Use of Leu M1 and antiepithelial membrane antigen monoclonal antibodies for diagnosing Hodgkin's disease.

Biopsies of 82 patients diagnosed as having Hodgkin's disease were reviewed. Seventeen were reclassified histologically as non-Hodgkin's lymphoma or reactive lymphoid hyperplasia. A substantial number of cases of Hodgkin's disease were negative when stained with Leu M1. Staining for Leu M1 was not found in the cases of non-Hodgkin's lymphoma or reactive lymphoid hyperplasia. With the exception of the lymphocyte predominant nodular subtype of Hodgkin's disease, epithelial membrane antigen staining was seen in a few cases of Hodgkin's disease and non-Hodgkin's lymphomas. This was not a useful discriminating feature.

T-cell lymphoblastic lymphoma of the uterus complicated by Chlamydia trachomatis pneumonia.

This is the first documented case of a T-cell lymphoblastic lymphoma arising in the uterus. At presentation, the patient also had a life-threatening pneumonia due to Chlamydia trachomatis which responded to erythromycin and tetracycline. Cytotoxic therapy produced partial tumour regression, but the patient died 14 weeks after diagnosis, probably as a result of intercurrent infection.

Diabetes mellitus and primary hepatocellular carcinoma.

In view of the increasing incidence of primary hepatocellular carcinoma in western Europe and concern that this may in part be related to long-term use of drugs which cause hepatic microsomal enzyme induction, we undertook a comparison of long-term drug use in 105 patients with hepatocellular carcinoma and equal numbers of age and sex-matched patients with colorectal tumours and with fractures of femur. We found no patients with hepatocellular carcinoma who were long-term anticonvulsant users and only one who used oral contraceptives. However, we observed a four-fold excess of diabetic patients among the group with hepatocellular carcinoma. This association did not appear to be due to pre-existing haemochromatosis, alcoholic cirrhosis or viral hepatitis. The association was strongest in patients receiving drug treatment for diabetes, but the data, although suggestive, were insufficient to determine whether any specific anti-diabetic agent could be responsible. Further studies are required to elucidate the nature of this unexpected association. An association of this magnitude with diabetes mellitus could account at least in part for the increasing incidence of hepatocellular carcinoma in western Europe.

Small bowel ulceration in patients receiving non-steroidal anti-inflammatory drugs for rheumatoid arthritis.

Evidence is presented for a non-specific ulcerative process in the small bowel of patients with rheumatoid arthritis. All three cases presented with lower gastrointestinal bleeding; on withdrawal of non-steroidal anti-inflammatory drugs no further bleeding occurred. Non-steroidal anti-inflammatory drugs may lead to small bowel ulceration in inhibiting mucosal glycoprotein synthesis and increasing intestinal permeability.

Collagenous colitis: possible response to sulfasalazine and local steroid therapy.

A patient with rheumatoid arthritis and collagenous colitis apparently responding to sulfasalazine and prednisolone enemas is reported. It is suggested that this form of therapy should be considered in patients with this rare disorder not only as a short-term measure but also, in the case of sulfasalazine, as long-term treatment in patients with chronic symptoms.

Immunocytochemical demonstration of p21 ras family oncogene product in normal mucosa and in premalignant and malignant tumours of the colorectum.

Study of the distribution of the p21 ras oncogene product as demonstrated by monoclonal antibody Y13-259 shows this protein to be apparently present in all epithelial populations of both premalignant and malignant tumours and throughout the normal foetal and adult epithelial crypt population in the colorectum. Metastatic tumour in liver shows a similar staining pattern which is less intense however than in the surrounding normal hepatocytes. Our results suggest that the presence of this protein is a widespread feature of normal cellular metabolism in certain cell types and is not restricted to those actively involved in cellular proliferation. It appears, furthermore, that neither cells at different stages of carcinogenesis nor those representing variants of a malignant phenotype can be identified using this particular antibody.

A cloned MCGF cDNA encodes a multilineage hematopoietic growth factor: multiple activities of interleukin 3.

We recently isolated from a mouse T cell cDNA library a full-length clone that encodes a mast cell growth factor (1). On the basis of sequence homologies, this cloned factor must be similar if not identical to purified interleukin 3 (IL 3) (2, 3). Here, we report the first biologic characterization of the cloned gene product expressed in COS-7 monkey cells. Our results establish that a single molecular species promotes the growth and differentiation of a wide spectrum of hematopoietic cell types, including multipotential stem cells and various committed progenitor cells. The relationship of cloned IL 3 to other colony-stimulating factors is discussed.

Histology of rectal gonorrhoea in men, with a note on anorectal infection with Neisseria meningitidis.

The histology of the rectal mucosa in 57 men with culturally-proven rectal gonorrhoea is described and the results compared with the findings in biopsies from 57 non-infected men. An acute inflammatory cell infiltration of the rectal mucosa was found in three (5.3%) and two (3.5%) infected and non-infected men respectively. Twenty-one (36.8%) men with rectal gonorrhoea and nine (15.8%) non-infected men had an increased number of chronic inflammatory cells in the lamina propria.

