[CRST syndrome. Ultrastructural and physico-chemical studies of calcifications (author's transl)].

The subcutaneous calcifications in a case of CRST syndrome were studied by electron microscopy and by several physico-chemical tests in order to establish their structure and development. Samples of powder, obtained by crushing and homogenisation of the deposits, were subjected to the following analyses: - radiocrystallography by X-ray of the raw and heated material - infra-red spectroscopy before and after heating - quantitative chemical analysis and differential thermal micro-analysis. Connective tissue fibres seemed to play an important part in the development of the calcifications. The initial deposits of calcium first appeared in the central part of elastic fibres whose general morphology remained nevertheless undisturbed. In the older deposits disorganised elastic (and possibly collagen) fibres and areas of bone formation were found. Evidence of an inflammatory reaction was always minimal or nil. There was considerable variation in the calibre of collagen fibres in the non-calcified zones, the smaller fibres predominating. These appearances, sometimes noted in scleroderma, seem to be associated with increased collagen synthesis. X-ray diffraction and infra-red spectroscopy studies show that the deposits consist essentially of type B hydroxyapatite carbonate, confirmed by differential thermic analysis. On electron microscopy the calcifications were shown to comprise straight, needle-like crystals, 100-600 nm in length and 30-100 nm broad, characteristic of the apatites. Very occasionally, target-pattern deposits conforming to whitlockite were encountered. Chemical analysis revealed a high proportion of calcium with a Ca/P ratio greater than 1-666 as is typical of synthetic hydroxyapatite carbonates. The deposits also contained up to 20% by weight of organic material.

[Treatment of complete acromioclavicular luxation with a coracoclavicular screw].

In 12 cases of acromioclavicular luxation surgical treatment was applied by applying a coracoclavicular screw. In 8 of the 12 operated cases the results were excellent. The patients could resume their old job. After removal of the device used for osteosynthesis, no symptoms or late relapse appeared. In 2 patients the results were good, in 1 fair and in one patient a relapse was observed. It is suggested on the basis of the good results that the method should be considered suitable for clinical use.

[An experimental study of the results of compression of the growth plate (author's transl)].

The authors, using external fixators, have applied measured compression forces through the growth plate of the lower end of the femur in rabbits. They have calculated the degree of compression necessary to slow down or to stop growth. It was shown that growth may become normal following release of the compression provided that the compression was not excessive. An histological study was conducted with showed lesions varying from a simple diminution in the height of the cellular columns to the development of bony bridges depending on the intensity of the compression.

Clinical results of the triangular external fixation device in serious lesions of the lower extremity.

Several types of external fixation devices exist among which those of Judet, Hoffmann, Wagner and Müller can be picked out, each of which has definite applications. Confronted with difficulty in the use and the defect of mechanical rigidity, we have been led to design a new type of the triangular external fixation device. As open fractures of the limbs in stage II and IV of Duparc and Cauchoix, and pseudoarthrosis infected need a considerable stability in addition to careful surgical treatment, so after a comparative mechanical study, and after codifying the most rigid installation, we have used it on several traffic accident victims.

[Direct surgery of traumatic injuries of the radial nerve (author's transl)].

33 cases of traumatic lesions of the radial nerve are reported. Etiological features, prognosis, surgical treatment including micro-surgical techniques, leading to a larger indication of direct nerve repair are discussed. Recent nerve wounds must be repaired early. Surgical exploration will be carried out on unopened lesions when no recovery occurs within two months. According to the lesion, peri-fascicular neurolysis or fascicular resection-graft will be achieved. Tendon transfer techniques will be proposed only after one or two years without recovering, according to the level of the injury.

[Abdominal tomometry. 1st results in exploration of the liver and pancreas].

Over a two month period, 120 patients were explored using an Acta Scanner tomometer for various hepatic disorders. In parallel, comparison was made of anatomical slices and computed tomography findings in cadavers and isolated livers. This made it possible to recognise normal structures seen at computed tompgraphy and to identify certain hepatic images which might be sources of error. Serial transverse axial sections were used to reconstruct the whole liver in space, thereby making it possible to eliminate certain cases of pseudotumoral hepatomegaly. Systematic densitometry was carried out using hepatic radio-anatomical and pathological studies. This new type of analysis led to a quantified diagnosis in certain diffuse hepatic disorders and a histological approach to tumour lesions. Hepatography using special contrast agents in very low dosage was used in liver computed tomography after animal experiments. This method, free of untoward effects, makes possible the diagnosis of hepatosplenic tumour lesions less than one centimetre in diameter.

Serial axial transverse sections were used to reconstruct the volume of the liver in space, thus making in possible to eliminate certain forms of pseudo-tumoural hepatomegaly. Systematic densitometric analysis was made by radiological anatomical study and in hepatic pathology. This new type of analysis led to a quantified diagnosis of certain diffuse hepatic diseases and a histological approach to tumour lesions. Hepatography using liposoluble agents in very low dosage was used in hepatic tomometry after animal experimentation. The method, free of untoward effects, makes possible the diagnosis of hepato-splenic tumor lesions less than one centimetre in diameter.
