Self-limited neonatal familial hyperparathyroidism associated with hypercalciuria and renal tubular acidosis in three siblings.

Three siblings with neonatal familial hyperparathyroidism diagnosed at age 4 months, 2 months, and 5 days, respectively, were treated. Hypercalciuria, nephrocalcinosis, and renal tubular acidosis were present in each child. In all three, there were higher responses of serum parathyroid hormone to serum calcium and higher elevation of serum calcium with oral calcium loading. The metabolism of vitamin D and calcitonin seemed to be intact. Hypercalcemia associated with the abnormal response of parathyroid hormone secretion disappeared when the children passed the age of approximately 2 years, although renal tubular acidosis and nephrocalcinosis remained. An autosomal recessive inheritance seems likely.

[A case of autoimmune hemolytic anemia and bullous pemphigoid-like skin lesion combined with idiopathic thrombocytopenic purpura].

A case of autoimmune hemolytic anemia (AIHA) and bullous pemphigoid (BP)-like skin lesion combined with Idiopathic thrombocytopenic purpura (ITP) is reported. A 25-years-old male, who had been diagnosed as ITP and treated at another hospital, was admitted in this hospital recently complaining of disseminated bullous-vesicular eruptions on the whole body and autoimmune hemolytic anemia. Examinations, disclosed that RBC was 364 X 10(4)/microliters, reticulocyte 40, platelet 3000/microliters, direct and indirect Coombs test positive, and platelet Coombs consumption test was positive leading to the diagnosis of AIHA and ITP, known as "Evans syndrome." Vesicular biopsy-findings and immunofluorescence study showed suspicion of BP, but clinical course and blister was not improved though the administration of prednisolone was performed. Reports of cases of BP complicated by Evans syndrome are very few. AIHA, ITP and BP are considered to have autoimmune disorders and their pathogenetic mechanism are discussed. This patient consulted another hospital one year later, when we heard that skin eruptions already had disappeared.

[A study on endoscopic hemostasis from the viewpoint of morphologic traits of Dieulafoy ulcer--an attempt at endoscopic hemostasis by local injection of aethoxysklerol and ethanol].

Characteristics of the vascular course of ruptured vessels in the submucosal layer of Dieulafoy ulcer were studied, and endoscopic hemostatic treatment was performed using Aethoxysklerol and ethanol. 1) Observations of vessel courses in serial sections revealed that ruptured vessels in Dieulafoy ulcer exhibit rhexis in the lateral portions of the vessel and long courses over the ulcer margin in the submucous layer. 2) More dependable hemostasis seemed to be provided by thrombosis not only at the source of bleeding. i.e., at the site of vessel rupture, but also in the region of the whole ruptured vessel in the submucous layer. 3) Hemostatic treatment by local injection of Aethoxysklerol and ethanol was performed in 4 patients with Dieulafoy ulcer, and complete hemostasis was achieved in all 4 patients.

[Reduction of erythrocyte membrane band 4.2 protein in patients with obstructive jaundice].

Alteration of erythrocyte membrane proteins was studied in patients with obstructive jaundice in comparison with healthy controls. Analysis of the erythrocyte membrane proteins by SDS-PAGE revealed a slightly reduced protein 4.2 (MW 72 kDa). The erythrocytes with reduced protein 4.2 contained normal amounts of all other membrane proteins. In this study, it was demonstrated that the reduction of 4.2-protein is a common alteration in abnormal erythrocytes, target cells, and is observed in obstructive jaundice.

Application of fast atom bombardment with tandem mass spectrometry and liquid chromatography/mass spectrometry to the analysis of acylcarnitines in human urine, blood, and tissue.

