[Angiographic study of cerebral convolutions and their area of vascularization].

The authors demonstrate the facility with which the principal fissures and sulci of the brain can be angiographically localized by reference to characteristic loops and bends in the courses of the branches of the anterior, middle, and posterior cerebral arteries. The study is based upon dissections of 150 human brains in which the arteries of interest were selectively injected. A propos to this anatomic-angiographic correlation the authors also describe the vascular territories of each cortical branch and propose a simple procedure for the evaluation of these territories during cerebral angiography.

Graphic-aided study of metabolic modifications of plasma in cancer using proton magnetic resonance spectroscopy.

Proton high-resolution MRS of human plasma allows the rapid detection, on the same spectrum, of many compounds originating from different metabolic pathways. In this paper, we illustrate the modifications of the plasma metabolic profiles recorded by proton NMR spectroscopy in different classes of cancers. These modifications can be easily monitored with graphic aids such as 'star plots' which define for each type of cancer a particular pattern describing the most altered metabolic pathways. By using 'star plots' three types of metabolic patterns have been distinguished: (i) the 'inflammatory' pattern characterized by an increase of glycosylated moieties of glycoproteins; (ii) a 'lipid modified' pattern, characterized by various modifications occurring mainly in the lipid moieties detected by MRS; and (iii) a pattern which is often observed in sarcomas and mainly characterized by an alteration in the N-acetyl glucosamine/N-acetyl neuraminic acid ratio. This study demonstrates the ability of proton MRS of plasma to rapidly detect the occurrence of metabolic modifications brought about by cancer evolution or therapy.

[Cerebral abscess. Analysis of 41 cases over a 10-year period].

Although treatment with antibiotics and computerized tomography (CT) of the brain have considerably improved the prognosis of intracranial purulent collections, brain abscess remains a lethal disease in about one out of ten cases. We present a series of 41 brain abscesses diagnosed between 1976 and 1986 and treated surgically. The clinical signs were more often neurological than infectious. Among the exploratory methods, only CT has a real diagnostic value. Treatment is both medical and surgical. Simple aspiration seems to have the triple advantage of producing prompt and effective decompression of brain tissue, enabling the pathogen(s) to be isolated so that the appropriate antibiotic can be chosen, and being a repeatable and little traumatic procedure with less sequelae than surgical excision.

Epidermoid cysts of the fourth ventricle.

The location of epidermoid cysts within the fourth ventricle is rare. The Authors present three cases operated on. The most frequent symptom was ataxia. Increased intracranial pressure and hydrocephalus were rare even when the tumors were large in size and filled up the fourth ventricle. Neuroradiological data are presented. Surgical removal was generally easy though the tumor invaded the cisterns and a true peritumoral capsule was found only in the lower part of the ventricular floor. The bursting of the cyst followed by continuous production of keratin with subsequent invasion of cisterns and foramina is suggested as a possible mechanism to explain the tumoral growing.

Modalities of origin of the middle cerebral artery. Incidence on the arrangement of the perforating branches.

In a series of 100 anatomical dissections on the basal brain arteries a case of real duplication at the middle cerebral artery level has been found. The importance of such a finding is discussed in the light of literature data. The knowledge of similar anatomical patterns is fundamental in the study of the lenticulostriate arteries arising from the sphenoidal segment of the middle cerebral artery. In fact the neurosurgeon must be familiar with all the possible variations of these perforators, given that the opening of the Sylvian fissure is by now an obligatory manoeuvre for the transcisternal access to the circle of Willis.

Hemangiopericytomas of the spinal canal. Report of four cases and review of literature.

The Authors report four cases of spinal hemangiopericytomas. The diagnosis has been ascertained by electron microscopy. Despite the malignant nature, the association of surgery and radiotherapy is interesting in that it prolongs the survival rate. Clinical, neuroradiological, microscopic and ultrastructural features of intraspinal hemangiopericytomas are evaluated in the light of the review of the relevant literature.
