Scintiscanning demonstration of thymoma: comparative study on scintiscans using 201Tl, 67Ga and 75Se.

Thymus scintigraphy was performed using 201Tl-chloride, 67Ga-citrate and 75Se-selenomethionine on 30 thymoma patients with or without myasthenia gravis. Mass negativity was observed in 6 out of 17 (35.3 per cent) and 3 out of 13 cases (23.1 per cent), respectively. A rate of 70 per cent (21 cases out of 30) of mass positivity was observed by thymus scan using 201Tl. With regard to the relation between thymus scan and cell type, 201Tl-scan exhibited a high rate of mass positivity, regardless of the cell type while the 75Se-scan showed a trend toward mass positivity in epithelial cell predominant cases. With 201Tl, mass positivity was observed when the CPM/g ratio for tumors and blood exceeded 3.0. This trend can serve as an index for the suitability of supplementary chemo- and radiotherapies, as well as for prognosis in cases of relapse, and in those for whom excision was not complete.

Liver morphology in marmosets infected with epidemic non-A, non-B hepatitis in India.

In this study the morphological changes in the livers of marmosets inoculated with stool extracts from epidemic non-A, non-B hepatitis patients in India were examined. The histologic changes of epidemic non-A, non-B hepatitis in marmosets consisted mainly of round cell infiltration in the portal tracts, spotty liver cell necrosis, sinusoidal lymphocyte infiltration, and Kupffer cell mobilization. By electron microscopy, liver cells from infected marmosets showed cisternal dilation of the endoplasmic reticulum, irregularly-shaped nucleus, and disorganization of the mitochondrial cristae. In some areas interaction of lymphocytes with hepatocytes was observed. Similar observations have been made in type B hepatitis infection, presumably due to liver cell damage mediated by immune mechanisms. The result of our study is also compatible with the interpretation that the liver cell damage in this experimental model may be mediated by immune mechanisms.

Pancreatic hypersecretion in dogs with obstructive jaundice.

We examined the hypersecretory function of the isolated, blood-perfused pancreas of dogs with experimentally obstructive jaundice. There was no difference in basal pancreatic secretion between control dogs and obstructively jaundiced dogs. Secretin- or pancreozymin-stimulated increase in total values for pancreatic juice flow, bicarbonate, protein and amylase were greater in dogs with obstructive jaundice than in control dogs; however, concentrations of bicarbonate, protein and amylase were unchanged in each group. Acetylcholine-stimulated increase in total values and concentrations of these factors were the same in dogs with obstructive jaundice and controls. These data suggest that hypersensitivity of the pancreas to secretin and pancreozymin may be one of the main mechanisms of pancreatic hypersecretion.

[A case of bilateral metastatic breast carcinoma from gastric carcinoma].

The patient was a 48-year-old woman who had undergone radical surgery for gastric carcinoma in 1981. Seventeen months after the operation, recurrence of gastric carcinoma at the anastomotic stoma was found, and total gastrectomy was performed. Fourteen months after the second operation, she was revealed to have bilateral breast tumors and underwent exploratory excision. Histological examination suggested primary malignant tumor of the breast. Bilateral standard radical mastectomy was done seven days after the excision. Final histological examination revealed metastatic breast cancer originating from the stomach. She died from general metastasis seven months after the radical mastectomy in spite of adjuvant immunochemotherapy.

[RI angiography with 99mTc and 201T1-chloride scintigraphy in determining the effectiveness of intra-arterial infusion in chemotherapy of locally advanced and recurrent breast cancer].

Eleven patients with local advanced and recurrent breast cancer were treated preoperatively with intraarterial infusion of chemotherapeutic agents. After the treatment, nine patients underwent radical mastectomy. RI angiography with 99mTc is a useful procedure for determining the correct position of the tips of an inserted cannula. The results of 201Tl-chloride scintigraphy have been found to be closely consistent with the results of the resected specimen histologically. Therefore, 201Tl-chloride scintigraphy is also a quite useful procedure for determining the effects of intraarterial infusion in cancer chemotherapy.

[Medical images; the clinical significance of the medical images of thyroid cancer].

Supplementary diagnosing methods for thyroid cancer were discussed, with particular reference to medical images. Although palpation is, of course, essential in the examination of nodules, we have also used soft X-rays and I-123 scintigraphy, as well as Tl-201 scintigraphy, ultrasonography and aspiration biopsy. In addition, various diagnosing methods have been employed to determine the degree of invasion in advanced cancer patients, such as CT scan for observing invasion of the surrounding tissues, naso-tracheoscopy for invasion of the trachea and a flow meter for vascular tracts. When the tumor size is under 2 cm, we use soft X-rays, ultrasonography and sometimes try additional aspiration biopsy. When the tumor is smaller, aspiration biopsy is employed with ultrasonography. For tumors larger than 2 cm, we use soft-X-rays, I-123 scintigraphy and ultrasonography. Tl-201 scintigraphy is employed to determine operative indication, following which aspiration biopsy and CT scan are performed. CT scan, tracheofiberscopy and doppler flow studies are carried out for advanced cancer cases to determine the degree of invasion of carcinomas.

[Distribution pattern of fibronectin in human lung cancer tissue].

