[Helicobacter pylori in patients with gastritis and gastric ulcer].

Seventy patients symptoms and signs compatible with gastritis and/or peptic ulcer were included in the study; they were 41 women and 29 men, with an age range of 9-84 years, and they underwent upper endoscopy. Brushing of the antrum was performed and 4 biopsies were taken from it. With the gastric mucus, a smear was prepared for Gram staining; one sample of tissue was placed directly in urea medium and another tissue sample in Skirrow medium; and two samples of tissue were stained with Hematoxylin-eosin and examined under the light microscope. Those patients who were taking antibiotics capable of inhibiting growth of Helicobacter pylori (Hp), the last four weeks prior endoscopy, were excluded from the study. Twenty-six patients (37.1%) were positive (+) for Hp the culture in urea medium. Of these 26 patients, 15 (21.4%) were also positive by the Skirrow method, and 11 (15.7%) by Gram stain. Epigastric pain was the most predominant symptom in (+) patients (80.7%) and (-) patients (68.1%) for Hp. Superficial acute gastritis within the antrum was the most frequent endoscopic finding in (+) (84.6%) and (-) (68.1%) patients. Chronic superficial gastritis was the predominant microscopic finding in (+) (42.3%) and (-) (38.6%) patients as well. Microscopic examination was positive for Hp in 6 (23%) patients by H-E stain. According to these results, there was no statistically significant difference in symptoms, endoscopic and microscopic findings between positive and negative patients for Hp.

Familial pachydermodactyly.

Pachydermodactyly is a rare form of digital fibromatosis. There are only 15 cases reported in the literature, all with a negative family history. Only one of the previously reported cases was a woman. We now report 2 female cases, mother and daughter, who, to the best of our knowledge, are the first patients with familial pachydermodactyly. The histological evolution of this disease over the years had not been previously reported.

Rapp-Hodgkin syndrome with pili canaliculi.

A 20-year-old woman and her 12-year-old brother had hypohidrotic ectodermal dysplasia, cleft lip and palate, midfacial hypoplasia with narrow nose from the nasal bridge to the tip, narrow dysplastic nails, and conical teeth and hypodontia, and hypospadias and hypoplastic uvula in the boy. The woman had major underdevelopment of intellectual capacity. The most important hair anomalies in both siblings were sparse eyebrows, pili torti, and pili canaliculi. Some of the pili canaliculi had two canals (pili bicanaliculi), and the cross section for scanning electron microscopy had a quadrangular aspect. This is the seventh family reported with Rapp-Hodgkin ectodermal dysplasia.

Late onset Papillon-Lefèvre syndrome? A chromosomic, neutrophil function and microbiological study.

We had the opportunity to study a family with one of the most destructive forms of periodontal disease known, the Papillon-Lefèvre syndrome. The parents had no consanguinity and were not affected, and were therefore to be considered carriers of the disease. 2 sisters, the eldest and youngest, showed periodontal breakdown and hyperkeratotic skin lesions, but their deciduous dentition was not affected. 2 brothers had skin lesions only and another brother and sister were healthy. Furthermore, 2 babies died at birth one after a 9-month pregnancy and the other after a 6-month pregnancy, and the mother also suffered 3 miscarriages. For 4 years, we studied the family: in the case of both sisters, mechanical periodontal treatment and antibiotics were unable to control the disease. In the chromosomic study of the 2 sisters affected, the GTG banding technique found no trace of anomalies in the cells analyzed, whose chromosomic formation was 46,XX. Before treatment, the chemotaxis of the PMN, the phagocytosis of opsonized Staphylococcus aureus, and production of superoxide radicals by PMN was significantly impaired in both sisters. Despite scaling and root planing, the periodontal lesions still progressed, but the PMN functions evaluated were now normal in both sisters. An orally asymptomatic but dermatologically affected brother showed no significant defect in the phagocytic activity and the production of superoxide radicals.

Epidermotropic metastasis coexisting with multiple primary cutaneous malignant melanomas.

A 45-year-old male, with a history of malignant melanoma on the thorax excised 4 years ago, presented a sudden eruption of more than 300 pigmented lesions. Fifteen excisions of tumors in different stages were carried out on four different dates. Dermatopathological examinations revealed features of primary as well as metastatic malignant melanomas. We suggest that besides the extensive dissemination there was also an activation of the resting melanocytes, that led to the formation of multiple primary melanomas.

Stereoselective R-(+) enantiomer of HA-966 displays anxiolytic effects in rodents.

Anxiolytic agents disinhibit suppressed behaviors in rodents in preclinical models of anxiety such as the non-conditioned social interaction and elevated plus maze assays and the conditioned conflict Cook and Davidson procedure. The (+) and (-) enantiomers of (+/-)-3-amino-1-hydroxy-2-pyrrolidinone (HA-966) have been resolved and revealed that R-(+)-HA-966 significantly disinhibits both non-conditioned and conditioned suppressed behavior similar to the benzodiazepine diazepam, while the S-(-) enantiomer was devoid of anxiolytic activity and only produced behavioral sedation. Furthermore, R-(+)-HA-966 lacked side-effects in rodents commonly associated with the administration of benzodiazepines such as motor incoordination and ataxia, significant interactions with ethanol, and amnesia. These data suggest that R-(+)-HA-966, an antagonist at the strychnine-insensitive glycine/NMDA receptor site, was anxioselective and lacked some of the side-effects associated with benzodiazepine anxiolytics.

[A topical solution of 40% idoxuridine in dimethyl sulfoxide compared to oral acyclovir in the treatment of herpes zoster. A double-blind multicenter clinical trial].

