[Evaluation of a new latex test for detecting human rotaviruses in feces].

We have sought the presence of rotavirus in 237 fecal samples. Three techniques were applied to each sample: ELISA, latex agglutination (LTX) with two reagents (Rotalex and SlidexRotaKit), and electron microscopy after negative staining. The specificity of ELISA and LTX is very good (more than 93.4 per cent), the sensitivity is decreasing from ELISA to SlidexRotaKit and Rotalex (respectively 83.6-74.5 and 67.3 per cent). The advantages of LTX include rapidity, simplicity, low cost price.

[Pharmacokinetics of vancomycin in chronic renal failure patients in continuous ambulatory peritoneal dialysis (CAPD) after intra-abdominal administration].

Peritonitis is one of the most serious complication concerning patients under continuous ambulatory peritoneal dialysis (CAPD). A Staphylococcus (aureus, epidermidis) is the causative pathogen in nearly 60% of cases. This prompted a study of vancomycin, a potent antistaphylococcal agent (MIC less than or equal to 3 micrograms/ml) in 13 patients with peritonitis. Vancomycin was used as single drug therapy. Kinetics were studied after a single injection of 1 g into the dialysate bag. Serum and peritoneal concentrations exceeding 3-4 micrograms/ml were found to persist for four days; half-life was 62.3 and 54.8 h in the dialysate and serum respectively. Peritoneum to serum diffusion varied widely across individuals. In 15% of patients, serum concentration exceeded the potentially ototoxic level (80 micrograms/ml) for a few hours.

Long-term results of displaced talar neck fractures.

Of 36 fractures of the talar neck without comminution of the body, eight were undisplaced, treated closed, and 28 were displaced and treated by open reduction. Twenty of the operations were less than 12 hours after injury. Nineteen of 20 were performed through a medial approach, six with a medial malleolar osteotomy. The long-term results were evaluated by a standard rating system based on classification by the fracture. A protective brace was developed for non-weight-bearing in two patients with complete avascular necrosis, and ankle protection with weight-bearing in ten with partial necrosis. Prompt open reduction and internal fixation, malleolar osteotomy, and protected weight-bearing are recommended in selected cases.

[Digitalized thoracic radiography in cancerology. Present contribution apropos of 200 examinations].

The authors report a double study comparing the conventional chest radiograph and digitalised images. The first study was to visualise normal mediastinal structures (the contour of the posterior mediastinum, the azygo-oesophageal recess, the bronchial sub-segments, intermediate, carina, trachea) and showed a superiority of the digitalised image. The second study was on the recognition of radiological abnormalities in lung cancer which showed no precise superiority of the digitalised image in the mediastinum (bronchial tumour, adenopathies) and a current deficiency at the pleuro-parenchymal level (pulmonary nodules, atelectasis pleural effusion). This double study presents parallel results to those obtained by the anglosaxon authors on different prototypes in whom the technology and possibilities were different. The principles, the advantages and the limits of these prototypes were described and in conclusion a chosen technique for the medium term is proposed by the authors.

Immunogenic effect of hepatitis B vaccine in children: comparison of two- and three-dose protocols.

The immunogenic effect of hepatitis B vaccine was evaluated in 183 seronegative infants from Senegal. Seventy-two seronegative infants received two 5-micrograms doses of vaccine at a two-month interval and 111 seronegative infants received three 5-micrograms doses at one-month intervals. All the children had a booster dose one year after the first injection of vaccine. No difference between the two groups was observed in the seroconversion rate (93.1% and 94.6%, respectively); in the proportion of high anti-HBs titer; or in the anti-HBs geometric mean titer (82 and 92 mIU/ml, respectively). These results demonstrate that two doses of 5 micrograms of hepatitis B vaccine are sufficient in infants to obtain a high immunogenic effect.

[Intraocular penetration of ceftriaxone in man. Comparison with other beta-lactamins].

