[Treatment of inveterate unilateral dislocations of the mandible by open repositioning of the condyle and meniscectomy].

A new method for treating inveterate forms of mandibular luxation is described, after a brief review of conventional therapeutic procedures. The route of approach chosen provides wide exposure of the temporo-mandibular joint, the endoarticular approach ensuring protection of pericondylar venous plexuses and of the internal mammary artery by allowing section of the external pterygoid tendon and the performance of a juxta-cervical capsulotomy. Excellent results were obtained in two patients with unilateral inveterate dislocation of the mandible operated upon using this procedure.

[Retrospective study of the prognostic value of histological criteria in lymphoma of the ORL region].

The histological study of 36 cases of Non-Hodgkin Lymphomas beginning for the E.N.T. area has allowed us to show that the Non-Hodgkin Lymphoma Working Formulation made for the lymph nodes may be used to ENT Localization and give a reliable prognosis. Other elements like: sex, clinical staging, treatment have no significant value concerning the survival rate in our study. This study agrees with the previous studies about the predominance cases to the high power of malignancy of these lymphomas.

[Anatomo-pathologic study of the temporal bone in our 4 cases of sudden infant death].

A new etiopathogenic theory is proposed by the authors about four cases of sudden infant death syndrome. A new histological method on the temporal bone was used applied in these four cases. Inner ear and meningeal injuries were observed. From this observation they proposed the possibilities of brain-stem injury through neurogenic or meningeal pathways. These injuries could be responsible for apnea syndrome and loss of alertness.

Surgical procedure and implanted material description.

The 12 intracochlear electrode placement is greatly facilitated by using our electrode-bearer which is introduced through the round window via a posterior tympanotomy approach. However, partial or complete ossification of the cochlea may oblige us to use again our first technique of one-by-one electrode placement, using Silastic pieces through 12 cochlear fenestrations. The microconnectors and the receptor are also described.

[Laryngeal immobility after intubation].

Based on findings in 14 cases of post-intubation laryngeal immobility simulating a paralysis, it is demonstrated that a mechanical cause bringing into play the arytenoids exists in every case. The origin may be from a synechia between the vocal cords, crico-arytenoid ankylosis, inter-arytenoid and plate of the cricoid fibrosis, or a combination of these lesions. Conclusive evidence of the type of lesion involved is rarely apparent from clinical or paraclinical examinations, an exact diagnosis requiring a median thyrotomy. This emphasizes the need for pathologic investigation of the arytenoidectomy surgical specimens. These findings suggest that neurogenic theories concerning the etiopathogenicity of post-intubation laryngeal immobility be revised.

Clinical results of the French multichannel cochlear implant.

Clinical assessment and results are described in 66 patients (among them 20 were less than 20 years old). Voice changes may be objectively observed on sonograms. Phonetic discrimination depends on the electrical and nervous status of the implanted deaf cochlea. Discrimination of words and sentences with reference list, but without lipreading, is commonly obtained. Discrimination of unknown words is more difficult and varies from one patient to the other. Discrimination with lipreading progress elicited by the device depends on the pre-operative status and the own possibilities of each patient. Psychoaffective improvement is evident in every case, especially in 3 cases of autistic deaf-mute children. We shall try to summarize the efficacity of the French 12-channel cochlear implant regarding the clinical results we obtained in 66 patients who have been implanted between October 1976 and September 1983. Among them, 39 suffered from pre-lingual deafness and 27 from post-lingual deafness.

Sound signal processing.

In the CHORIMAC-12, the French device, the whole speech information despite its redundancy is transmitted to the few cochlear nerve fibers that are still present. Only loud speech intensities between 40 db and 100 db are introduced in a filter battery which gives out the frequency information in 12 channels including the 100-6000 Hz. This 60 db intensity scale is transmitted in a compression rate of about 6 db. The time sampling is 3 msec. A 12 electrode bearer or 12 separate electrodes (in case of ossified cochlea) transmit the electrical information. The electrical stimulus is a square wave: its frequency is equal or inferior to 300 Hz; its voltage may vary between 2.5 and 6.5 volts; its duration may vary between 0 and 200 mu sec. The performances of the system is demonstrated by means of a system of computerized psychophysic tests and speech intelligibility tests.

