Comparison of decentration and tilt between one piece and three piece polymethyl methacrylate intraocular lenses.

BACKGROUND: The extent of the decentration and tilt was prospectively compared between one piece polymethyl methacrylate (PMMA) and three piece PMMA intraocular lenses (IOLs) which were implanted in the capsular bag after performing continuous curvilinear capsulorhexis. METHODS: 91 patients underwent a one piece PMMA IOL implantation in one eye as well as the implantation of the three piece PMMA IOL with polyvinylidene fluoride loops in the opposite eye. The length of the lens decentration and the angle of the tilt were quantitated using the anterior eye segment analysis system (EAS-1000) at 1 week as well as 1, 3, and 6 months postoperatively. RESULTS: The mean length of the decentration in the one piece IOL was smaller than that in the three piece IOL at 1 week (p = 0.0092), 1 month (p = 0.0044), 3 months (p = 0.0069), and 6 months (p = 0.0010) postoperatively. However, no significant difference was found in the degree of the tilt between the two types of IOLs throughout the observation periods. CONCLUSION: These results clarified that the one piece PMMA IOL with rigid PMMA haptics implanted in the capsular bag provides a better centration than the three piece PMMA IOL with flexible haptics, whereas the tilt was the same between the two types of IOLs.

Malignant insulinoma with extensive liver metastases presenting as disturbance of consciousness.

A 56-year-old man was referred to our hospital for evaluation of episodic disturbance of consciousness. Hypoglycemic symptoms were noted and Whipple's triad was satisfied. The 75 g OGTT and the glucagon test revealed a high baseline insulin level and hyperreactivity to glucagon. A pancreatic tumor and liver metastases were found by abdominal computed tomography (CT). Based on the finding of liver biopsy, the final diagnosis was malignant insulinoma with liver metastasis. He selected conservative treatment and no hypoglycemic crisis has occurred for one year since discharge. Early diagnosis and long-term follow-up is necessary since this tumor is slow growing.

Partial deficiency of hypoxanthine-guanine phosphoribosyltransferase manifesting as acute renal damage.

A 32-year-old man who had had frequent gouty arthritis over the past 17 years, was admitted for acute renal failure. Acute renal failure was improved rapidly after medication was resumed and the patient was sufficiently hydrated. The hypoxanthine-guanine phosphoribosyltransferase (HPRT) activity in the patient had been reduced to about 30% of the normal control. Therefore we considered that this patient suffered from a partial deficiency of HPRT. A point mutation of HPRT gene 68G (guanine) to T (thymine) was detected. This is a mutation that has not been previously reported. Familial analysis indicated that his mother and sister were heterozygotes.

Effect of posture change on control of ventilation.

To clarify the control mechanism of ventilation during posture change, ventilatory parameters, PETCO2, and ventilatory response to CO2 were examined in 11 healthy male subjects at supine (0 degrees) and 75 degrees head-up tilt positions. Minute expiratory ventilation (V.E), tidal volume (VT), respiratory frequency (f), end-tidal and transcutaneous PCO2 and CO2 output (V.CO2), and ventilatory response to CO2 were measured during a steady state condition. V.E (V.A) and VT increased significantly at 75 degrees tilt with significant decrease in PETCO2 from 40.1 mmHg (0 degrees) to about 36.1 mmHg (75 degrees). Transcutaneous PCO2 also decreased during tilt, by 3.3 mmHg. Physiological dead space (VD/VT) and V.CO2, however, remained unchanged, and ventilatory equivalent (V.E/V.CO2, V.A/V.CO2) increased significantly. The CO2-ventilatory response curve shifted upward (or leftward) without significant change in the response slope. At 75 degrees tilt, EMG activity of gastro-cnemius muscle increased. These findings suggested that PETCO2 decreased because of increased V.E (V.A) with a leftward shift of CO2-ventilatory response curve. Various signals such as afferents from lower extremities might have net stimulatory effects on a CO2-ventilation control system to reset the controlled level of PETCO2 to a lower range, but without significant change in CO2-ventilatory response during upright position.

[Renal damage in a chronic active hepatitis C patient receiving interferon-alpha therapy].

