Viral expressions in Reed-Sternberg cells.

In the murine system natural hybridoma formation was observed first in 1968-9. In the #620 to 818 system a mouse leukemia virus-(MLV-) producer diploid lymphoma cell fused with an immune plasma cell. The tetraploid fusion product cells grew in suspension cultures and as ascites tumors in mice and continued the production of MLV particles and MLV-neutralizing antibodies. Analogy between the #620 to 818 system and the origin of RS cells is proposed. Indirect evidence suggests retroviral infection of the mononuclear HD cell which presumably is an interdigitating reticulum (IR) cell. Reactive B and T cells interact in an abnormal manner and fuse with the retrovirally infected IR cell. The fusion product cells display hyperdiploidy and a disarray of markers as IR markers are lost due to dedifferentiation (and regained upon differentiation induction) and B and/or T cell markers are gained. Conventional theories for the origin of RS cells fail to explain the great heterogeneity of their markers. Derivation of RS cells from IR cells and B and/or T lymphocytes as natural hybridomas offers plausible explanation for all the features of RS cells.

Multiple signal transduction pathways lead to extracellular ATP-stimulated mitogenesis in mammalian cells: II. A pathway involving arachidonic acid release, prostaglandin synthesis, and cyclic AMP accumulation.

We have previously shown that extracellular ATP acts as a mitogen via protein kinase C (PKC)-dependent and independent pathways (Wang, D., Huang, N., Gonzalez, F.A., and Heppel, L.A. Multiple signal transduction pathways lead to extracellular ATP-stimulated mitogenesis in mammalian cells. I. Involvement of protein kinase C-dependent and independent pathways in the mitogenic response of mammalian cells to extracellular ATP. J. Cell. Physiol., 1991). The present aim was to determine if metabolism of arachidonic acid, resulting in prostaglandin E2 (PGE2) synthesis and elevation of cAMP levels, plays a role in mitogenesis mediated by extracellular ATP. Addition of ATP caused a marked enhancement of cyclic AMP accumulation in 3T3, 3T6, and A431 cells. Aminophylline, an antagonist of the adenosine A2 receptor, had no effect on the accumulation of cyclic AMP elicited by ATP, while it inhibited the action of adenosine. The accumulation of cyclic AMP was concentration dependent, which corresponds to the stimulation of DNA synthesis by ATP. The maximal accumulation was achieved after 45 min, with an initial delay period of about 15 min. That the activation of arachidonic acid metabolism contributed to cyclic AMP accumulation and mitogenesis stimulated by ATP in 3T3, 3T6, and A431 cells was supported by the following observations: (a) extracellular ATP stimulated the release of [3H]arachidonic acid and PGE2 into the medium; (b) inhibition of arachidonic acid release by inhibitors of phospholipase A2 blocked PGE2 production, cyclic AMP accumulation, and DNA synthesis activated by ATP, and this inhibition could be reversed by adding exogenous arachidonic acid; (c) cyclooxygenase inhibitors, such as indomethacin and aspirin, diminished the release of PGE2 and blocked cyclic AMP accumulation as well as [3H]thymidine incorporation in response to ATP; (d) PGE2 was able to restore [3H]thymidine incorporation when added together with ATP in the presence of cyclooxygenase inhibitors; (e) pertussis toxin inhibited ATP-stimulated DNA synthesis in a time- and dose-dependent fashion as well as arachidonic acid release and PGE2 formation. Other evidence for involvement of a pertussis toxin-sensitive G protein(s) in ATP-stimulated DNA synthesis as well as in arachidonic acid release is presented. In A431 cells, the enhancement of arachidonic acid and cyclic AMP accumulation by ATP was partially blocked by PKC down-regulation, implying that the activation of PKC may represent an additional pathway in ATP-stimulated metabolism of arachidonic acid. Adrenal and ovarian steroid hormone responses to gonadotropin-releasing hormone agonist treatment in polycystic ovary syndrome.