The intraocular penetration of a new antibiotic, ceftriaxon, was studied in 30 patients. Serum samples in 29 patients showed an average level of 42 micrograms/ml. Penetration into the aqueous humour flow studied in 28 patients under general anesthesia at one, two, three, four and six hours after the injection. An average level of 0,53 micrograms/ml was found which is 1,25% of average serum level. In 23 patients the dry level in tears averaged 5,75 micrograms/ml (13% of the serum level). In one case of retinal detachment the level in the subretinal fluid was 1,56 micrograms/ml. The kinetics drug secretion in tears was studied in 3 patients without anesthesia every 2 hours until the 16 th to 22 nd hours. Lower levels than those obtained under general anesthesia were obtained. This study shows that ceftriaxon has an intraocular penetration at levels above the minimal inhibitory concentrations for most commun intraocular infection.

Immune response to hepatitis B vaccine in infants and newborns: control trial in an endemic area (Senegal).

In 1978 it was suggested that hepatitis B (HB) vaccine should be used to prevent the early hepatitis B surface antigen (HBsAg) carrier state in children. Immunization was effected by 3 injections of HB vaccine at one-month intervals followed by a booster injection after one year. Children in a control group were immunized with DT-polio vaccine according to the same schedule. The anti-HBs response of the children to HB vaccination was studied in relation to their hepatitis B virus (HBV) serum markers prior to immunization. Of the seronegative children, 70.5% responded to immunization after 2 injections of 5- g doses of HB vaccine and 94% after the third injection. The efficacy of the vaccine was demonstrated by comparison of HB events after one year in 309 seronegative children immunized with HB vaccine and 252 seronegative children immunized with DT-polio vaccine, and after two years in 101 and 119 children, respectively. The incidence of the HBsAg carrier state was reduced by 80% in susceptible children. In order to eliminate the perinatal transmission occurring in newborns with HBsAg-positive mothers, a study of immunization at birth has been instituted. A total of 86 newborns responded to the vaccination as well as older children, irrespective of the HBV status of their mothers. After one year, the incidence of the HBsAg carrier state was reduced by 80%. In Africa, immunization teams have a limited amount of time to devote to each rural community. The immunogenic effect of 2 doses of HB vaccine given at an interval of 2 or 6 months has therefore been investigated. All were given a booster dose one year after the first injection of vaccine. No difference was observed in the seroconversion rate or in the anti-HBs titres as between the two protocols. These results demonstrate that 2 doses of 5 micrograms of HB vaccine are sufficient to obtain a high immunogenic effect in infants. In addition, an investigation was carried out on the immune response to HBsAg and tetanus toxoid antigen when administered simultaneously to children as HB vaccine and DT-polio vaccine. The immune response was at least equal to that observed after administration of these vaccines separately.

[Passage of ceftriaxone in the aqueous humor and tears. Comparison with other beta-lactams].

The intraocular distribution of ceftriaxone (a new cephalosporin of the third generation) was studied in 33 patients undergoing cataract surgery. The mean level of antibiotic in the aqueous humor was of 0,53 micrograms/ml (1.25% of the serum level), in the tears show an average rate of 5.75 micrograms/ml (13% of the serum level) was found. A study of the kinetics in the tears shows increasing levels for the first 18th hours.

Acute thoracolumbar burst fractures in the absence of neurologic deficit. A comparison between operative and nonoperative treatment.

The treatment of thoracolumbar burst fractures in the absence of neurologic deficit remains controversial. The present study is a retrospective analysis of 52 of these acute burst fractures among 104 cases of thoracolumbar burst fractures treated either operatively or nonoperatively. Results are expressed in terms of neurologic function, pain, work status, and complications. All patients who had surgical treatment and no unrelated disability returned to full-time work. Twenty-five percent of the patients treated nonoperatively were unable to return to work full time. Of the patients in the nonoperative group, 17% developed neurologic problems. Prophylactic stabilization and fusion of acute burst fractures without neurologic deficit have significant advantages over conservative management.

Comparison between square-ended distraction rods and standard round-ended distraction rods in the treatment of thoracolumbar spinal injuries. A statistical analysis.

This is a retrospective clinical and roentgenographic study of two different types of instrumentation used in the treatment of thoracic and lumbar spinal fractures. Dual square-ended distraction rods contoured into lordosis produced improved results when compared with round-ended distraction rods in terms of both the degree of reduction obtained on the operating table and preventing recollapse of the fracture in the ensuing month. The importance of restoring lordosis was more apparent in the thoracolumbar junction and lumbar spine than in the thoracic region.
