Role of bone scanning for evaluation of carcinoma of the cervix.

The value of X-ray studies and bone scan studies of cervix cancer patients was evaluated by a review of patient charts and bone scans or X-ray studies. No in situ cancer patients had positive studies. A low frequency of positive X-ray studies was observed ranging from 1% with stage I to 10% with stage IV disease. Recurrent carcinoma had 10% positive studies. Bone scans correlated well with positive X-ray studies but 10% false-positive scans were seen, mostly osteoarthritis. The bone scan was much more sensitive and appeared to detect an earlier stage lesion, usually associated with symptoms which with radiation and other treatments could follow a more indolent course than the patient with a positive X-ray study. The present study evaluates the appearance and value of these studies, the frequency and circumstances under which useful information was obtained, and what type of survival followed the establishment of the diagnosis.

Discordant 99mTc sulfur colloid and 67Ga-citrate scintigrams in Aspergillus splenic abscesses.

A 43-year-old man with acute undifferentiated leukemia developed high fever and stomatitis followed by severe abdominal pain after completion of a first course of chemotherapy. A 99mTc-sulfur colloid (SC) scan showed multiple, irregular defects in an enlarged spleen. A 67Ga-citrate scan showed uniform tracer distribution except for a large defect in the upper portion of the spleen. The size of the spleen in the 67Ga-scan was much larger than in the SC scan. At surgery an abscess cavity was found between the spleen and the greater gastric curvature with multiple smaller abscesses on the splenic capsule and throughout the splenic parenchyma. A hematoma was present in the upper pole of the spleen. The disparate splenic size and seemingly discordant distribution patterns of tracer in the 99mTc- and 67Ga-scans were fully explained by histopathological study of the surgically resected spleen.

Extraosseous localization of 99mTc-MDP in ganglioneuroblastoma.

A 2 1/2-year-old boy with a posterior mediastinal ganglioneuroblastoma was evaluated by bone scintigraphy using 99mTc-DMP and showed a marked increased uptake in the tumor mass. Although a routine chest X-ray and body CT scan failed to demonstrate calcification, X-ray examination of the removed tumor specimen revealed small scattered calcification. The proposed mechanisms of the radiopharmaceutical localization in benign and malignant tumors other than calcification/mineralization are discussed. Regardless of the mechanism, localization of a bone scanning agent in a tumor provides a rapid, simple, and noninvasive means of demonstrating tumor size, configuration, and the relation of the surrounding structures to the tumor.

Complementary role of reticuloendothelial and hepatobiliary imaging agents in the assessment of liver disease.

The merit of concomitant hepatic imaging with Tc-99m-sulfur colloid (SC) and Tc-99m-HIDA was examined in 40 patients with abnormal liver function tests. The studies were performed sequentially within a 2 to 5 day interval with 5 mCi each of SC or HIDA. Routine static images in all cases, and dynamic studies with HIDA in 15 patients, were graded and compared with the liver function tests and clinical diagnoses. Of the 40 patients, the images with SC showed an abnormality in 22 while HIDA images were abnormal in 38. Thirteen of 21 patients with hepatocellular diseases showed an abnormality of the SC images, and 20 of the same 21 patients had abnormal HIDA images. Abnormal HIDA images were more closely related to abnormal liver function tests. These results suggest that alterations of hepatic function reflected by abnormal liver function tests are better demonstrated with hepatobiliary imaging agents than with reticuloendothelial imaging agents, while either type of imaging agents will detect focal anatomic lesions.

Basal ganglia infarction demonstrated by radionuclide brain imaging.

Four cases of basal ganglia infarction demonstrated by radionuclide brain imaging are presented. Bilateral basal ganglia infarctions in two patients were probably related to methanol intoxication and meningoencephalitis, and unilateral basal ganglia infarctions in two other patients were presumably due to cerebral atherosclerosis and/or hypertension. Various causes and mechanisms of basal ganglia infarction as well as positive findings of radionuclide brain imaging are briefly reviewed.

Value of bone scanning in detecting occult skeletal metastases from adenocarcinoma of the endometrium.

Bone scans were obtained in patients with adenocarcinoma of the uterus in order to search for occult skeletal metastases. Radiological studies in 350 patients with adenocarcinoma of the uterus treated at the University of Kentucky Medical Center were reviewed. All patients studied in Stage 1A, Stage 1BG1, and Stage G2 were negative. Bone scans performed since 1973 were reviewed. One out of 11 patients studied with G3 lesions had a positive scan. Four out of 41 patients with Stage 2 lesions had positive scans, seven out of 18 in Stage 3, and two out of nine in Stage 4 disease. Four out of 71 patients with recurrent tumor had positive scans. In general, all positive scans were associated with significant symptoms in the patients studied. The results correlated well with x-ray findings as evaluated by conventional radiologic studies.

The role of radioimmunodetection in the management of testicular cancer.

Five patients with testicular cancer received an intravenous injection of between 1 and 2.5 mCi of iodine 131-labeled antibody to human chorionic gonadotropin (HCG) or alpha-fetoprotein (AFP), followed by total-body photoscanning to visualize areas of abnormal radioactivity. Blood-pool and nontarget sites of radioactivity were reduced by subtracting the images derived by injection of technetium Tc 99m-labeled components from the iodine 131 scans. The HCG-immune scintiscans proved helpful in tumor localization and in the selection of appropriate therapy, while the AFP scan presented corroborative evidence of widespread tumor. Elevated serum levels of these two markers did not hinder successful tumor detection and localization by this method of radioimmunodetection. Cancer radioimmunodetection with antibodies to HCG and to AFP appears to be a useful procedure for the pretreatment and posttreatment evaluation of patients with testicular cancer and can reveal sites of tumor not detected by other methods.