The distribution pattern of fibronectin in human lung cancer tissues has been studied using the indirect immunoperoxidase technique. In adenocarcinoma fibronectin is observed in the basal side of cancer cells and tumor stroma. In squamous cell carcinoma, it is seen in the basal side of cancer cells, surface of cancer cells and tumor stroma. The difference between adenocarcinoma and squamous cell carcinoma in invasion and metastasis was investigated to have some relation to the distribution pattern of fibronectin.

[Advanced thyroid carcinoma--possible induction of the host's antitumor immunological response by 131I therapy].

UNLABELLED: The patient was a 76-year-old man. When he was 31, he had undergone right lobectomy for thyroid cancer. Following this, had repeated recurrences, and finally reached total-thyroidectomized status. Consequently, he was given thyroxin and PS-K thereafter. When he was 75, 131I therapy was undertaken for lung metastases. The changes of serum anti-microsomal antibody (AMA), immunosuppressive substance (ISS) and thyroglobulin (TG) were as follows: three months after the therapy, AMA was 40(2), although it had always been negative before. Nine months later, ISS and TG markedly decreased. CONCLUSION: Under the administration of immunomodulating agents, 131I therapy might induce antitumor immunological responses by the effect of enhancement of cancer cell antigenicity.

[Problems in the treatment of early cancer in major organs--thyroid cancer].

In order to establish the concept of early carcinoma of the thyroid and the operative method, 97 patients with cancer of the thyroid measuring less than 1cm in maximum diameter were studied from among 939 patients with thyroid cancer treated at the Second Department of Surgery, Shinshu University School of Medicine, between 1953 and 1984. After excluding occult carcinoma, minimal carcinomas measuring less than 0.5 cm almost never showed metastases to lymph nodes or distant sites, being therefore, equipped with conditions required for early cancer. Since the detection of minimal carcinomas measuring less than 0.5 cm is difficult, however, problems arise as to its clinical application. The prognosis of minimal carcinomas measuring less than 1cm, on the other hand, showed no remarkable difference from that of carcinomas measuring less than 0.5 cm. At present, consequently, minimal carcinomas measuring less than 1cm after exclusion of occult carcinoma may be classified as early carcinoma. No removal of regional lymph nodes is required for carcinoma measuring less than 0.5 cm in the operative procedure. For minimal carcinomas measuring between 0.5 and 1.0 cm, it is necessary to perform modified neck dissection. Strict follow-up observation is necessary for cancers under the histopathological diagnosis of encapsulated type (invasive type) or infiltrative type.

In vitro study on release of thyroid hormone in solitary autonomously functioning thyroid nodules using cell culture method.

In vitro release of thyroid hormone was investigated under basal and TSH-stimulated conditions in the solitary autonomously functioning thyroid nodules (AFTN). A small portion (0.5 g of wet weight) of the nodules and adjacent thyroid tissues removed surgically from five patients with solitary AFTN were prepared for the dispersed cell culture. In the experiment on non TSH-stimulated (basal) conditions, those culture media which were totally replaced on the 5th day after primary culture were utilized for the determination of thyroxine (T4) and triiodothyronine (T3) by radioimmunoassay. T4 and T3 levels in culture media of the functioning nodules were 1.15 +/- 0.33 microgram/dl (mean +/- SEM) and 2.72 +/- 0.68 ng/ml, contrasted with levels of 0.67 +/- 0.09 microgram/dl and 1.24 +/- 0.22 ng/ml in the paranodular tissues. The mean ratios of T3/T4 of the nodules and paranodular tissues were 0.25 +/- 0.02 and 0.19 +/- 0.02, respectively (p less than 0.05). Meanwhile, in another experiment under TSH stimulatory conditions employing 40 and 80 microU/ml of human TSH, there were no significant differences in T4 and T3 releases when the two groups were compared.

[Clinical evaluation of ototoxicity associated with intravenous drip infusion of amikacin].

On the 53 patients with ENT (ears, nose, and throat) diseases, ototoxicity after intravenous drip infusion of amikacin (Biklin AMK) was studied. Each dose of AMK was 400 mg/day in adults and 4--8 mg/kg/day in children. From audiometric analysis, there were no patients with any hearing disturbances and subjective complaints concerning labyrinth injury. Also abnormal laboratory findings were not found in them. An intravenous drip infusion of AMK in adequate dosage would be beneficial to use against some infectious diseases of otorhinolaryngologic field.

Gastric carcinoma and intestinal metaplasia. Significance of types of intestinal metaplasia upon development of gastric carcinoma.

For clarifying significance of subtypes of intestinal metaplasia upon the development of stomach carcinoma, 34 stomachs surgically removed for mucosal carcinoma were morphologically studied. Intestinal metaplasia was divided into incomplete and complete types by alcian blue-PAS staining. The incomplete type has been understood as an immature stage of the development of the metaplasia as previously reported by us. In this, study, it was revealed that the incomplete type of metaplasia showed more dysplastic features than the complete type. Analysis of the metaplasia of the gastric mucosa surrounding cancer tissue revealed that well differentiated adenocarcinoma frequently came into contact with the incomplete type of metaplasia in small cancer, less than 1 cm. This relationship became less evident as carcinoma became larger. In poorly differentiated carcinoma, however, any correlations between types of metaplasia and size of cancer were not demonstrated. From these results, it was suggested that well differentiated adenocarcinoma arises from the incomplete type of metaplasia.