A 68-year-old male patient with chronic active hepatitis C was treated with interferon-alpha (IFN-alpha) for a period of 5 months. The patient responded well to the IFN therapy showing substantial improvement in liver function and disappearance of HCV-RNA. However, one year after the treatment he was found to have developed proteinuria and showed a reduction in Ccr. Renal biopsy findings were as follows: Light microscopy showed diffuse expansion of mesangial cells with a focal/local increase in cellularity accompanied by capillary loop thickening. Splitting of the basement membrane was also present. An immunofluorescent study showed that IgA was localized predominantly in the peripheral capillary wall. Electron microscopy showed that there was mesangial cell interposition between the peripheral capillary wall and endothelial cells. Furthermore, endothelial cells were expanded and numerous platelets were seen in the capillary lumen. These findings were compatible with focal MPGN accompanied by activation of endothelial cells. These histological data suggest two clinical disease entities: late-onset renal damage induced by IFN-alpha alone, and HCV-induced renal damage possibly modified by the direct effect of IFN-alpha on the endothelium. The present case suggests that IFN therapy for HCV may produce a particular type of renal damage, under the influence of either IFN or HCV infection, and/or both.

[Preventive effects of diclofenac ophthalmic solution on post-cataract-surgical cystoid macular edema].

Using eyes undergoing phacoemulsification followed by implantation of a foldable acrylic intraocular lens (IOL) designed for small-incision cataract surgery, a multi-center study was performed to compare a non-steroidal ophthalmic solution (0.1% diclofenac) to a steroidal ophthalmic solution (0.1% fluorometholone) in preventing cystoid macular edema (CME) and inducing disruption of the blood-aqueous barrier determined by laser flare cellmetry. The incidence of CME, noted in 3 out of 53 eyes (5.7%) receiving diclofenac and in 29 out of 53 eyes (54.7%) receiving fluorometholone, was significantly lower in the diclofenac group. The flare in the anterior chamber was also significantly lower in the diclofenac group; when compared between eyes with and without CME, the amount of flare was significantly higher in the former group. These findings suggest that diclofenac effectively prevents CME following cataract surgery and that CME is closely related to the breakdown of the blood-aqueous barrier.

Uptake of D- and L-serine in C6 glioma cells.

Cultured C6 glioma cells were able to accumulate [3H]D- and [3H]L-serine in a temperature- and Na+-dependent and saturable manner. The kinetic analysis of these accumulation phenomena indicates that the D- or L-serine uptake into the glioma cells might occur by a single-component system with an apparent Km value around 2480 microM (for D-serine) or 110 microM (for L-serine) and a Vmax value around 7 nmol/min per mg protein. The ratio of the L- to D-serine uptake affinity was similar to that previously reported in the astrocytes of rat cerebrum. The inhibition profiles of D- and L-serine uptake by various amino acid and related compounds resembled each other and those of the ASCT-like neutral amino acid transporter. The present findings therefore suggest that C6 glioma cells may accumulate D- and L-serine through a common ASCT-like transporter and provide a useful model for the glial uptake of extracellular D- and L-serine in the brain.

Phosphatidylinositol 4,5-bisphosphate stimulates phosphorylation of the adaptor protein Shc by c-Src.

The adaptor protein Shc was prepared as glutathione S-transferase fusion proteins (GST-Shc) and used as in vitro substrate for c-Src. Since phosphotyrosine-binding domain of Shc has been shown to bind phosphatidyl-inositol 4,5-bisphosphate (PtdIns(4,5)P2) [Zhou et al. (1995) Nature 378, 584-592], effect of PtdIns(4,5)P2 on the phosphorylation of GST-Shc by c-Src was examined. PtdIns(4,5)P2 stimulated the phosphorylation of GST-Shc without any effect on the c-Src activity as judged by both its autophosphorylation and phosphorylation of exogenous substrate, Cdc2 peptide. On the other hand, phosphatidylserine, phosphatidic acid, phosphatidylinositol, and phosphatidylinositol 4-phosphate but not phosphatidylcholine stimulated the c-Src activity itself. Km for GST-Shc in the presence of 1 microM PtdIns(4,5)P2 was calculated to be 90 nM. The PtdIns(4,5)P2-dependent phosphorylation of GST-Shc was inhibited by a GST-fusion protein containing the phosphotyrosine-binding domain of Shc. These results suggest that PtdIns(4,5)P2 can act as a regulator of phosphorylation of Shc by c-Src through its binding to Shc.

Residues within the polycationic region of cGMP phosphodiesterase gamma subunit crucial for the interaction with transducin alpha subunit. Identification by endogenous ADP-ribosylation and site-directed mutagenesis.

