Circumscribed pilary dysembryoplasia of the palms.

The published 9th case of circumscribed pilary dysembryoplasia of the palms is presented. Differences between this case and Becker's pigmented hair nevus, congenital smooth muscle hamartoma, circumscribed acquired hypertrichosis and pigmented nevus are discussed.

Chloracne caused by ingestion of olive oil contaminated with PCDDs and PCDFs.

1. All members of a Spanish family (father, mother and six children) developed chloracne. 2. The causative agent was found to be the family's stock of olive oil, which had become contaminated with polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs), pentachlorophenol, and hexachlorobenzene. 3. The more highly chlorinated PCDDs, in particular octachlorodibenzo-p-dioxin, were the predominant congeners in the oil. 4. Three members of the family exhibited either an overt or a sub-clinical disturbance of kidney function. The father also had a chronic respiratory problem. These changes could not be unequivocally attributed to the PCDDs. 5. Experimental toxicity of the oil was limited to the development of an hepatic porphyria in mice. 6. A serum sample, taken 5 years after consumption of the oil ceased, contained high levels of the PCDDs and PCDFs. Extrapolation back to ingested dose was used to validate dosage estimates. 7. The use of toxicity equivalence factors (TEFs) provided estimates of cumulative dosage to produce chloracne as 0.13-0.31 micrograms 2378-TCDD kg-1 (using EPA TEFs) or 6.7-16 micrograms 2378-TCDD kg-1 (using Nordic/NATO TEFs). 8. This is the first incident in which human toxicity is related primarily to ingestion of PCDDs and for which estimates of dosage can be made.

[AM3 (a biological response modifier) in the treatment of recurrent aphthous stomatitis. Clinical evaluation and preliminary studies on NK (CD16) cells and their pathogenic role in the syndrome].

In an open-controlled trial--oral washes (20 patients) versus test (19 patients)--, we have studied the effects of AM3 (a new oral BRM) on clinical evolution of the recurrent stomatitis (RAS) syndrome. The results obtained at 6th month showed significant decreases on ulcer numbers (p less than 0.001) as well as in their mean duration time (p less than 0.001) due to the AM3 treatment. From a pathophysiologic point of view, the study of the NK peripheral blood cells (Leu 11/CD16) suggests the existence of two kinds of RAS-patients: those showing normal NK cell numbers (approximately 33%) and those ones showing a partial lack in the NK numbers (approximately 67%). These results suggest different rational new approaches to treatment, based on new pathophysiological concepts.

[Serum sialic acid bound to lipoproteins. Its contribution to the diagnosis and follow-up of melanoma].

Plasma lipid bound sialic acid (LSA) was measurement in 41 patients with malignant melanoma to determine the utility of this biochemical marker in the stating and in monitoring the post-operative follow-up. LSA was determined in 26 cases pre and post-operation, and in 15 cases only post-operation. Multiple determinations was done in 12 cases during two years of post-operative follow-up. Patients with preoperatory determination exhibited LSA levels significantly higher (p 0.0001) than the healthy control group. Relationship between LSA highest levels and tumoral size up 2 cm. in diameter was found (p 0.02). A second determination after 2 months of cases. LSA multiple post-operative controls showed continuing elevation of LSA levels up the normal rates was corresponding with metastasis in 100% of cases.

[Treatment of condylomata acuminata using liquid nitrogen].

We treated 105 patients with anogenital warts with cotton swabs soaked in liquid nitrogen. 90.1% of the patients were cured, but recurrences were observed in 19.5%. With this method we obtained better clinical results than those obtained with the cryoprobes system. Good results were obtained when the lesions were accessible for treatment, but neither the time of evolution nor the size influenced the cure.

[Treatment of skin ulcer using oil of mosqueta rose].

Oil rose of mosqueta (Rosa aff. Rubiginosa L.) is a concentrated solution in linoleic (41%) and linolenic acid (39%), that offers benefit therapeutic effects in the wound healing. Ten patients affected of leg ulcers and post-surgical wounds were treated by 26% oil concentrated rose of mosqueta with very notable improvement on its healing compared with the control group. Due to the lack of side effects, we believe rose of mosqueta oil is very usefull to these conditions. Mechanism of actions and others indications are discussed.

[Pseudoatrophic pityriasis versicolor].

The pseudoatrophic pityriasis versicolor is an atypical form of this disease with cutaneous atrophy provoked by the prolonged application of topical corticosteroids. The term "pseudoatrophic" is indicative of its reversible character.

[Cystic lymphangioma].

A case of clinical and dermopathological characteristic cystic lymphangioma is reported. The atypical localization of the tumor and its delayed apparition were pointed out. A relation with any precipitating factor could not be established. Treatment was surgical remove.

[Wells syndrome (3 cases)].

This report describes three cases of eosinophilic cellulitis (Wells' syndrome) observed during six years. In the author's opinion this condition is not frequent, but neither exceptional. A revision of clinico-pathological, etiopathogenic and therapeutics aspects is carried out.

[Anogenital warts in children].

Fourteen cases of anogenital warts in children are reported. Girls were more frequently affected than boys (9/5). Three children had been infected during birth; two had autoinfection from non-anogenital warts; two girls had apparently been sexually abused. In the remaining 7 cases the origin of the warts was unknown. The authors discuss the epidemiological, therapeutic and social implications of this little known but important paediatric pathology, pointing to the possibility of sexual transmission in cases of "apparently" unknown origin.

[Acute neutrophilic febrile dermatosis (Sweet syndrome) associated with melanoma].

A 45-years-old woman affected of Acute febrile neutrophilic syndrome is presented, with no reliable etiology. In a rutinary control, a pigmented ledion of one year of evolution was observed and a surgical excision was performed, with histological diagnosis of spreading superficial melanoma. In the next weeks of post-operatory, the Sweet's syndrome was decreased, disappearing completely two months later. No recurrences of Sweet's syndrome or malignant melanoma were observed after two years of follow-up. Association between Sweet's syndrome and neoplasias, in particular solid tumors, is commented, and the literature is reviewed.

Nevoid basal cell carcinoma syndrome: combined etretinate and surgical treatment.

Aromatic retinoids have been shown to be useful in the management of various genodermatoses, including nevoid basal cell carcinoma syndrome (NBCCS). A patient with this condition was treated by combined etretinate and surgical therapy. Over the course of 12 months, the patient was placed on oral etretinate therapy, with a dosage of 1 mg/kg/day for 3 months, and 0.5 mg/kg/day for 9 months. In the first period, notable tumor reduction was observed in the larger tumors. Complete clinical regression was seen in only 26% of BCCs with an overall tumor size of less than 1 cm in diameter. In the second period, no tumor changes were observed, but no new tumors appeared. Surgical excisions of the remaining tumors were carried out 3 months after the end of the etretinate treatment, with excellent results and no recurrences after 6 months of follow-up. Treatment with etretinate permits a less aggressive surgical excision and recurrence rate than surgical excision alone.