The effect of cimetidine and 15(S) 15-methyl prostaglandin E2 on the healing of laser-induced gastric mucosal damage.

Delayed perforation, a complication of laser photocoagulation, could be reduced by accelerating healing and inhibiting gastric secretion. With the use of the Nd:YAG laser, a beam was passed through a single glass fiber (400 micrometer) with 10 degrees divergence at a constant power of 80 W for 1 sec at a 1-cm distance from the gastric mucosa. Rats were then randomly allocated to receive cimetidine (50 mg/kg), 15(S)15-methyl prostaglandin E2 (50 micrograms/kg), or saline intraperitoneally twice daily for 4 or 7 days. The surface area of ulceration at 4 days for the control rats and the cimetidine- and prostaglandin-treated rats was 29, 34, and 28 mm2, respectively, and was much larger than the initial lesion of 15 mm2. Histologically, all lesions showed the typical appearances of peptic ulceration with perforation in half the prostaglandin-treated rats. By 7 days, the surface area in the cimetidine-treated rats appeared to decrease, but the light microscopic changes were similar to the saline- and prostaglandin-treated rats. Neither cimetidine nor prostaglandin enhanced healing in this study.

Crohn's disease of the mouth: an indicator of intestinal involvement.

Nineteen patients with clinical evidence of oral Crohn's disease but no intestinal symptoms were studied. Oral lesions in all patients were shown histologically to have lymphoedema with or without chronic granulomas consistent with Crohn's disease. Seven patients (37%) had demonstrable intestinal disease on rectal biopsy and four of these had abnormal bowel radiology. All seven had evidence of nutritional deficiency. Patients with clinical features suggesting oral Crohn's disease may have evidence of Crohn's disease in the intestine, although this may not be clinically apparent.

Sigmoidoscopic and microscopic appearance of the rectal mucosa in homosexual men.

The aims of the study were to determine what microscopic changes occur in the rectal mucosa of men who have had anal intercourse and to correlate the sigmoidoscopic and microscopic appearances. Histological abnormalities were found in 29 of 100 men who attended consecutively a sexually-transmitted diseases clinic. The histopathology of rectal gonorrhoea, as observed in 18 patients, is described as are the microscopic findings in the rectal mucosa of 10 patients with early syphilis. Of 70 men without any detectable rectal infection, biopsies from 15 (21.4%) were abnormal. Intestinal spirochaetosis was observed in biopsies from 36 of these 100 men. With the use of strict criteria to describe the macroscopic appearance of the rectal mucosa, the sigmoidoscopic findings correlated well with the histology.

Coeliac disease associated with recurrent aphthae.

Fifty patients presenting sequentially with a history of recurrent aphthae were investigated for evidence of nutritional deficiencies and coeliac disease. In the group, two patients were found to have coeliac disease and their recurrent aphthae cleared soon after starting a gluten free diet. This study confirms the presence of an increased prevalence of nutritional deficiency and of coeliac disease in aphthous patients. However, it is recommended that jejunal biopsy be carried out in these cases only where there is evidence of malabsorption.

Evaluation of endoscopic brush and biopsy touch smear cytology and biopsy histology in the diagnosis of carcinoma of the lower oesophagus and cardia.

A four-year prospective study of endoscopic cytology and histology was carried out on 64 patients suspected of having a malignant lesion at the junction of the lower oesophagus and cardia. The final diagnosis was malignant in 18 and benign in 46 cases. Biopsy touch smear cytology was the most accurate technique with a sensitivity of 100% compared with 82% for brush cytology and 89% for biopsy histology. There was one false positive endoscopic histology report, but no false positive cytology result. Biopsy touch smear cytology is at present a method little used in the digestive tract. This study demonstrates its value in the diagnostically difficult area of the gastro-oesophageal junction.

Effect of an elemental diet (Vivonex) on the absorption abnormalities and histological appearances of the jejunum in untreated adult coeliac disease.

The effect of an elemental diet (Vivonex) together with a gluten-free diet on the absorption of water, sodium and chloride in the jejunum was studied in 4 patients with untreated adult coeliac disease before and after a 1-month course of therapy. The morphology of the jejunum was also studied by jejunal biopsy taken at the same time as the intestinal perfusion. The results were compared with those obtained in 4 patients with adult coeliac disease treated with a gluten-free diet alone. No marked improvement was noted in the transportation of water, sodium and chloride after either treatment with Vivonex and a gluten-free diet or after a gluten-free diet alone, and no marked histological changes were found. Clinical improvement occurred in both groups of patients, in that the diarrhoea improved in all patients and they generally felt better. There appears to be no additional advantage of using an elemental diet with a gluten-free diet in the initial management of adult coeliac disease.