Interaction between the gamma subunit (Pgamma) of cGMP phosphodiesterase and the alpha subunit (Talpha) of transducin is a key step for the regulation of cGMP phosphodiesterase in retinal rod outer segments. Here we have utilized a combination of specific modification by an endogenous enzyme and site-directed mutagenesis of the Pgamma polycationic region to identify residues required for the interaction with Talpha. Pgamma, free or complexed with the alphabeta subunit (Palphabeta) of cGMP phosphodiesterase, was specifically radiolabeled by prewashed rod membranes in the presence of [adenylate-32P]NAD. Identification of ADP-ribose in the radiolabeled Pgamma and radiolabeling of arginine-replaced mutant forms of Pgamma indicate that both arginine 33 and arginine 36 are similarly ADP-ribosylated by endogenous ADP-ribosyltransferase, but only one arginine is modified at a time. Pgamma complexed with Talpha (both GTP- and GDP-bound forms) was not ADP-ribosylated; however, agmatine, which cannot interact with Talpha, was ADP-ribosylated in the presence of Talpha, suggesting that a Pgamma domain containing these arginines is masked by Talpha. A Pgamma mutant (R33,36K), as well as wild type Pgamma, inhibited both GTP hydrolysis of Talpha and GTP binding to Talpha. Moreover, GTP-bound Talpha activated Palphabeta that had been inhibited by R33,36K. However, another Pgamma mutant (R33,36L) could not inhibit these Talpha functions. In addition, GTP-bound Talpha could not activate Palphabeta inhibited by R33,36L. These results indicate that a Pgamma domain containing these arginines is required for its interaction with Talpha, but not with Palphabeta, and that positive charges in these arginines are crucial for the interaction.

Depletion of cerebral D-serine in non-ketotic hyperglycinemia: possible involvement of glycine cleavage system in control of endogenous D-serine.

Tissue concentrations of D-serine and other chiral and non-chiral amino acids were measured post-mortem in the cerebral cortex of the neonatal or infantile individuals with (three cases) or without (seven cases) non-ketotic hyperglycinemia (NKH) using high performance liquid chromatography with fluorometric detection. In the cortical tissues of the NKH patients lacking activity of glycine cleavage system, there was a marked reduction and elevation of the contents of D-serine and glycine, respectively, compared to non-NKH controls. Systemic administration of an inhibitor of glycine cleavage system (GCS), cysteamine, mimicked the changes in the cortical concentrations of these amino acids in the 8-day-old rats. Augmentation of brain glycine levels by means of intraperitoneal injection of glycine itself resulted in an increase in cortical D-serine contents in the neonatal rats with normal activity of GCS. These findings provide the first evidence that GCS might be implicated in the biosynthesis or content regulation of endogenous D-serine in the mammalian brain.

Reduction in the area of the anterior capsule opening after polymethylmethacrylate, silicone, and soft acrylic intraocular lens implantation.

PURPOSE: To report reduction in the area of anterior capsule opening at various postoperative intervals after continuous capsulorrhexis and to compare any differences in the area reduction between polymethylmethacrylate, silicone, and soft acrylic intraocular lenses. METHODS: Prospectively, 240 eyes of 240 patients undergoing phacoemulsification and intraocular lens implant surgery were randomized into three groups based on the type of intraocular lens used: polymethylmethacrylate, silicone, and soft acrylic. The area of the anterior capsule opening was quantitated, and examinations were performed at 1 week and 1, 3, and 6 months postoperatively. RESULTS: After intraocular lens implantation, the mean anterior capsule opening area 3 months postoperatively was significantly smaller than that at 1 week in all groups (polymethylmethacrylate, P = .0090; silicone, P < .0001; soft acrylic, P = .0127). Mean percentages in area reduction at both 3 and 6 months postoperatively were significantly greater than those at 1 week and 1 month (P < .0001). In comparing the three intraocular lenses at 1 week postoperatively, no statistical significance was observed regarding the anterior capsule opening area between the three groups. However, the mean anterior capsule opening areas in the polymethylmethacrylate and soft acrylic intraocular lens groups were significantly larger than those in the silicone intraocular lens group at 1 month (P = .0024), and 3 months (P = .0032), and 6 months (P = .0022) postoperatively. CONCLUSIONS: After continuous capsulorrhexis and intraocular lens implant surgery, the area of the anterior capsule opening gradually decreased for up to 3 months postoperatively. The contraction of the anterior capsule opening was greater in the silicone group than in either the polymethylmethacrylate or the soft acrylic groups.