It has been postulated that in polycystic ovary syndrome ovarian steroids can influence adrenal steroidogenesis. To test this hypothesis, basal and dexamethasone-suppressed-corticotropin-stimulated steroid hormone responses were compared among three groups of women before, during, and after gonadotropin-releasing hormone agonist treatment for 3 months. The groups were characterized as follows: (1) women with polycystic ovary syndrome with high dehydroepiandrosterone sulfate levels (greater than 400 micrograms/dl), (2) women with polycystic ovary syndrome with normal dehydroepiandrosterone sulfate levels (less than 300 micrograms/dl), and (3) normal ovulatory women. In response to gonadotropin-releasing hormone agonist, basal serum luteinizing hormone, follicle-stimulating hormone, estradiol, estrone, 17-hydroxyprogesterone, androstenedione, and testosterone in all three groups were suppressed to similar levels. Basal serum dehydroepiandrosterone sulfate levels in the group with high levels declined, but they did not reach the normal, unaltered concentrations in the other two groups. Two subjects with polycystic ovary syndrome in this group with high levels, who showed the greatest declines in basal serum dehydroepiandrosterone sulfate levels (34%, 40%), also had evidence of 3 beta-hydroxysteroid dehydrogenase deficiency before treatment, which was resolved by the end of treatment. In both groups with polycystic ovary syndrome, the increase in maximum incremental rise of dehydroepiandrosterone and dehydroepiandrosterone sulfate levels in response to a pharmacologic dose of corticotropin from a dexamethasone-suppressed baseline (adrenal androgen capacity) remained unaltered during gonadotropin-releasing hormone agonist administration. We conclude that ovarian steroids may promote excessive adrenal androgen secretion in women with polycystic ovary syndrome, may induce 3 beta-hydroxysteroid dehydrogenase deficiency as a mechanism for adrenal involvement in some women with polycystic ovary syndrome, and do not influence adrenal androgen capacity.

CROCODILE: an automated apparatus for organic crystal growth from solution.

CROCODILE (CROissance de Cristaux Organiques par DIffusion Liquide dans l'Espace) is a space instrument dedicated to crystal growth from solution. The selected material N (4 nitrophenyl) (L) prolinol (NPP) is the result of studies on organic crystal in the frame of an extended program initiated by CNES for many years. The apparatus was flown aboard PHOTON, an automatic satellite, in April 1990, for a flight duration of more than 15 days. This paper describes the instrument design, with emphasis on specific and original technology well adapted to crystal growth from solution, and extendible to any space experiment on fluids. Preliminary details of the flight campaign will also be discussed.

Simultaneous carpal tunnel release and Dupuytren's fasciectomy.

Simultaneous operations for carpal tunnel syndrome and Dupuytren's disease have been reported in only one study which recommended staged treatment because there was an 87% complication rate with simultaneous treatment. The present study reviewed simultaneous treatment of 30 patients during a 20-year period. Complications and unfavourable results were compared to similar groups of patients who underwent either Dupuytren's fasciectomy or carpal tunnel release as sole procedures. Early complications (13%) were far less than in previous studies and were easily corrected. The incidence of recurrence and extension (20%) was lower than in the Dupuytren's fasciectomy group and much lower than in previous reports. Simultaneous carpal tunnel release and Dupuytren's fasciectomy are strongly recommended when these conditions are both present in the same hand.

Autoregulation of cerebral blood flow in patients with orthostatic hypotension after spinal cord injury.

Two groups of patients who developed orthostatic hypotension (OH) after spinal cord injury (SCI) were studied. In the first group all patients (4 females and 6 males) were asymptomatic, whereas in the second group (1 female and 9 males) all had clinical manifestations of hypotension. All but 3 patients were tetraplegic, and these patients were paraplegic above the T6 level. For this study blood pressure (BP), heart rate and cerebral blood flow (CBF) velocity were measured simultaneously on a tilt table at 0, 30, 60, and 80 degrees. Cerebral blood flow in the middle cerebral artery was measured bilaterally utilising the transcranial Doppler technique. In asymptomatic patients the mean baseline (0 degrees) BP (110 +/- 16/70 +/- 77 mm Hg systolic/diastolic) was not significantly different from the BP (106 +/- 16/68 +/- 11 mm Hg) of symptomatic patients. The mean maximal change in BP during tilting in the asymptomatic group (-23 +/- 10/10 +/- 7 mm Hg) was also not significantly different when compared to the symptomatic group (-29 +/- 13/11 +/- 6 mm Hg). CBF in the symptomatic group during the hypotensive reaction at 80 degrees was 32.5 +/- 5 cm/sec, while at the same body position in the asymptomatic group it was 40.9 +/- 8 cm/sec (significant at the p less than 0.02). In addition, CBF decreased in the symptomatic group at 80 degrees to 55.5 +/- 9.6% of baseline, while in the asymptomatic group the fall was 69.3 +/- 7.2% (p less than 0.001). Our data suggest that autoregulation of CBF rather than systemic BP plays a dominant role in the adaptation to OH in patients with SCI.

