Induction of antigen presentation by macrophages after phagocytosis of tumour apoptotic cells.

Due to their resistance to classical chemotherapies, most human colorectal cancers have a high incidence and a poor prognosis. Immunotherapy using interleukin 2 (IL2) has provided disappointing results in the treatment of these cancers. Recently, however, we have demonstrated that a treatment combining a cell-differentiating agent, sodium butyrate (NaBut) with IL2 resulted in a remission of established peritoneal colorectal carcinomatosis in rats. Separately, neither NaBut nor IL2 treatment cured these tumour-bearing rats. NaBut is known to induce cell differentiation and subsequent apoptosis in epithelial cells, while IL2 stimulates the immune cells capable of participating in tumour rejection. We postulated that the significant therapeutic effect of NaBut/IL2 treatment could be attributed to a NaBut-induced increase in the immunogenicity of the cancer cells. We report here that NaBut induced an apoptotic process in rat colon tumour cells in vivo and in vitro. We observed, in an efficient cure, colocalization of apoptotic bodies and monocytes/macrophages at the periphery of the tumour. We propose that these apoptotic bodies are phagocytosed in vivo by the macrophages. We also showed in vitro that a subpopulation of macrophages involved in the phagocytic clearance of apoptotic cells expresses cell surface molecules associated with antigen presentation and stimulates the proliferation of naive splenocytes. Our data suggest that therapies that recruit massive induction of the apoptotic process in tumour cells could favour tumour antigen presentation via their specific phagocytosis by antigen-presenting cells (APCs). We propose that the development of specific therapies that stimulate both tumour cell apoptosis and the immune system could offer new opportunities in anti-cancer treatments of poorly immunogenic cancer cells.

Mechanical properties of skin in Ehlers-Danlos syndrome, types I, II, and III.

Mechanical properties of skin were evaluated in vivo in 17 children suffering from Ehlers-Danlos syndrome (EDS) types I, II, and III. These were compared with normal values from 63 healthy children. Noninvasive measurements were performed under suction of 500 mbar using a Cutometer equipped with a 4-mm probe. Prominent increases in skin extensibility and elasticity were the most distinctive and diagnostic features of all three EDS types. Differences in the average values of biomechanical variables were present among the three types. Patients with EDS-I were the most affected, whereas those with EDS-III had virtually normal skin. However, interindividual differences in the severity of rheological alterations were found within each type, illustrating a continuum in the variation of mechanical properties of EDS skin rather than yielding step differences among the types.

Penetration enhancers assessed by corneoxenometry.

Chemical enhancers abating the barrier function of the stratum corneum act in a number of ways. Their relative effects in mixtures are notoriously difficult to determine. Corneoxenometry is a bioassay where xenobiotic interactions with corneocytes are assessed using reflectance colorimetry. It was used to evaluate comparatively the effects of ethanol and laurocapram at various dilutions. In addition, a 10% propylene-glycol-based gel supplemented with N-acetyl-L-cysteine, urea and salicylic acid in various concentrations was tested. The relative index or irritancy (RII) was calculated for each formulation. RII is a surrogate marker of lipid removal and disorganization, and of protein denaturation in the stratum corneum. It is likely a good predictor for barrier function alterations and represents a cheap and rapid alternative to animal testing.

Postural skin colour changes during the corticosteroid blanching assay.

OBJECTIVE: To assess the effect of the position of a limb (dependency) on the cutaneous vasoconstrictor (blanching) assay (VCA) using topical corticosteroid preparations. METHOD: Two studies were performed on the forearms of healthy volunteers using tristimulus reflectance colorimetric assessments. The first one conducted in 60 normal adults aimed at quantifying the range of variation in the skin chromaticity a* when the arm was successively positioned horizontally and vertically, either in the upward or downward direction. In the second study, 16 volunteers were selected according to a weak spontaneous postural dependency of the limb in a* (<2 units). The blanching effect of 0.1% mometasone furoate, 0.1% betamethasone valerate and petrolatum were compared to the colour of an untreated site. Chromaticity a* and the colorimetric variable (DeltatL*(2) + Deltata*(2))(0.5) were measured in time following a 2-hour application. RESULTS: The postural variations in skin colour varied among subjects. The majority of them (80%) presented a difference in a* higher than 2 units between the upright and downward arm positions. The upright position appeared to be the most sensitive to show significant differences in the VCA. Mometasone furoate exhibited the most prominent effect, significantly higher than betamethasone valerate and controls. CONCLUSION: The corticosteroid VCA is influenced by the position of the limb. The upright position increases the sensitivity of the test. Mometasone furoate is more potent than betamethasone valerate.