Modulation of the synaptic drive to respiratory premotor and motor neurons.

The characteristics of GABAergic inhibitory modulation of respiratory bulbospinal neuronal activity and short-term potentiation (STP) of phrenic motoneuronal activity were studied. Extracellular unit recording and picoejection techniques in anesthetized dogs showed that both the spontaneous rhythmic and reflexly induced discharge patterns of inspiratory (I) and expiratory (E) premotor neurons were proportionately amplified by the localized application of picomole amounts of bicuculline (Bic), a competitive GABAA antagonist. Intracellular recording and paired-pulse stimulation techniques in anesthetized rats demonstrated an STP of phrenic motor output that appears to be mediated by NMDA receptors and is associated with facilitation of EPSPs and prolonged depolarization of individual phrenic motoneurons. We speculate that both GABAergic gain modulation of premotor neuronal activity and NMDA-mediated STP of phrenic activity may be neural substrates which are involved with the optimization of respiratory and non-respiratory behaviors, via adaptive and/or differential control of breathing.

Corneal shape changes after scleral buckling surgery.

PURPOSE: A prospective study was conducted to investigate the corneal shape changes due to scleral buckling surgery. These changes were analyzed based on the type of buckling procedures performed. METHODS: A total of 89 eyes from 88 patients were stratified into four groups based on the type of buckling procedures used, including:group A, local buckling; group B, encircling; group C, encircling with vitrectomy; and group D, encircling with additional segmental buckling. These eyes underwent keratometry and videokeratography examinations before surgery as well as at 1, 3, and 6 months after surgery. RESULTS: No statistical significance was observed in the amounts of the induced corneal astigmatism and the refractive cylinder among the four groups. After local or segmental buckling (groups A and D), corneal steepening, which corresponded to the buckle, occurred at a high incidence. After encircling (groups B and C), either peripheral corneal flattening with focal central steepening or flattening on one side with coupled steepening on the opposite side was observed. Such corneal changes persisted for up to 6 months in an irregular and asymmetric configuration. CONCLUSIONS: All four types of circumferential scleral buckling surgery were found to produce prolonged irregular and asymmetric corneal shape changes, whereas the patterns of the changes differed depending on the buckling procedures used.

Decentration and tilt of polymethyl methacrylate, silicone, and acrylic soft intraocular lenses.

PURPOSE: The purpose of the study is to investigate the periodic changes regarding the decentration and tilt of the intraocular lens (IOL) and to compare any differences in the decentration and tilt among polymethyl methacrylate (PMMA), silicone, and acrylic soft IOLs. METHODS: A total of 225 cataractous eyes undergoing IOL implant surgery were randomized into 3 groups based on the type of IOL: group A, one-piece PMMA IOL; group B, three-piece silicone IOL; and group C, three-piece acrylic soft IOL. Both the length of the decentration and the degree of the tilt of the IOL were quantitated using the Anterior Eye Segment Analysis System (EAS-1000). All eyes underwent EAS-1000 examinations at 1 week as well as 1, 3, 6, 9, and 12 months after surgery. RESULTS: All IOLs were confirmed to be implanted accurately in the capsular bag after continuous curvilinear capsulorhexis was accomplished. No statistically significant differences were observed regarding the IOL decentration or tilt between the various postoperative periods in any of the three IOL groups. Furthermore, the differences regarding both the IOL decentration and the tilt between the three IOLs were not determined to be statistically significant throughout the observation period. CONCLUSIONS: As long as the IOL was placed properly in the capsular bag after the continuous capsulorhexis, neither the decentration nor the tilt of the IOL showed a significant progression up to 12 months after surgery. Furthermore, both the extent of the decentration and tilt almost were the same among the PMMA, silicone, and acrylic soft IOLs.

In vivo evidence for the link between L- and D-serine metabolism in rat cerebral cortex.