[Classification of laryngotracheal stenoses in the adult].

A new classification for laryngotracheal stenoses in adults is proposed, based on 145 cases treated between 1965 and 1980. Three factors were analyzed when establishing this classification: the extent of laryngeal and tracheal cartilage involvement, laryngeal mobility, and the complexity of the lesions as a function of whether they were systematized or not. This new approach enables a more functional therapy for these stenosis to be instituted.

[Etiology of laryngotracheal stenoses in the adult].

Laryngotracheal stenosis in 145 adults was the direct result of laryngeal lesions in 27 cases and of iatrogenic etiology in 118. External injury was at the origin of most of the direct cases, the resulting complex anatomical lesions being treated with difficulty and having a very poor prognosis. Lesions of an indirect origin were principally sequelae from intensive care, their analysis and treatment being more easily systematized.

[Preliminary attempt at a classification of rhinopharyngitis].

A study was conducted in 113 children with recurrent attacks of rhinopharyngitis, a frequent affection treated in ENT practice, to determine a classification and establish the etiopathogenicity of the disorder. Children were divided into two groups according to age, and clinical, radiologic, bacteriologic, biologic, and histologic data analyzed. An allergic or immunity deficiency origin could be confirmed in some cases, but etiopathogenicity was impossible to determine in many individual cases.

[Preliminary results of a preoperative polychemotherapy protocol in carcinomas of the pharyngo-larynx. Oncovin, methotrexate and bleomycin (author's transl)].

213 squamous carcinomas of the pharyngo-larynx (piriform fossa, pharyngo-laryngeal junction and larynx) received preoperative chemotherapy in the form of oncovin, methotrexate and bleomycin (OMB). All the patients then underwent total or partial laryngectomy, associated with lymph node excision of some sort, followed by radiotherapy. A symptomatic improvement after OMB was seen in 21.2% of cases. Overall tumour response regardless of the site was 32%. Tumour response was all the better when lymph nodes were histologically free of disease (38.7% tumour response in N- as against 26.7% in N+). There was no significant difference in 1 year survival rates between those patients in whom there was a tumour response and those in whom tumour size remained the same.

[Tracheotomy in the surgical treatment of carcinomas of the pharyngo-larynx (author's transl)].

The authors studied the local postoperative course and period of hospitalisation after 890 total and partial pharyngo-laryngectomies according to whether the operation was started under general anaesthesia by intubation or after tracheotomy under local anaesthesia. The authors show the improvement in postoperative course as well as the decrease in the period of hospitalisation when intubation was used first.

DNA hybridization for detection of human cytomegalovirus in bronchoalveolar lavage and pharynx biopsy.

A simplified hybridization procedure was used for detection of cytomegalovirus (CMV) in human specimens. The probe was a 32P-labelled cloned DNA fragment of CMV strain AD169. This probe did not hybridize to DNA from uninfected cells or other herpesviruses (herpes simplex virus, Epstein-Barr virus). Specific hybridization was obtained with unselected bronchoalveolar lavage specimens, but the sensitivity of the test (33%) was lower than that of culture (80%) and immunofluorescence (60%) assays which are routinely performed in our laboratory. The detection procedure was also carried out with pharynx biopsy specimens which had been kept frozen at -70 degrees C. CMV DNA was detected in 14% of tumour specimens and only in 1.7% of control specimens (p less than 0.05). The indications of DNA hybridization for CMV diagnosis are discussed.

The velo-impedancemetry. A new technique for dynamic study of soft palate.

Using a slightly modified electro-glottograph, we designed a device for the registration of soft palate movements. Correlating such curves with corresponding phonogram and physio-pathological data, we have been able to define an iconographic semiology, which may be useful in the functional pathology of the soft palate and indications of uvulo-pharyngoplasty for snoring.