A randomized, placebo-controlled trial of topical retinol in the treatment of cellulite.

BACKGROUND: Cellulite occurs to varying degrees on the thighs and buttocks of many otherwise healthy women. Among the many purported treatments for cellulite, only a handful have been tested in clinical trials. OBJECTIVE: The aim of this study was to critically explore the reputed effect of topical retinol in the treatment of cellulite. MATERIALS AND METHODS: The study compared the effect of topical retinol to a placebo formulation in a left-right randomized trial in order to eliminate the massage-effect. The study was conducted in 15 women aged from 26 to 44 years who had requested liposuction to improve mild to moderate cellulite. RESULTS: After 6-months of treatment, skin elasticity was increased by 10.7% while viscosity was decreased by 15.8% at the retinol-treated site. Such an effect on the tensile properties of skin was more prominent where the mattress phenomenon was the only evidence of cellulite. The lumpy-bumpy appearance of the skin showed either little response or was not responsive to the treatment. Although gross microanatomical differences were not disclosed between the comparative sites at completion of the study, evidence for a shift in the phenotype of connective tissue cells was obtained. The main retinol-related change consisted of a 2- to 5-fold increase in the number of factor XIIIa+ dendrocytes both in the dermis and fibrous strands of the hypodermis. CONCLUSIONS: We hypothesize that the functional and phenotypic changes seen in this study were linked and represent the result of a direct or indirect modulating effect of retinol on cellulite. Such features ultimately improve the resting tensions inside the skin which should in turn smooth the skin surface.

[Differential diagnosis of a red nose].

A red nose is a semiological sign shared in common by a series of diseases. The differential diagnosis is important to establish. A careful clinical examination often allows to reach the precise diagnosis. A skin biopsy is required when erythema is accompanied by a cutaneous infiltration suggesting some granulomatous and tumoral processes.

[How I prevent erysipelas and its consequences and recurrences].

Erysipelas is a serious infection of the skin. In case of delay in initiating adequate antibiotic treatment, complications, sometimes dismal, can supervene. In addition, erysipelas shows a tendancy to recurrences. The prevention of an episode of erysipelas calls for correct personal hygiene and adequate use of topical antiseptics in case of skin effraction, even when minimal. When erysipelas is established, a rapidly initiated antibiotic treatment for a prolonged period prevents streptococcal gangrene complications. Elastic contention of any leg edema from venous or lymphatic origin and prophylactic antisepsis of discrete wounds help in preventing erysipelas recurrences.

[Interest of percutaneous biliary drainage following percutaneous transhepatic cholangiography in certain obstructive jaundice (author's transl)].

In addition to the well-known usefulness of percutaneous transhepatic cholangiography, that procedure demonstrates a supplementary interest in certain special cases of dilated biliary tract when it is associated with a external biliary drainage; that drainage is performed by means of a method designed after Seldinger's percutaneous catheterization. That single two-step procedure is a sparing transient palliative policy for certain types of obstructive jaundice; it also permits a more comfortable survival when surgery cannot be achieved.

[The diagnostic value of radiological examinations in Conn's syndrome: angiographic findings in 58 cases (author's transl)].

Selective adrenal phlebography with measurement of hormone levels was used to investigate the majority of 58 cases of primary hyperaldosteronism. The authors stress the importance of pre-operative diagnosis of the lesion as surgery is indicated for adenomas only, and because of their small size they are often missed in adrenal scintigrams and on manual exploration during operation. They are in agreement with most other authors that adrenal phlebography is the most valid investigation, as it gives a precise diagnosis of the lesion in 92% of cases. The angiographic findings in the most common lesions are described, the difficulty in diagnosing peripheral small adenomas stressed, and the problems associated with the diagnosis of nodular hyperplasias outlined. They also underline the fundamental importance of selective hormone levels measurement, as these results are of equal value to those of the images themselves.