Using a precursor-ion scan function on a triple quadrupole mass spectrometer, acylcarnitines were detected in the target matrices at or below concentrations of 1 nmol per gram by fast atom bombardment mass spectrometry. Acylcarnitine profiles from patients with known metabolic disorders were consistent with previously acquired data. Putative acylcarnitine signals were confirmed in one case by administration of stable isotope-labeled carnitine, which equilibrated rapidly with the endogenous pool. The addition of a continuous flow system enabled rapid sequential analysis without operator intervention, indicating the potential for automation of the analytical procedure. Incorporation of a micro-LC column enabled on-line liquid chromatographic/mass spectrometric analysis of selected patient samples. Large-scale screening and quantitative analysis of urine or blood for diagnostic acylcarnitines are now practicable.

Gastrocarcinogen N-methyl-N'-nitro-N-nitrosoguanidine is an antiandrogen.

The present study has investigated comparatively the in vivo effects of physiologic saline drink, N-methylN'-nitro-n-nitrosoguanidine (MNNG) drink, saline-MNNG drink and hydrocortisone treatment in terms of 1 functional parameter (daily liquid consumption) and 3 weight parameters (weights of whole body, forestomach and glandular stomach) using Swiss/ICR mice of both sexes. The results obtained are as follows: 1) The sole use of hydrocortisone treatment significantly increased daily water consumption in mice of both sexes. 2) The promoting effect of the hormone on liquid consumption was further accelerated by concomitant use of physiologic saline. There was no sex difference in the extent of drink increase, but the figure of male mice was more variable than that of female mice. 3) The introduction of the 3rd stressor MNNG significantly reduced the salt-hydrocortisone-linked acceleration of liquid consumption. The extent of MNNG suppression was more marked in male mice than in female mice. 4) The responses of the 3 weight parameters to hydrocortisone were found to be tissue- and sex-specific. The dynamic changes of the above 4 parameters under the influence of the 3 stressors are in agreement with the interpretation that androgen plays a positive role in the salt-hydrocortisone-linked acceleration of liquid consumption, and that MNNG reverses the drink increase by inhibiting the binding of androgen with its receptor.

Adult T-cell leukemia/lymphoma and cutaneous T-cell lymphoma. Are they related?

Comparative studies were performed on clinical and laboratory features of four patients with different types of T-cell lymphoma of the skin; adult T-cell leukemia/lymphoma (ATLL), Sézary syndrome, mycosis fungoides, and Ki-1-positive lymphoma. All neoplastic cells studied showed a helper-inducer T-cell phenotype. A Ki-1-positive lymphoma is distinct from other types of cutaneous lymphomas because of unique morphologic and phenotypic features. Clonal proliferation of lymphocytes infected by human T-cell lymphotrophic virus (HTLV)-1 distinguishes ATLL from other T-cell lymphomas of the skin, especially in the endemic area of ATLL. From the pathogenic point of view, ATLL should not be included in a group with mycosis fungoides and Sézary syndrome.

Short-chain acyl-coenzyme A dehydrogenase deficiency in mice.

A murine model for short-chain acyl-coenzyme A dehydrogenase (SCAD) deficiency has been identified and characterized in BALB/cByJ mice. These mice have undetectable SCAD activity, severe organic aciduria; excreting ethylmalonic and methylsuccinic acids and N-butyrylglycine, and develop a fatty liver upon fasting or dietary fat challenge. The mutant mice develop hypoglycemia after an 18-h fast, and have elevated urinary and muscle butyrylcarnitine concentrations. Most of these findings parallel those of human disorders associated with SCAD deficiency and other beta-oxidation defects. This mouse model presents important opportunities to investigate the biology of mammalian fatty acid metabolism and the related human diseases.

Affinity resins for the purification of opioid-binding materials.

Two new affinity resins for the purification of opioid-binding materials were prepared. One was AH-Sepharose coupled with [D-Ala2, D-Leu5]enkephalin and the other was AF-Amino Toyopearl with [D-Ala2, Leu5]enkephalin. Solubilized-opioid receptors from rat brain were treated with these affinity resins and the materials with opioid-binding activities were purified. On sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis, the purified materials showed one major band with a molecular weight of 62000-64000. The results suggested that the prepared resins are useful tools for the purification of opioid receptors.