BACKGROUND: Both topical 40% idoxuridine in dimethylsulfoxide (IDU) and oral acyclovir (ACV) are useful in herpes zoster (HZ). This is the first clinical trial which compares the efficacy of both drugs in the course of the disease and in the prevention of post-herpetic neuralgia (PHN). METHODS: Patients of both sexes older than 18 years, with a HZ of less than 4 days were selected. Patients with otic or ophthalmic zoster, serious concomitant illness or pregnant or breast-feeding women were excluded. Following a double dummy design, the patients received at random topical IDU and oral placebo, or oral ACV and topical placebo. Topical treatment was applied during 4 days and oral treatment during 7 days. The evolution of the disease (number of individual lesions, evolution of symptoms, use of analgesic drugs and eventual appearance of complications) was controlled on days 0, 2, 4, 6, 8, and weekly until its resolution. RESULTS: The group of patients treated with IDU (85) showed a better evolution of the disease than those treated with ACV (86) in some of the parameters controlled: day of first and all vesicles drying (p less than 0.05), last day of moderate-intense pain (p less than 0.05), hyperaesthesia (p less than 0.05) and itching (p less than 0.05) and last day of analgesic use (p less than 0.01). The appearance of new vesicles during treatment was lower in the IDU treated group (p less than 0.01). A tendency favouring IDU can be observed in the appearance of PHN. CONCLUSIONS: In our study topical 40% idoxuridine in DMSO was better than oral acyclovir in 7 of the 14 clinical parameters studied.

Behaviour modification in obese subjects with type 2 diabetes mellitus.

We have followed prospectively, 46 obese, type 2 diabetic patients for a 55-week period, in order to evaluate the efficiency of an educational programme based on behaviour modification to enhance weight loss and changes of other cardiovascular risk factors. No patient received pharmacological treatment during the study. At the end of the follow-up the patients obtained an average weight loss of 9.250 kg (range: 0.500-17.500 kg); the BMI was reduced from 34.2 +/- 0.8 kg/m2 to 30.6 +/- 1.1 kg/m2 (P less than 0.01); fasting serum glucose descended from 7.9 +/- 0.4 to 6.1 +/- 0.5 mM (P less than 0.05); SBP (systolic blood pressure) decreased from 145.7 +/- 3 to 126.4 +/- 5.1 mmHg (P less than 0.01); DBP (diastolic blood pressure) decreased from 83.5 +/- 2.5 to 65 +/- 2.6 mmHg (P less than 0.01); triglyceride levels were lowered from 164.5 +/- 12 to 109.7 +/- 10 mg/dl (P less than 0.01); HDL-cholesterol levels increased from 1.27 +/- 0.05 to 1.53 +/- 0.12 mM (P less than 0.01). Serum glucose 2 h after a 75 g glucose oral load decreased from 14.9 +/- 0.6 to 12.7 +/- 0.9 mM (P less than 0.05) on week 35 of follow-up. Twelve patients no longer presented a diabetic curve (8 normal oral glucose tolerance test (OGTT) curves, and 4 impaired glucose tolerance (IGT) curves). No significant changes in the parameters studied were obtained in the group of patients on conventional treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

Total upper lip reconstruction with bilateral fan flaps.

Reconstruction of the total upper lip may be easily performed using fan flaps. However, two details must be taken into consideration: 1) the wide pedicle in the commissural angle, including the facial artery, must be designed, and 2) the mucosal flap must be 8 mm longer, in the inner portions, than the skin flap used to reconstruct the lip semimucosa.

[Tinea faciei: epidemiology].

Because of its clinical peculiarities responsible for many diagnostic errors resulting in potentially toxic treatments, tinea faciei should be regarded as a process with a personality of its own. The authors have reviewed 46 cases of tinea faciei, evaluating and highlighting their clinical and epidemiological peculiarities. As regards age and sex, two peaks of maximum incidence can be found: one in childhood, the other in adults older than forty. Many lesions progress over more than 12 months before they are diagnosed. Their peculiar clinical features suggests various initial diagnoses, the most frequent of which are related to photosensitivity (lupus erythematosus, polymorphous light eruption). Among the most frequently isolated dermatophytes, T. mentagrophytes var. granulosa and T. rubrum are outstanding.

Sexually transmitted diseases in homosexual males in Seville, Spain.

BACKGROUND AND METHODS: The absence of any official statistics on the prevalence of STD in homosexual men in Spain induced us to carry out a prospective study of new homosexual patients who consulted the STD Clinic of the School of Medicine in Seville, between January 1988 and December 1989. The aim of the study was to determine the prevalence of symptomatic and asymptomatic infections in this group of patients. RESULTS: 1805 patients were seen during the study period; 318 patients were homosexual of whom 309 agreed to participate in the study. Of the 309 homosexual men, 108 (35%) had symptoms and the remaining 201 (65%) were asymptomatic. In the symptomatic group the diagnoses were: syphilis 28 (25.9%); urethritis 40 (37%) (of these 40, 11 had Neisseria gonorrhoeae, five had Chlamydia trachomatis, five had Ureaplasma urealyticum, one had Herpes simplex virus and in 18 no pathogen was detected); genital herpes seven (6.4%). Eleven (10%) had concomitant infections. The following infections were found in the asymptomatic group: syphilis 23 (11.4%), N gonorrhoeae six (3%), C trachomatis two (1%), Herpes simplex virus one (0.5%). Antibodies against HIV were detected in 30 (9.6%) of the total group. CONCLUSIONS: Sexually transmitted diseases are common amongst homosexual men in Seville and many of these are asymptomatic.
