[Diagnosis of gastrointestinal diseases in the aged].

It is a paper that surveys the multivarious diagnostic problems of gastrointestinal diseases in the aged. The particular characteristic of the acutely or chronically sick of advanced years presents itself, above all else, as a change in the organism itself, in a change of personality, and occasionally in the doctor's attitude towards his aged patients becoming different too. Relevant aspects of diagnosis in practice are of particular interest in this work.

[Use of anamnestic questionnaires in gastrointestinal diseases in the aged].

The authors relate their experiences in using questionnaires for taking the history of patients with gastroenterological enterological ailments. They have found that the older the patient, the more the results of the questionnaire diverge from those obtained by conventional methods of history-taking. Consequently the authors advise a cautious approach to the use of questionnaires when dealing with elderly patients.

[Endangering the water-electrolyte balance in gastrointestinal diseases in the aged].

In old people the range of operation, as well as the adaptability of the electrolyte and water metabolism, is reduced by the involutive reduction of the renal, pulmonary and endocrinic functions, as well as by a reduced feeling of thirst, a reduction of the buffer mechanisms, and a dwindling amount of body water. The threat to the maintenance of the requisite amount of body-water and to a normal electrolysis, caused by gastro-intestinal illnesses, is discussed from the point of view of geriatrics. Hydro-mineral upsets occurring in gastroenterology are primarily caused either by an inadequate supply, inadequate absorption, or by an extra-ordinary loss of intestinal secretions, as well as by interference with the normal electrolyte and water metabolism due to medical treatment. Situations in which such hydro-mineral disturbances wright occur are indicated and therapeutic measures suggested. We assert and prove that in old people the danger of extreme reduction of body-water and electrolysis frequently presents a greater threat to life than the illness from which it proceeds, and we maintain that a due consideration of this problem can in many cases literally save the life of the old patient.

Age-dependencies in the process of enteric absorption.

Founded upon experimental and theoretical investigation at age-dependence of the absorping system in human beings is analysed. Starting from a compartment model the absorption, velocity of absorption and effectivity of absorption are defined in the physical sense. The experimental investigations results in a general decrease of the velocity of all the processes in the absorping system with increasing age. This is the reason for a temporal changed kinetic behaviour of aged systems. By comparison between young and aged systems with reference to their kinetic properties we find the following results: With increasing age we observe a decrease of the velocity of absorption and of the effectivity of absorption, but with regard to the quantity of intestinal absorption not a difference between young and aged is pointed out.

The influence of biologically active substances on the regenerative processes of enterocytes in the small intestine of the rat.

The influence of a prolonged treatment with procaine or Turigeran on the cell population kinetics of the mucous membrane of the small intestine was studied by means of 3H-thymidine histoautoradiography in 240 12-month-old male Wistar rats. The investigations produced the following findings: 1. There are no differences in the cell population kinetics of the mucous membrane of the small intestine between procaine- and Turigeran-treated animals. But as, on the other hand, significant differences are found between treated and untreated animals in terms of all cell-kinetic characteristics, this effect has to be attributed to procaine alone. 2. In treated animals the partial stages of the cell cycle are prolonged as a result of which the generation time is delayed. 3. Due to the fact that the transit time of the cells in the crypts and on the villi is reduced, a greater number of labelled cells can finally be observed despite reduced proliferation rates in the crypts (labelling index) of treated animals. 4. Procaine will continue to act with undiminished effectiveness for at least two weeks after discontinuation of treatment.

[Addressing the problem of terminal care in medical education--results of a survey of 565 students and physicians].

Care for the dying is an important task for physicians. The ways in which this problem is dealt with during the medical training and the expectations of the people concerned were examined in 1989 and in 1994 by means of a questionnaire developed for this purpose. Altogether 565 junior doctors and medical students in Germany were asked. The number of people who declared that this topic was dealt with during their studies increased from 32.7% to 51%. The number of junior doctors and medical students who stated that they felt sufficiently prepared for care for the dying was low both in 1889 with 4.3% and in 1994 with 12.7%. Dealing with this problem seemed necessary to more than 97% of the people asked on both occasions. Results regarding the expectations and ideas of how and when this topic should be dealt with are also shown. Our results show that while there is a deficit in dealing with the topic "care for the dying" during medical training, there is a considerable interest in this subject among junior doctors and medical students.
