UNLABELLED: From January 1985 to December 1986 one hundred and five patients (145 lower limbs) have been treated by redosurgery followed by sclerotherapy. It is a selected group because redosurgery decision was taken both by surgeon and angiologists on the basis that a significant reflux between the profunda system and the superficial system was present. The aim of this retrospective study was to assess results obtained by this therapeutic association with a follow-up from 5 to 6 years. Failure mechanisms of initial treatment, symptoms, preoperative investigations, redosurgery procedures are analysed. Results were estimated both by a questionnaire and an independent audit. Patient estimation: symptoms improvement 65%; cosmetics improvement 68%. Audit estimation: SYMPTOMS: excellent or good results: 78%, fair results: 12%, poor results: 10%. Cosmetics: excellent or good results: 30%, fair results: 53%, poor results: 17%. 82% of patients plebiscited redosurgery in cooperation with sclerotherapy.
The susceptibility to ampicillin and trimethoprim-sulfamethoxazole (TMP-SMZ) was determined for 15,824 isolates of Shigella obtained from patients attending a treatment center in Dhaka, Bangladesh, from 1983 through 1990 and for 520 isolates obtained during community surveys from 1988 through 1990. Susceptibility to nalidixic acid was determined for isolates obtained after 1985. In 1983 13% of isolates were resistant to ampicillin, 23.5% to TMP-SMZ, and 0.8% to both drugs. By 1990 51.2% of isolates obtained at the Diarrhea Treatment Centre were resistant to ampicillin, 47.7% to TMP-SMZ, and 40.5% to both drugs (for comparison with figures for 1983, P less than .001). Resistance to nalidixic acid increased from 0.8% in 1986 to 20.2% in 1990 (P less than .001). In 1990 71.5% of Shigella dysenteriae type 1 isolates were resistant to ampicillin, 68.5% to TMP-SMZ, 67.7% to both drugs, and 57.9% to nalidixic acid. The resistance pattern of isolates obtained during community surveillance was similar to that of Treatment Centre isolates. In Bangladesh ampicillin and TMP-SMZ are no longer useful for treatment of infection with any species of Shigella, and nalidixic acid is no longer useful for treatment of infections due to S. dysenteriae type 1.

In contrast with the usual electric spark gap scalpels which burn and carbonise tissues, making them unsuitable for section or fine dissection, Surgitron, which uses high frequency radio waves, cuts and coagulates without excessive heating and without carbonisation. It possesses 4 types of current, all with adjustable power levels: section, section-coagulation, coagulation and fulguration. It can be used with ordinary scalpel blades or with the electrodes supplied with the apparatus. It can be used in all stages of the operation: skin incision, fine dissection, coagulation and allows an appreciable gain in time combined with great comfort for the user and for the patient with a reduction in postoperative oedema.

The repair of large chest wall defects has been done on 23 patients who had 28 operations. Twenty-two patients had a neoplasm of the thoracic cage, while 1 had a large inflammatory mass. Nine patients had a partial lung and 3 a partial diaphragmatic resection done en bloc with the ribs. Seven resections were on the sternum. The repair was made either with a soft prosthesis (nylon mesh in 3 and Marlex mesh in 12 operations) or with a composite prosthesis of methyl methacrylate and metallic or Marlex mesh (13 operations). The association of methyl methacrylate and Marlex combines the solidity and the easy shaping of methyl methacrylate with the advantages of the mesh for an easy fixation and excellent incorporation. When the skin and the muscles are resected with the osteocartilaginous wall, an omentum flap is placed between the skin and the prosthesis to facilitate healing. There was one postoperative wound infection, which cleared with appropriate antibiotics. No prosthesis has extruded. The cosmetic and functional results are satisfactory. Repair of very large chest wall defects after resection can be done safely.

Clots of bovine fibrin, with both coarse and fine structure, and ligated to different extents by fibrinoligase, have been broken up by ultrasonic agitation and the sonicates have been examined by ultracentrifugal sedimentation. Sonication is followed by gross aggregation of the fragments unless guanidine hydrochloride is introduced (order of 1 M). In that case, sonicates of gamma-ligated fine clots contain two species whose sedimentation coefficients correspond to fibrin monomer and an oligomer with twice the monomer cross-section area and at least 20 monomer units, presumably with the structure of lateral dimerization with staggered overlapping. If the gamma ligation is incomplete, shorter oligomers are identified. The monomer and oligomer with degree of polymerization greater than 20 appear also in sonicates of coarse clots, but in smaller amounts, the principal product consisting of larger aggregates. The implications of these results with respect to metastability of the fine clot and the pattern of polymerization are discussed.
On the basis of two cases, one a fistula and the other a cyst of the first brachial cleft with chiefly parotid manifestations, a review of the embryology is given which explains the anatomical features. The elements in their clinical diagnosis, complemented by fistulography and sialography are compared with operative findings, the essential steps in which are as follows: 1) Identification of the facial nerve which by necessity overlies the lesions. 2) Partial resection of the superficial lobe of the parotid. 3) The essential resection of the external auditory meatus, the keystone of radical cure of these cysts and fistulae.

The utilization technique has been slightly modified with use. At present, the glue is prepared in advance and kept in closed 5 ml. syringes. Formalin is no longer applied in the liquid form by means of a syringe and needle; but only with Spongel swabs impregnated with formalin and cut to the size of the bleeding point. This prevents the formalin from leaking on to the mucosae and burning them.
