[Early bladder function disorders following Wertheim surgery. Causes and therapeutic consequences].

The bladder function of forty five women was studied once before and three times following a Wertheim Operation for Carcinoma of the Cervix. Three quarters of the patients had a diminished bladder capacity two weeks after surgery with an increased bladder filling pressure and a decreased urethral occlusion pressure. After eight and twenty weeks the pre-operative state was almost reached. However there was no complete restoration of the function at that time. Almost all women had a disturbance of the bladder sensitivity. Modern investigations of bladder physiology show that the lax bladder is an expected post-operative neurogenic problem of the vesical plexus. At first this is masked by a hypertonic bladder due to muscle trauma. The postoperative decrease of the urethral occlusion pressure appears to be caused by a partial injury of sympathic nerve fibers. The problem of functional infravesical obstruction is discussed. The treatment is at first accomplished by general perioperative measures. Later on receptor effective drugs are used as necessary following a repeat urodynamic investigation.

[Primary oxalosis in childhood. Plain film findings during the course of the disease (author's transl)].

The radiological changes in the kidneys and skeleton in primary oxalosis in a patient are described. Crystalline calcium oxalate deposits, which are a secondary phenomenon, are responsible for the findings, which are of great significance both functionally and prognostically. Hyperoxalaemia is due to an enzyme defect, which leads to hyperoxaluria; this causes a reduction in renal function and can lead to rapid progression of the condition.

[The value of oral gastrografin in the diagnosis of ileus (author's transl)].

One hundred and thirty examinations with oral gastrografin were performed in 110 patients with obstructive symptoms, who did not require immediate operation. They consisted of 49 patients with mechanical ileus, eighteen patients with paralytic ileus and 63 patients with incomplete mechanical ileus. All patients with complete mechanical and three patients with paralytic ileus were investigated surgically and treated in this way. Small bowel transit time longer than five hours provides considerable certainty of complete mechanical ileus. The method has been tried for four years; it makes few demands on the patient and has practically no contra-indications, except for deranged electrolyte balance. In children, the amount of contrast medium should be limited and it should be diluted or an isotonic contrast medium should be used. The method provides definite differentiation between mechanical and functional obstruction. In patients with adhesions and recurrent incomplete obstruction, further laparotomies may be avoided in this way.

[Angiographic localisation diagnosis via intra-arterial cytostasis catheters for the perfusion of orocavitary and pharyngeal tumours (author's transl)].

Correct positioning of the intra-arterial vascular catheter is essential for the technical execution of a fractionated, locally-acting (as far as possible) perfusion of medicaments with a minimum of side effects--now that selective chemotherapy has been introduced as part of a multi-method treatment of tumours in the head and neck region. For this purpose, angiographic imaging of the catheter topography with the relevant dependent vascular area is recommended. This procedure is relatively easy and allows immediate correcting of the catheter position, while taking into consideration the individual blood flow conditions. In a group of 70 patients subjected to this procedure by the author, no local complications were seen.

[X-ray-findings in intra-arterial chemotherapy of tumours of the head and neck (author's transl)].

On the basis of x-ray-findings in 70 patients with tumours of the head and neck, who had intraarterial chemotherapy the authors demonstrate the varying approaches to an indirect catheterisation of the external carotid artery. Paying due attention to possible complications arising from i.a. catheterisation, they stress the need for x-ray checks of the correct position of the catheter before commencement of intraarterial chemotherapy.

[Radiologic diagnosis and therapy of the viscerocranial extramedullary plasmocytoma].

A report is given about ten patients with an extramedullary plasmocytoma situated in the head/neck region. The diagnostic possibilities, the spontaneous development and the effect of radiotherapy with or without surgical treatment are evaluated. Ablation and subsequent irradiation with a focal dose of at least 50 Gy offer the best changes to achieve a recovery without recurrences. In case of osseous destruction, especially if the primary tumor is localized in the maxillary sinuses, it is very difficult to attain a permanent remission. If only soft tissues, are affected, a combined surgical and radiologic treatment is possible even in case of an affection of regional lymph nodes.

[CT diagnostic of ovarial tumors].

The diagnostic value of computed tomography was examined in 34 patients by the clinical diagnosis of an ovarial tumor established immediately before the operation. In 78,9% of all cases, a carcinoma could be surely recognized, often only by indirect signs of malignancy. Benign processes were correctly recognized in 83,3% of all cases. The staging of carcinomas was correct in 57,9%. A disadvantage of CT lies in the fact that it does not show peritoneal disseminations of smallest metastases as well as metastatic formations in not enlarged lymph nodes, which often leads to an understaging.

[Urinary bladder complications following Wertheim operations--recommendations for a controlled follow-up with comparable conditions (author's transl)].

At defined times prior and following a Wertheim operation for cancer of the uterine cervix the patients were investigated for the functional parameters of the lower urinary tract. The residual urine was determined. Stress incontinence and urgency incontinence were measured with urethrocystometry. The sensitivity and the micturition were evaluated from subjective data. The results of 237 investigations in 192 operated women are described and compared to previous papers on the subject. Stress incontinence increased to double the pre-operative incidence following the Wertheim operation. An urgency incontinence was quite rare. A lack of feeling of fullness to the bladder as a sign of disturbed bladder sensitivity was present 5 months post-operatively in 6 of 45 patients (13%). This was present in only 1 of 119 patients (1%) after five years. 1/3 of the women had difficulty with spontaneous micturition five months following the operation. 1/10 of the women had difficulty with spontaneous micturition 5 years after the operation. There was no residual urine five months after the operation in 9/10 of the patients and five years following the operation almost 100% of the patients had no residual urine. It was noted that difficulty with spontaneous micturition with residual urine and urgency incontinence did not necessarily result in concomitant bacteriuria. Suggestions are presented how future investigations with similar questions may lead to comparable results.

[Ulno-volar bayonet hand: its differential diagnosis from Madelung's deformity (author's transl)].

The ulno-volar bayonet hand related to the mostly hereditary multiple exostoses is compared to Madelung's forearm deformity under clinical and roentgenological view in differential diagnosis. The ulno-volar bayonet hand is considerably more seldom, basing upon dysplasia of the lower part of the ulna, less inconvenient in function, and hardly tending to the development of early arthrosis.

[The radiological diagnosis of urinary incontinence in a female (author's transl)].

Basing upon 77 patients examined the lateral cysturethrography with metallic bead chain is demonstrated as a special X-ray method in the diagnostic management of the female urinary incontinence in good reference to its diagnostic value in advance. A methodic limitation to the lateral voiding cysturethrography only means lack of information about the relations of the pathophysiologically important vesico-urethral connection in "relaxed" and "straining" state. The clinical graduation of stress urinary incontinence shows no proportionality in quantity according to the angles of the vesico-urethral connection revealable by roentgenogram.

[Radiological and clinical aspects of diaphragmatic injuries (author's transl)].

In experience of generally increasing frequency of diaphragmatic injuries, nowadays mostly caused by blunt body traumata in traffic accidents, a review is made about diagnostic and morphologic aspects outlining syndromal relations of 70 cases treated from 1950 until 1980 in the Department of Surgery of the Bonn University. Diaphragmatic tears following indirect (closed) trauma are surely more often associated with a transposition of abdominal organs (73%) than those secondary to direct (penetrating) wounds with an amount to 23%. In typical associated injuries like multiple rib fractures (29%, in relation to blunt injuries even 42%) and pelvic fractures (24% resp. 35%) diaphragmatic damage has to be investigated for if there is any doubtful abdominal or cardiorespiratory symptom. Especially in less symptomatic intervals roentgenographic methods come to use predominantly.