Intracellular metabolism of biogenic amines in paraneurons.

Serotonin is one of the representative biogenic amines produced from tryptophan in the gastrointestinal tract as well as in the brain. The pineal gland also synthesizes serotonin, but as an intermediate in the biosynthesis of melatonin. The first enzymic step in the biosynthesis of serotonin is the hydroxylation of L-tryptophan catalyzed by tryptophan 5-monooxygenase. The 5-hydroxy-L-tryptophan (L-5HTP) formed is then decarboxylated to serotonin. Since the second step is catalyzed by non-specific aromatic L-amino acid decarboxylase, whether or not a given cell has the capacity to synthesize serotonin depends on the existence of tryptophan monooxygenase, and regulation of the serotonin biosynthesis is achieved mainly by modulating the activity of the first step enzyme. The activity of tryptophan monooxygenase was detected in extracts of the mouse stomach and intestines, although this activity was as low as approximately 1/20 of that in similar extract of the mouse brainstem. The upper small intestine and colon had a higher level of activity than other parts, and in the upper small intestine the enzyme was found to reside primarily in enterochromaffin cells of the mucosa. The intestinal tryptophan monooxygenase shared a common antigenic character with the enzyme from murine mastocytoma and was immunologically different from the brain enzyme. The enzymic properties were also similar to those of mastocytoma tryptophan monooxygenase which requires loosely-bound functional iron (Fe2+) for the activity. It seemed likely that the activity of tryptophan monooxygenase in paraneurons such as mastocytoma and enterochromaffin cells depends on available ferrous iron, and is therefore regulated, at least in part, by the cytosolic level of chelatable iron.

[Alteration of erythrocyte membrane proteins in a family with hereditary spherocytosis].

Erythrocyte membrane proteins in a family of nine with hereditary spherocytosis in Okinawa were analyzed by polyacrylamide gel electrophoresis in the presence of 0.1% SDS. An abnormality in the membrane protein component band 4.2 (MW 72 kDa) on the electrophoresis was observed. Deficiency in band 4.2 was found in three sibling in the family and a small but significant decrease was noted in three other members. However, change of this component was not found in the remaining members of the family or in normal subjects.

Reconstitution of partially purified opioid receptors with a GTP-binding protein.

Partially purified opioid receptors, obtained from rat brains using an affinity resin, AF-Amino Toyopearl with [D-Ala2, Leu5]enkephalin, were reconstituted with an inhibitory GTP-binding protein (Gi). In the reconstituted system, the displacement curve for the binding of a delta-agonist, [D-Ala2, D-Leu5]enkephalin, showed two states, high and low affinity binding ones, with different affinities for the agonist. The high affinity binding was eliminated by the addition of a guanine nucleotide analog to the system. These results directly showed that opioid receptors, at least the delta-type, could interact with Gi.

[Combination chemotherapy including cisplatin in lung cancer by bronchial artery infusion].

Chemotherapy with bronchial artery infusion (BAI) was given to 34 patients with primary lung cancer. Treatment regimens usually employed cis-diammine-dichloroplatinum (CDDP) plus peplomycin for squamous cell carcinoma, and CDDP plus vindesine for adenocarcinoma. The provisional therapeutic effects were evaluated roentgenographically with reference to histological type, T factor and degree of vascularization. Out of 10 cases of squamous cell carcinoma, 7 cases (70%) showed tumor regression greater than 50%, in contrast to 4 of 17 cases (23.5%) of adenocarcinoma. The effects in cases of squamous cell carcinoma were correlated with tumor vascularity. Twenty-two surgically treated cases were examined for the histological effects of BAI. Five of 6 cases (83.3%) of squamous cell carcinoma showed IIb effects by Shimosato's criteria. These results showed that the therapeutic effect of BAI was excellent in cases of squamous cell carcinoma in comparison with cases of adenocarcinoma. Serious side effects including esophago-bronchial fistula, massive hemoptysis and esophageal ulcer were observed in 4 cases.