To obtain an insight into the metabolic pathways of endogenous D-serine in mammalian brains, we have investigated in the infant rat the effects of systemic administration of L-serine, D-serine, and related amino acids, including glycine and threonine, on the amino acid contents in the cerebral cortex. Intraperitoneal injection of L-serine induced a rapid and transient elevation of the levels of L-serine itself in the neocortex, with its peak at 3 h post injection, and a delayed and prolonged increase in D-serine contents from 1.5 h to at least 24 h thereafter. Similarly, a significant augmentation in cerebral D-serine contents was observed 6 h after intraperitoneal administration of glycine, which also elevated the cortical L-serine levels. In contrast, L-threonine injection affected the concentrations of neither D- nor L-serine in the cortex of the pups. D-Serine given systemically, in turn, increased the neocortical contents of L-serine as well as D-serine itself, but failed to alter those of glycine and L-threonine. These in vivo data suggest the possible link between metabolic pathways of D- and L-serine in the cerebral cortex of the rat.

An in vitro brainstem-heart preparation of the neonatal rat with intact right vagus nerve.

An in vitro brainstem preparation of the neonatal rat with intact right vagal (X) innervation of the right atrium, and intact medullary roots of the left X and glossopharyngeal (IX) nerves for stimulation was developed. The preparation was continuously superfused with artificial CSF at 25 degrees C. The electrical activity of the right atrium was recorded to determine the heart rate. Applications of atropine or propranolol to the superfusate did not alter the heart rate. Electrical stimulation (0.5 ms pulse, 20 Hz) of the left IX and X afferents elicited a reduction in the heart rate from 70.3 +/- 13.2 to 50.6 +/- 13.2 beats/min (mean +/- SD, p < 0.05), which was abolished after division of the right X or application of atropine to the superfusing solution. A similar reflex bradycardia was seen in a preparation with intact left vagal-right atrium innervation during right IX and X afferent stimulation. Cervical spinal cord transection affected neither the baseline heart rate nor the magnitude of the reflex bradycardia. Longitudinal sectioning of the medulla oblongata in the mid-line down to the level of the posterior inferior cerebellar artery abolished the heart rate response. After bilateral cervical vagotomies, electrical stimulation (0.5 ms pulse, 20 Hz, up to 100 microA) of the ventrolateral medulla oblongata, lateral funiculus at C2 or intermediate nucleus of the spinal cord at Th1-4 did not affect the heart rate. These results indicate that the functions in the lower brainstem are preserved in this preparation, at least in regard to the generation of reflex bradycardia. The results also suggest that the laterality of cardiac vagal innervation and sympathetic innervation will develop during the postnatal period. This preparation may be useful for the study of the central neuronal network controlling the heart rate.

[A study in cognitive complexity of the self: an evaluation of the Linville's index].

The purpose of the present study was to evaluate H statistic, proposed by Linville (1985, 1987), as an index for cognitive complexity of the self. Linville asserted that high self-complexity would act as a buffer against life stress or depression. One hundred and eighty-seven undergraduates sorted 40 personality-trait adjectives into as many categories as necessary in order to describe themselves. In addition, 126 participants filled out several scales including self-consciousness and esteem. Main findings were as follows: (a) H statistic was not significantly associated with any variable related to the self-ratings, and showed no stress-buffering effect. (b) On the other hand, participants who had high cognitive complexity for the negative aspects of the self, as operationalized by Woolfolk, Novalany, Gara, Allen, and Polino (1995), were low in self-esteem and high in public self-consciousness. The results suggest that cognitive complexity of the negative self may indicate a predisposition for depression or neurosis. (c) Also, women scored significantly higher than men on cognitive complexity of the negative self.

A hidden immunoglobulin G2 in patients with rheumatoid arthritis detected by nuclear magnetic resonance.

OBJECTIVE: The ratios of immunoglobulin (Ig) G1 and IgG2 in patients with rheumatoid arthritis (RA) were examined by nuclear magnetic resonance (NMR) and the results were compared to data obtained by ELISA. METHODS: The IgG of 11 patients with RA were prepared with a DE-52 column and the specific signals for IgG1 and IgG2 were measured by NMR. The ratios of IgG1 and IgG2 were determined by the intensity of this signal. The samples were also measured by ELISA. RESULTS: The ratios of IgG2 in patients with RA measured by NMR were increased significantly compared to controls. However, there were no significant differences in the data determined by ELISA. Thus, a discrepancy exists in the analysis of IgG2 ratios between NMR and ELISA methods. CONCLUSION: There was a discrepancy in the IgG2 ratios of patients with RA between NMR and ELISA methods, and we attribute this to a conformational difference in IgG2 in patients with RA.