Decreased incidence of urinary tract infections in patients with spinal cord injury: effect of methenamine.

This study was designed to determine the effect of methenamine on the frequency of urinary tract infections (UTI) in hospitalized patients after spinal cord injury. The study included 56 patients with neurogenic bladder dysfunction treated with intermittent catheterization. A group of 34 patients was treated with methenamine, 1g twice daily; the other group of 22 patients was the control group receiving no antimicrobial therapy. On a weekly basis urine samples from all patients were obtained for urinalysis and culture. A total of 500 urine samples were analyzed. The patients treated with methenamine had 23.4% positive urine cultures which was significantly lower than 57.5% in the untreated control group (p less than 0.001). Our data suggest that methenamine therapy is an effective prevention of UTI in paralyzed patients with neurogenic bladder dysfunction during the rehabilitation in hospital.

Contrast responses to bright slits of visual cells in the superior colliculus of the albino rat.

Contrast is the most effective stimulus in the visual system. The response of single cells to changes in stimulus contrast has been studied in a large variety of animals and the contrast response function determined. In the rat, studies on responses to contrast have been focused primarily in the geniculocortical pathway and there are relatively few in subcortical structures. We report here for the first time the contrast response function of single units located in the superior colliculus (SC) of the albino rat to several stimulus contrast. Cells in the SC require a relatively high contrast to elicit a reliable response and the dynamic response range is restricted to a short contrast interval.

Apnea, transient episodes of bradycardia, and periodic breathing in preterm infants.

The occurrence of central apnea of 15 seconds or longer, transient episodes of bradycardia (TEB), and periodic breathing were studied in 66 healthy premature infants when at least 1 week old and between 32 and 36 weeks postconceptual age. Eight-hour cardiorespiratory recordings were visually scanned for the presence of these patterns. Central apnea of 15 seconds or longer was seen in almost half of the infants. The TEB were numerous, and the majority were not associated with central apnea; however, all but five of the apneic episodes that lasted 15 seconds or longer were accompanied by a TEB. Infants spent as much as 40% of their time in periodic breathing. The frequency with which these patterns are seen in healthy premature infants strongly suggests that they are normal findings. Our results do not support the opinion that brief periods of apnea are abnormal when accompanied by a TEB.

Effect of phosphate on antibiotic and extracellular protein production by Myxococcus coralloides D.

The effect of inorganic phosphate concentrations on antibiotic and extracellular protein production by Myxococcus coralloides D have been examined. Antibiotic production by growing cells of this myxobacterium was maximal at phosphate concentrations of 10-20 mM, but was inhibited by concentrations higher than 20 mM. The total extracellular protein and the extracellular protein per cell ratio were independent of phosphate levels in the culture broth.

Lateral-posterior and pulvinar reaching cells--comparison with parietal area 5a: a study in behaving Macaca nemestrina monkeys.

In a previous study we have demonstrated the existence of pulvinar (puv) cells which were optimally activated when a monkey executed reaching movements with his limbs (Acuña et al 1983). We now describe further observations in four Macaca nemestrina monkeys trained to perform goal directed reaching movements aimed at four different positions in space. Extracellular unit activity in the lateralis posterior (lp) and puv nuclei, together with electrooculograms were recorded during the execution of the task. Seven hundred and sixty neurons were studied in the lp-puv complex. One hundred and twenty three cells (16%) showed changes in activity related to the reaching movements. Reaching related cells fell into two categories: goal direction sensitive (28/123 = 23%) and pandirectional (95/123 = 77%). Goal direction sensitive cells showed different responses depending on the direction of the goal relative to the starting point of the movement. The responses of the pandirectional cells were independent of goal direction. The activity of the remaining cells (637/760) could not be correlated with reaching movements. In a smaller number of area 5a (PE) cells (n = 109) studied in one monkey, 82 (75%) were classified as reaching related cells. Of these, 76% (62/82) were goal direction sensitive and 24% (20/82) pandirectional. The lp-puv cells were more dependent on the intentionality of movement than area 5a cells, and not reliably activated by passive manipulation of the limb. After injection of HRP-WGA in area 5a, where the reaching cells were recorded, labeled cells and terminals were located in the lp-puv zones where reaching cells were also found.(ABSTRACT TRUNCATED AT 250 WORDS)

Sensation-seeking: electrodermal and behavioral effects of stimulus content and intensity.

