[Myeloma localized to the mandibuloparotid region (author's transl)].

Myeloma is a malignant hematopathy with multiple lesions, mainly in bones. The authors report a case in which the myeloma was present in the mandibuloparotid region only. The atypical nature and rarely observed location (mainly parotid) of the lesion raised diagnostic, prognostic, and therapeutic problems that were resolved by close cooperation with a team of hematologists. Therapy was by surgery only, in this particular case, because of the early stage of the lesion, but regular close surveillance is essential.

[Malignant non-Hodgkin type lymphomas of the main salivary glands. A report on 11 cases (author's transl)].

Malignant non-Hodgkin type lymphomas (MNHL) are relatively frequent tumors but their localisation in the salivary glands is rarely reported in the published literature. The authors describe 11 such cases, seen in the stomatology Department of the Salpêtrière Hospital in Paris over a period of 4 years, in which the MNHL was located in the salivary tissues (2 purely glandular, 1 of the mixed type, and 8 extranodal tumors). Treatment by multiple chemotherapy and radiotherapy demonstrated that survival, whatever the anatomical type or even the histological form, was improved in relation to that observed with tumors in other locations.

[Primary mandibular condyle tumors. Osteomas, chondroma, fibrosarcoma, giant cell tumor (author's transl)].

Six out of 11 cases of primary condylar tumors treated in the Department of stomatology and Maxillofacial Surgery were analyzed. They included 3 patients with osteomas, and one case each of fibrosarcoma, chondroma, and multinucleated giant cell tumor. Typical signs were lateral displacement of the mandible (2 cases), reduction in the buccal opening (3 cases), and pain (3 cases) and swelling (3 cases) in the pretragal region. Swelling is never observed with osteomas, which develop antero-internally. The presence of the tumors was confirmed by radiological examination in all cases. Usual treatment is condylectomy, by means of C. Hosxés approach, confirmation of diagnosis requiring pathological examinations.

[Parotid cystadenolymphomas : histo-enzymological and ultrastructural study in 6 cases (author's transl)].

Histo-enzymological and ultrastructural studies in 6 cases of cystadenolymphoma were able to confirm the marked metabolic activity of the oncocytic cells, which from the major part of these tumours. The oncocytes demonstrate greatly enhanced enzymatic activity, particularly of those enzymes concerned with oxydative metabolism and the diaphorases. Electron microscopy of the oncocytes shows that they are filled with mitochondria of very varied size and form, sometimes associated with the presence of several intracytoplasmic crystalloids. The second, lymphoplasmocytic component of these tumours present no particular morphological features.

Adenolymphoma

[Mandibular condyle metastasis : report of three cases (author's transl)].

Three cases of mandibular condyle metastasis of particular interest are reported: -- In two out of the three cases they were detected early enough for the primary tumour to be diagnosed; -- they all presented with articular symptoms; -- all three were treated surgically by excision of the condyles. Diagnosis was confirmed by histological examination, a rare event when mandibular metastasis are concerned, these usually being diagnosed after the primary tumour and then irradiated without histological proof. Primary tumours were respectively bronchial, pancreatic, and mammary.

Adenocarcinoma

[Desmoid fibroma of bone. An histoenzymological and ultrastructural study of the mandible in one case (author's transl)].

A case of post-traumatic desmoid tumor of the mandible is reported in a 4-year old girl. This tumor was considered typical clinically and by light microscopy. Four years later, the patient was symptomless, without evidence of recurrence. Histoenzymology showed a high level of leucine aminopeptidase activity. On electron microscopy, a part from a few primitive mesenchymal cells, abnormal fibroblasts represented the principal cellular component. These cells showed evidence of intracytoplasmic collagen fibers synthesis and of myoid differentiation (numerous filaments and endocytosis vacuoles). Myofibroblasts, with a microfibrillary network showing areas of dense osmiophilic condensation in the vicinity of cell membrane were also present. This atypical ultrastructural feature suggests, in this case, the non-neoplastic nature of this lesion. It seems to reflect an abnormal reaction to a traumatic aggression, characterized by proliferation of myofibroblasts (which normally disappear in the late stages of healing) and to have, thus, the signification of pathologic scar.

[Intracranial penetration of the mandibular condyle: report on three cases (author's transl)].

Three cases of intracranial penetrations of the mandibular condyle condyle are reported, the published literature reviewed, and therapy discussed. Often unrecognized during initial examinations, they are diagnosed when the patient is seen several months later with permanent constriction of the jaws. Frontal and sagittal tomographic examinations should therefore be performed in the articular region in all cases when it is involved in injuries. Orthopedic reduction gives good results in recent injuries, long-standing cases with temporomandibular ankylosis requiring sub-condylar resection by the pre-auricular approach.

[Intraoral minor salivary gland tumors. Report of 296 cases (author's transl)].

296 intraoral minor salivary gland tumors were observed among 702 salivary tumors. The study emphasizes a frequent palatine location, observed in more than 50 per cent of theses tumors. Pleiomorphic adenomas (or so called mixed tumors), are frequently encountered; but it is noteworthy that the proportion of muco-epidermoid tumors or adenoid cystic carcinoma is higher among theses tumors than among major salivary glands; this fact explain a poor overall prognosis of minor salivary gland tumors.

[Inflammatory granuloma of the face with a malignant course. Atypical Hodgkin's disease (author's transl)].

A case of Hodgkin's disease beginning in the skin of the face with local lymph node involvement is reported. This was a lymphogranulomatous form without characteristic Reed-Sternberg cells in skin biopsy specimens, rendering the diagnosis difficult. There was a combination of granulomatous inflammation, plaques of necrosis and lesions of fibrinoid necrosis in the vessel walls. This form entered the classification of clinical Stage IV, with its associated serious prognosis.

[Aneurysmal cysts located in the maxilla or mandible (author's transl)].

Aneurysmal cysts of the mandible and maxilla are rare conditions, though not exceptional, which are seen in particular in the child and adolescent. A part from facial asymmetry and tumefaction, it is rare to find other clinical manifestations. No neurological signs were seen in the present study. An aneurysmal cyst gives a radiological appearance of non-characteristic lysis compatible with a large range of diagnoses. Histological study permits early diagnosis but must always involve the whole specimen with careful examination to detect any other associated lesion. The pathogenesis of the condition remains obscure and, at the extreme, it may be questioned whether the aneurysmal cyst is a separate entity or a particular form of chronic osteopathy.

Adolescent

[Chromatography of urinary amino-acid in bismuth encephalopathy (author's transl)].

The urinary excretion of amino-acid have been determined by ionic exchange chromatography for 10 cases of bismuth encephalopathy and for 15 normal controls. The ninhydrine positive constituant reported by Burns in 5 cases of encephalopathy has not been detected. We are thinking it may be related to the use of a rich amino-acid solution during perfusions. We did not find any significant difference between the urinary excretion of amino-acid average value to the two groups. We found an increase excretion when severe encephalopathy were observed. Separate analysis of individual amino-acid excretion showed the following results: i) average percent of tyrosine excretion is lower in controls as in encephalopathies; ii) among the different ratios tested only SER + THR)/TYR and VAL/TYR are different in the two groups.

[Surgical treatment of loss of substance of the lower lip].

Repair of loss of substance of the lower lip has been the object of numerous studies. Each author has used his own ingenuity to describe a new type of flap aimed at restoring a particular form of loss of substance. The techniques suggested here are capable of solving a majority of problems on the basis of simple and original solutions. These flaps make it possible to resolve the problems posed by most cases of loss of substance of the lower lip. The choice of these plastic procedures permits restoration which is at one and the same time functional and anesthetic.