Biologic gastric emptying time using Tc-99m TETA polystyrene resin in various clinical conditions.

In 82 subjects 82 gastric emptying studies using Tc-99m triethylene tetramine polystyrene resin were retrospectively evaluated. All six patients with diabetic gastroenteropathy (GE) had delayed biologic gastric emptying time (BGET) that responded well to metoclopramide (MP) injection. In 11 diabetics without GE, only two had prolonged BGET with good response to MP. All 11 patients with gastric outlet obstruction also had prolonged BGET, but no significant response to MP was found in 7 studies. Five of 7 patients with active gastric ulcer had delayed BGET. Three of ten patients with previous Billroth I or II operation had accelerated BGET, and 3 of 9 patients with previous vagotomy had delayed BGET with good response to MP. Markedly prolonged BGET with significant response to MP was also observed in 3 patients with disordered gastric motility. Delayed BGET was found in one patient with bile reflux gastritis and in 2 of 6 patients with reflux esophagitis. No prolongation of BGET was observed in 6 symptomatic patients whose radiographic and endoscopic examinations were negative. BGET studies with this agent appear to be reliable and very helpful in the management of patients with gastric symptoms and for obtaining an objective measurement of the response to therapy.

Functional alterations of muscle perfusion in type II muscle atrophy: demonstration by lower extremity perfusion scans.

Significant progress in the clinical assessment of peripheral vascular disease has resulted from the use of radionuclide scans to evaluate alterations of blood flow through small vessels. Two patients with Type II muscle atrophy and normal vessels on muscle biopsy were examined; abnormal lower extremity perfusion scans were obtained after the administration of 99mTc-labeled human albumin microspheres directly into the femoral arteries. These case studies illustrate the use of this technique for delineating muscle groups affected by a myopathic process. Muscle perfusion scans can be used as an aid in selecting biopsy sites and may provide a means of following the response to therapy.

Human chorionic gonadotropin radioantibodies in the radioimmunodetection of cancer and for disclosure of occult metastases.

Radioimmunodetection (RaID) of tumors containing human chorionic gonadotropin (hCG; choriogonadotropin) was evaluated in 25 patients by injecting 131I-labeled goat antibody IgG against hCG and performing total-body photoscans with a gamma scintillation camera 24 and 48 hr later. All 10 testicular cancer patients with proven tumor sites had positive RaID results, whereas three cases without known tumor were negative. Four patients with hydatidiform mole and one with degenerative products of conception showed positive RaID results consistent with elevated serum hCG titers. Two putatively false-positive results were obtained in patients with lung or ovarian cancer, whereas a false-negative metastasis to the liver of a patient with lung cancer and an elevated serum hCG titer was observed. Of 14 tumor sites found by RaID in 10 testicular cancer patients, 4 were revealed by RaID prior to any other detection method used and provided a lead time to definitive diagnosis by other measures of a few days to greater than 1 yr. Although a number of patients had high serum hCG levels, even exceeding 3 microgram/ml, the xenogeneic antibody was capable of localizing in tumor. No adverse effects were noted in any of the patients studied. Thus, hCG RaID appears to be a safe and effective method of detecting and locating hCG-producing tumors and has been found to disclose occult testicular cancers.

Reliability of the dual-isotope Schilling test for the diagnosis of pernicious anemia or malabsorption syndrome.

To evaluate the dual-isotope Schilling test for the diagnosis of pernicious anemia or malabsorption syndrome, 65 studies were selected for clinical correlation. Criteria for pernicious anemia included mean corpuscular volume greater than 100 cu micrometer, serum B12 greater than 100 ng/l, megaloblastic marrow, achlorhydria, reticulocytes greater than 5% on B12 therapy, atrophic gastritis, and elevated serum antibodies to parietal cells or intrinsic factor. Criteria for malabsorption syndrome included: decreased serum B12, folate, and carotene; increased fecal fat; abnormal D-xylose absorption; abnormal radiographic and biopsy findings. 58Co-cyanocobalamin and 57Co-cyanocobalamin bound to intrinsic factor were given orally to fasting patients; 1 mg of nonradioactive B12 was injected intramuscularly within two hours. Aliquots of 24-hour urine samples were counted. If the excretion of 58Co was less than 7% and the 57Co/58Co ratio was greater than 1.7, the test indicated pernicious anemia; a ratio less than 1.7 indicated malabsorption syndrome. Sensitivity, specificity, and accuracy of the dual-isotope Schilling test were 83%, 98%, and 94% for pernicious anemia, and 67%, 90%, and 86% for malabsorption syndrome, respectively.

Avid thyroid uptake of [Tc-99m] sodium pertechnetate in children with goitrous cretinism.

Three children with goitrous hypothyroidism had thyroid scans with [Tc-99m] sodium pertechnetate, which showed symmetrically enlarged thyroid glands with uniformly increased activities compared to little activities in the salivary glands and low body background activities. These scan findings, simulating those of Graves' disease, reflect acid trapping of this tracer, analogous to that seen with I-131. Perchlorate discharge test was positive in two patients, indicating an organification defect.