Although a number of studies support an arousal theory interpretation of differential sensation-seeking behavior, the conditions under which arousal correlates of this personality dimension are manifest is as yet unclear. Both theory and research suggest that among the external factors affecting the differential arousal response are stimulus relevance and stimulus intensity. The present study assessed the impact of these factors on both psychophysiological and behavioral responses. Subjects were preselected to represent the extremes of the sensation-seeking dimension, then exposed to auditory and visual presentations of a series of sexual and aggressive stimuli of systematically varied intensity. High sensation seekers gave larger amplitude SCRs to violent stimuli and larger initial responses to sexual stimuli presented visually, while verbal response intensities showed an opposite pattern. Overall, results provided support for an arousal theory interpretation of sensation-seeking, but suggested that the probability and magnitude of group differences is a somewhat complex function of stimulus intensity, stimulus modality, and perhaps other factors not yet assessed.

Location of neurons projecting to the retina in mammals.

In this study, horseradish peroxidase (HRP) was used as a retrograde tracer in order to investigate the existence of centrifugal pathways to the retina in mammals and to locate the somas of the retinopetal neurons. After the application of HRP to the stump of the cut optic nerve in monkeys, cats, guinea-pigs and rabbits, labeled neurons were located in various areas. The largest number of labeled neurons was found bilaterally in the hypothalamus in the premammillary area, and some neurons were also found slightly more rostral and dorsally towards the posterior hypothalamic area. At the mesencephalic-metencephalic junction, a few labeled neurons were observed in the dorsal raphe nucleus and more ventrally in the tegmental area situated between the medial longitudinal fascicle and the superior cerebellar peduncle. Furthermore, labeled neurons were found in the contralateral medial pretectal area in guinea-pigs, in the ipsilateral tegmental mesencephalic reticular formation in monkeys, in the laterodorsal tegmental nucleus in rabbits, and in the dorsal hypothalamic area near the nucleus of the anterior commissure in monkeys. We were unable to find labeled cells in the dorsal raphe nucleus or in the hypothalamus in rabbits or in the above-mentioned tegmental area in guinea-pigs. These divergences may be due to species differences or may simply be false-negative results due to the known difficulty of using the currently available tracers to label retinopetal neurons.

Heterogeneities in regional volumes of distribution and flows in rabbit heart.

The heterogeneity of volumes of distribution in the heart influences the rates of uptake and washout of substrates and metabolites; thus it is important to evaluate their variability in the normal heart. Several tracers were injected intravenously into anesthetized adult closed-chest rabbits, and time was allowed for equilibration in the heart. Tracer microspheres were injected into the left ventricular cavity at the apex for the measurement of regional flows, the chest was opened, another set of microspheres was injected, and the heart was frozen rapidly in situ with liquid nitrogen-cooled Freon-22. Each heart was divided into 72 pieces of less than 0.1 g weight, and the tracer content of each was determined by multichannel gamma-counting and the water content by desiccation. The regional myocardial flows were (closed chest) 0.62 +/- 0.16 ml.g-1.min-1 and (open chest) 0.63 +/- 0.37 ml.g-1.min-1. The volumes of distribution (ml/g) for the 432 pieces for six rabbits, given as mean +/- SD (% coefficient of variation), were as follows: for plasma, VP = 0.11 +/- 0.03 (26%); erythrocytes, VRBC = 0.041 +/- 0.015 (37%); vascular space, VV = 0.15 +/- 0.04 (26%); extracellular space, VECF = 0.33 +/- 0.05 (15%); interstitial space, VISF = 0.21 +/- 0.03 (15%); and water space, VW -0.79 +/- 0.022 (2.8%). Regional hematocrits were 77% +/- 9% of the large-vessel hematocrits.

