The capillary tube plasma viscometer--an automatical instrument for measuring plasma viscosity.

A new capillary viscometer was developed to eliminate the measuring errors due to insufficient cleaning or the residue of cleaning solution in the capillary. A disposable polyurethane tube which can be discarded after each determination is used as a measuring chamber. This device has been constructed for bed-side application (light weight, dry, thermostat-controlled measuring capillaries, short measuring period, automated process, and easy operation). The PEARSON correlation coefficient for comparative measurements with the COULTER-HARKNESS viscometer was r = 0.979.

The hematocrit-electrolyte-concentration-apparatus (HECA).

An instrument was developed which can simultaneously determine the hematocrit and the total electrolyte concentration in blood and plasma respectively. It is possible to correct the hematocrit value for fluctuations of the total electrolyte concentration. Pilot studies of this device have been conducted on physiological and rigid erythrocyte suspensions and on plasma with various total electrolyte concentration. In the future, this device will be employed in analyzing blood and plasma of patients undergoing hemodialysis.

The hematocrit-erythrocyte-disaggregation-apparatus (HEDA).

We have developed a device for the determination of the minimum shear force necessary to maintain complete disaggregation of erythrocytes and to simultaneously determine the hematocrit of blood samples (red cells in autologous plasma). A functional relationship between this shear force and the hematocrit was determined. The first derivative of the (measured) gamma Tmin as a function of the hematocrit was found to be identical (within limits) for all normal blood samples tested.

A new device for the routine measurement of erythrocyte deformability.

We present a device for the routine measurement of the deformability of individual red blood cells, the so called Single-Erythrocyte-Rigidometer (SER). The time required for individual erythrocytes to traverse a single pore membrane (pore diameter = 5 micron, pore length = 25 micron) is taken as an index of red cell deformability. For example we present the results from patients with insult, polyneuropathy and morbus binswanger. Compared to the results of erythrocytes of healthy probands a significantly increased rigidity of the RBC of the patients can be diagnosed.

Quantification of characteristic blood-flow parameters in the vessels of the retina with a picture analysis system for video-fluorescence angiograms: initial findings.

Fluorescence angiograms of the retina were performed on 12 healthy, adult subjects between the ages of 20 and 52 years with a 30 degrees fundus camera (C. Zeiss), a low-light TV camera and a video recorder. Characteristic blood-flow parameters for the retina were obtained from the video recording using the picture-analysis system (Mikrovideomat 3, C. Zeiss). These included: the arm-retina time (ART), the arteriovenous passage time (AVP) and the arterial contrast-medium velocity (ACMV). Normal values for these parameters were derived for the four retinal quadrants and then discussed. In addition, the filling characteristics of venules were measured and discussed.

Rheologic findings in patients with acute central retinal artery occlusion.

The most important rheologic parameters were examined in six patients with acute occlusion of the central retinal artery. Blood samples were taken as part of the initial clinical examination on the day of the occlusion before the onset of therapy, and the following parameters were studied: (1) yield shear stress; (2) hematocrit; (3) plasma viscosity; (4) extent of erythrocyte aggregation; (5) erythrocyte deformability; (6) plasma fibrinogen concentration. A definite shear stress could be measured in all patients, something not possible in rheologically healthy volunteers. Hematocrit values were in the upper normal range; plasma viscosity was significantly increased. The extent of erythrocyte aggregation was greater than normal, erythrocyte deformability was markedly reduced, and the plasma fibrinogen was markedly higher than the normal level. Aside from the well-known pathologic findings in the carotid arteries of many patients with acute retinal vascular occlusion, rheologic factors seem to be of great importance in this disease.

Analysis of paramagnetic intermediates at the reaction of hemoglobin with nitrite using EPR.

The autocatalytic oxidation of hemoglobin by nitrite was analysed. It was shown that the obtained paramagnetic species have a strong temperature dependence. The time of annihilation of these species decreases about 4 times in the temperature region between 10 degrees C and 30 degrees C, whereas the time of formation decreases only 1.7 times. These data explain the strange temperature dependence of the formation of methemoglobin by nitrite and support the assumption that the short-lived radical is localized on the hemoglobin molecule.

[Conservative surgery of the spleen].

Conservative surgery of the spleen only has to overcome certain prejudices to become generally accepted. Today, we are convinced of the essential role of the spleen in the body's anti-infection defence mechanisms and the surgeon now has the means to repair a damaged spleen. We report the simplicity, effectiveness and reliability of three techniques of splenorraphy. Two of these techniques are our own and have been well tested in very extensive animal experiments and in our first clinical applications. Capsular sutures or partial splenectomy can treat the great majority of the lesions seen. When this is not possible, devascularisation of the spleen or autotransplantation of the splenic tissue are simple palliative measures, although there functional value has not been clearly established. The results of this approach are now numerous with adequate follow-up and have revealed no complications, immediate or delayed, inherent in the technique.

[Acute primary ulcer of the small intestine. Present concepts (author's transl)].

Acute primary ulcer of the small intestine is an exceptional occurrence. Diagnosis is established only after a complication occurs. In most instances, this complication is perforation of the gut. Acute primary ulcer of the small intestine is accurately defined by specific histologic criteria. It can be unequivocally distinguished from the many other causes of spontaneous perforation of the small intestine. Experimental studies, although numerous, have not improved understanding of pathophysiology. Nosologic classification remains unclear. Many features are similar to those encountered in acute peptic ulcer but stress is usually absent. Other features resemble those of transient necrotizing enterocolitis ; acute primary ulcer of the small intestine may be a very localized form of this latter condition. Ischemia seems to be the most significant factor. However, for an ulcer and finally perforation to occur, ischemia must probably be associated with a number of other factors.

[Prognostic features of upper gastro-intestinal hemorrhages in adult over 65 (author's transl)].

Some prognostic features out of 57 cases at upper gastrointestinal hemorrhage, in people more than 65 years old, are reported. The age has no significance but after 70, gastroduodenal ulcers represent the more frequent etiology, with a very particular severity in cases of chronic and acute gastric ulceration. The mortality is still high, mainly due to decompensation of pre-existent renal, cardio-vascular or pulmonary taints, frequent in aged people.

Determination of yield point: methods and review.

The question whether blood exhibits pseudoelastic behavior i.e. if it behaves like a solid under negligible external forces, is of great theoretical interest for ths clarification of stasis phenomena in the microcirculation. The determination of yield point, that is the change from the solid to the fluid phase, was at first only of abstract scientific importance for blood. In the diagnosis and treatment of patients with flow anomalies this phenomenon is increasingly important, as numerous possibilities now exist to modify the flow behavior of blood by therapeutical changes of composition. Therefore it seems to be of high priority to review and discuss the numerous methods for the quantification of a yield shear stress.
