[Effects of melatonin alone and in combination with aceten on chronostructure of diurnal hemodynamic rhythms in patients with hypertension stage II].

AIM: To study effects of monotherapy with melatonin and combination melatonin + aceten on circadian pattern of hemodynamics in patients with essential hypertension stage II. MATERIALS AND METHODS: 20 EH patients were randomized into 2 groups, 10 patients each. Group 1 patients were given melatonin alone (6 mg at 22.00). Group 2 received aceten (25 mg at 18.00) and melatonin (6 mg at 22.00). Before treatment and after 10 days of it, hemodynamic parameters were measured each 3 hours for 24 hours. Arterial pressure was measured by Korotkov method, heart rate--by ECG R-R interval, stroke volume--by tetrapolar chest rheography according to Kubichek. Cardiac index, total and specific peripheral resistance, double product were estimated routinely. RESULTS: Internal and external dyssynchronism was registered in hemodynamic parameters of EH patients. Melatonin normalized circadian hemodynamic rhythms. It produced a hypotensive effect, reduced energy requirements of the myocardium. Combination of melatonin with aceten had a hypotensive and vasodilating effects but failed to restore circadian hemodynamic rhythms. CONCLUSION: Melatonin, epiphysial neurohormone, and its combination with aceten have hypotensive and vasodilating effects but melatonin monotherapy normalizes circadian hemodynamic rhythms while the combination was uneffective in this respect. This may be explained by interaction of melatonin and aceten actions.

[Comparative study of the effectiveness of Cozaar monotherapy and Cozaar and melatonin combined therapy in aged patients with hypertension].

Effectiveness of Cozaar monotherapy (50 mg at 8 a.m.) was compared to that of Cozaar combination with melatonin (3 mg before sleep) in 21 patients with hypertension stage II (mean age 62 years). Clinical symptoms and hemodynamic parameters were evaluated with ECG and blood pressure monitoring. The findings were processed with statistical methods and Kosinor-analysis. Cozaar alone had moderate hypotensive and vasodilating effects, affected circadian rhythm of systolic and diastolic blood pressure, heart rate. Combination Cozaar + melatonin reduced blood pressure more noticeably, diminished cardiac output, energy consumption of the myocardium, normalized normal chronostructure of blood pressure and heart rate circadian rhythms.

[Effects of disturbances of natural magnetic field of the Earth on melatonin production in patients with coronary heart disease].

The obtained results clearly evidence for the influence of magnetic disturbances and storms on melatonin production in patients with ischemic heart disease. Improvement of 24-hour rhythm of melatonin secretion during the period of magnetic disturbances says in favour of this influence while an overall fall of melatonin production during magnetic disturbances and storms is an apparent negative factor.

[Emotional stress and blood melatonin levels].

Blood serum melatonin concentrations were measured in male Wistar rats exposed to acute water-immersion emotional stress (ES) during intraperitoneal injections of physiological saline or different doses of melatonin. ES increased blood melatonin levels in rats receiving physiological saline 1 hour before or just after ES. When physiological saline was administered just ES, blood melatonin concentrations remained unchanged. The blood levels of melatonin both in control and stressed animals injected with exogenous melatonin were higher than those in rats given physiological saline alone. Immersion ES decreased blood melatonin concentrations in rats receiving different exogenous melatonin doses just before ES. However, in rats given melatonin, 1.0 mg/kg, after ES, its blood levels after ES was higher than those in the animals unexposed to stress. The findings suggest that it is just exogenous rather than endogenous melatonin that is consumed in rats injected with its different doses just prior to immersion ES.

[Hierarchical thermodynamics and gerontology].

The phenomenon of ageing of living creatures may be studied in the context of hierarchic thermodynamics. Ageing (ontogenesis) is regarded as a process of formation of the body's submolecular structure. A concept of the degree of ageing (the completion of ontogenesis) which is evaluated from changes in the Gibbs specific function of formation of the submolecular structure of the body's biological tissues is introduced. Quantitative criteria are proposed to find differences between the chronological and thermodynamic age of biological objects. The experimentally based thermodynamic approach reveals the impact of environmental parameters on longevity. The new discipline thermodynamic gerontology which will, in terms of the physical theory, substantially affect human longevity may be hoped to be formed in the coming years.

[Effects of smoking and coffee consumption on indicators of arterial pressure in 24-hour monitoring].

27 essential hypertensives were included in the study to evaluate the influence of cigarette smoking and coffee drinking on 24-hour blood pressure and its variability. The patients were untreated with antihypertensive drugs for at least 4 weeks. All the patients were coffee drinkers (4-5 cups a day) and cigarette smokers (during 25.2 +/- 7.4 years before study and about 10-20 times a day during last year). 24-hour ambulatory blood pressure monitoring (ABPM) was performed 3 times a week at one-day intervals. During the first and third ABPM the amount of coffee and cigarettes as well as schedule of both were equal, during the 2nd ABPM-smoking and coffee were prohibited. There were no significant differences between mean BP values and variability during the 1st and 3d ABPM. Variability of 24-h and daytime systolic blood pressure (SBP) was higher in the 1st--and 3d vs. 2nd ABPM. The 2nd ABPM revealed a significant SBP decrease at daytime (152.5 +/- 9.2 mm Hg vs. 147.2 +/- 8.9 mm Hg, p < 0.05) without changes in nighttime as well as in the whole 24-h period. Values of diastolic BP did not change. The results suggest that cigarette smoking and coffee drinking increase mean daytime and overall 24-hour SBP values and variability as well as heart rate, while diastolic BP is more stable.