Effectiveness of doxazosin in systemic hypertension.

Most alpha-receptor blocking drugs require divided daily administration because of a short plasma half-life. This multicenter study examined the effectiveness and safety of once-daily administration with doxazosin, a quinazoline analog alpha 1-receptor blocking drug with a plasma half-life of 19 hours. Patients with diastolic blood pressure (BP) of 90 to 115 mm Hg entered 4 weeks of single-blind placebo therapy and then were randomized to double-blind treatment with doxazosin (63 patients) or placebo (67 patients). After 10 weeks of titration, standing arterial BP was lowered by 14/11 mm Hg with doxazosin and by 0.5/0.9 mm Hg with placebo (p less than 0.001). Measured hourly for 12 hours after the dose, all standing and supine arterial BP values were significantly lower in the doxazosin group at each hour. Pulse rate increased slightly in both groups int he double-blind phase, but the increase with doxazosin never significantly exceeded that of placebo. Dizziness was the most common complaint with doxazosin, but syncope did not occur. Side effects were mild and transient and did not necessitate withdrawing any participants from the study. Body weight increased by 1.5 kg in the doxazosin group and decreased by 0.2 kg in the placebo group (p less than 0.01). Safe and effective in once-daily administration, doxazosin is suitable for initial therapy in mild and moderate hypertension.

Reducing inadvertent PEEP by controlling end-tidal pressures in the trachea.

Mechanical ventilation using exhalation times too brief for completion of exhalation results in inadvertent positive end-expiratory pressure (IPEEP) and increased functional residual capacity (FRC). The endotracheal (ET) tube with side lumen allows us to monitor tracheal airway pressures and to determine the contributions of the ET tube to IPEEP. We hypothesized that, during rapid rate ventilation, controlling the positive end-expiratory pressure (PEEP) level in the trachea rather than at the ET tube adaptor will counter effects of the ET tube on IPEEP and result in less increase of FRC. Thirteen anesthetized rabbits were ventilated at rates of 30, 60, 90, and 120 breaths per minute (BPM). Peak inspiratory pressure was held constant, and PEEP was adjusted to 2 cmH2O, measured conventionally at the proximal end of the ET tube. Pulmonary function tests were made and then repeated while PEEP was held constant in the trachea, measured at the distal end of the ET tube. Controlling PEEP conventionally resulted in mean (+/- SE) FRC values of 13.7 +/- 3.8, 14.8 +/- 3.9, 17.1 +/- 4.5, and 21.1 +/- 5.3 ml/kg at 30, 60, 90, and 120 BPM, respectively. Controlling PEEP at the trachea yielded FRC values of 13.7 +/- 3.8, 13.6 +/- 3.4, 15.3 +/- 4.4, and 16.3 +/- 5.4 ml/kg, respectively. Increasing the ventilator rate above 60 BPM did not affect minute ventilation or blood gases. These results suggest that controlling PEEP in the trachea reduces effects of IPEEP on FRC by countering the contribution of the ET tube to the resistance of gas flow during exhalation.

The need for definitive therapy in the management of perforated gastric ulcers. Review of 202 cases.

A retrospective study of 202 patients who presented to three Louisiana hospitals with perforated gastric ulcers was performed. Patients were treated surgically, medically, or were not treated. Only those patients with gastric ulcers were included in the study, not those with "channel ulcers" or peptic ulcers. The mortality rate of the 185 patients who were treated with operation was 18%. Of those patients treated nonoperatively, the mortality was 95% (one survivor). There was a definite difference in mortality among those patients treated with definitive operative procedures (11.3%) versus those patients treated with nondefinitive surgery (22.9%). This difference was consistent even in the presence of purulent exudate in the abdominal cavity and among those patients presenting in shock. The mortality among those patients who presented with a initial blood pressure of 90 mmHg or less were defined as being in shock, and the mortality in this subgroup was 52.8%. The mortality among those patients who had purulent fluid within the abdominal cavity was about the same, regardless of type of surgical procedure. The patients who underwent nondefinitive procedures required subsequent operative treatment in 25.7% of cases. Also noted was the lack of correlation between symptoms of ulcer disease and perforation. Those patients treated with definitive surgery (57%) and those treated with nondefinitive surgery (41%) reported no symptoms consistent with ulcer disease before operation.
