitterfeld and Wolfen only an insignificant decrease of the morbity rate with increasing age of the children.

Accidental discovery of a moderate and durable increase of transaminases in an asymptomatic patient is a more frequent possibility thanks to the development of methodological case findings activities. Most of the time, it is an isolated anomaly of which import and consequences have been already reviewed and published. Steatosis and chronic hepatitis are, by far, the most frequent etiologies in Western Countries. But the right importance of that syndrome in tropical hepatology has not been yet described. If we want to transpose results and what has to be done namely liver biopsy, to African medical practice, we have to take into due consideration local pathology, diagnosis facilities and/or therapeutic possibilities. The present article reviews the present knowledge on chronic liver cytolysis syndrome and discusses its interest in tropical Medicine.
Prognosis of chronic viral hepatitis can be severe. One have to keep in mind the possibility of cirrhosis and hepatocellular carcinoma. Thanks to anatomopathological and immunological tests, it is possible nowadays to evaluate the extent of chronic viral hepatitis and its prognosis. The most efficient therapies are actually the antiviral drugs (vidarabine, interferon): against chronic viral hepatitis B: vidarabine or interferon preceded by a cure of corticoids lead to about 35% of H Be seroconversion--against chronic viral hepatitis B-Delta: interferon Has a suppressive effect in about 50% of the cases with an inevitable relapse when the treatment is stopped--against chronic viral hepatitis C: interferon is efficient in about 50% of the case when given for six months. The high proportion of relapse justified some treatments expended up to 12 or 18 months.
Tropical sprue is a diagnosis about which we have to think when we are confronted to a patient back from overseas. We examined a young man back from Central African Republic where he got diarrhoea which keeps going on in France, with alteration of his general condition and stigmata of malabsorption. Taking advantage of such observation, the authors report on the present knowledge about that disease peculiar to tropical zone. Biological elements of malabsorption are associated to partial emaciation of villi of small intestine. As a matter of fact, diagnosis is made by elimination. Its pathogenesis remains unknown and is linked to some troubles of intestinal microbism of which exact causes has not been yet put into evidence. An easy treatment by tetracycline and folic acid is a true diagnosis test, as general condition is set up again, as well as villi intestinalis.
Such aspects are numerous from a disease with multiple factors among them immunodeficiency playing a key-role. O.R.L. aspect are more frequently observed than some ten years ago: this is mainly due to Kaposi's sarcoma in its epidemic form. Nevertheless one has to keep in mind that Kaposi's sarcoma in this endemic african form coexists with its second form without any well-marked border. Finally, many uncertainties are still existing among others its very peculiar cancerous nature.
Omnipresent disease, Burkitt's lymphoma is the most frequent non-Hodgkin malignant lymphoma in children (NHML). As a matter of course, maxillo-facial lesions are not exclusive, as we thought thirty years ago, but it is evident that they are extremely frequent in tropical areas. They strike all facial quadrants. Their aspects are always impressive, they perfectly diminish by a well applied polychemotherapy.
Using our knowledge of recent discoveries on chronic HBV hepatitis, we consider the indispensable addition that the close study of HBV replication represents. An accurate evaluation of this replication can be obtained through quantitative research of the HBc antigen on liver. The detection is currently facilitated by the production and marketing of good quality antisera and revelation systems. The routine application of these techniques by classical histology departments is possible.
In 1986 prevalence rates of anti-HIV1 antibodies in randomly selected asymptomatic gabonese population ranged from 0.2% in rural areas to, 1.8% in Libreville (the largest town). We report the first description of AIDS cases observed in the hospitals of Libreville and Franceville between 1985 and 1987. 23 patients (14 males, 9 females) met the clinical diagnosis criteria for AIDS according to the provisional WHO clinical case definition for AIDS. 21 of them were gaboneses. The mean age of patients was 32.8, only one child was observed (blood transfused sickle-cell anemia). The only risk factor that we observed three times was blood transfusion. The clinical presentation of those cases will be described. 22 were positive for anti-HIV1 antibodies by Western-Blot analysis of their sera, one was anti-HIV2 positive (a congolese man hospitalized in Franceville for severe weight loss and pulmonary tuberculosis).
