Adenovirus detected by polymerase chain reaction in multidose eyedrop bottles used by patients with adenoviral keratoconjunctivitis.

PURPOSE: We investigated the potential of a multidose eyedrop bottle used by patients with adenoviral keratoconjunctivitis as a source for spreading infection. DESIGN: Prospective consecutive case series. METHODS: The contents of multidose eyedrop bottles given to patients with adenoviral conjunctivitis and in use for 1 week were analyzed by polymerase chain reaction for adenovirus after as long as 9 weeks of preservation at room temperature. RESULTS: Of 26 patients with adenoviral keratoconjunctivitis, the eyedrop bottles of 19 patients (73%) were positive for adenovirus. The maximum detection interval was 9 weeks. Significantly higher prevalences of intrafamilial infection (P =.0098) and of corneal subepithelial opacity (P =.046) were observed among cases with adenoviral contamination than among cases without contamination. CONCLUSIONS: Multidose bottles used by patients with adenoviral keratoconjunctivitis are a possible vector for viral transmission for as long as 9 weeks.

Acute asthma attack caused by ophthalmic application of antiallergic agents.

BACKGROUND: To investigate the induction of acute asthmatic attacks caused by several kinds of antiallergic eyedrops for the treatment of allergic conjunctival diseases in a patient with bronchial asthma and aspirin sensitivity. CASE: A 42-year-old man with a 10-year history of bronchial asthma and with aspirin sensitivity, who had been given disodium cromoglycate (DSCG) to be applied topically, developed asthma after applying DSCG drops. OBSERVATIONS: After the instillation of DSCG, tranilast, ibudilast, and ketotifen, the peak expiratory flow rate (PEFR) decreased in this patient and asthmatic signs developed. However, there was no decrease in the PEFR after challenge with pemirolast, levocabastine, or fluorometholone (0.1%) eyedrops, or saline as control. CONCLUSIONS: This case suggests that mast-cell-stabilizer eyedrops might induce an asthma attack in patients with a history of asthma and aspirin or nonsteroidal antiinflammatory drug allergy. Mast-cell-stabilizing eyedrops should be prescribed for such patients with special precautions unless the patient is known to tolerate mast cell stabilizers without difficulty.

Predominance of infiltrating IL-4-producing T cells in conjunctiva of patients with allergic conjunctival disease.

PURPOSE: Previous reports have suggested that type 2 cytokine responses at the site of inflammation are important in the pathogenesis of allergic disease. In this study, we investigated the frequency of IFN-gamma- or IL-4-producing T cells from conjunctiva or peripheral blood mononuclear cells (PBMC) from patients with allergic conjunctival diseases. METHODS: We obtained conjunctival samples using Cytobrush and peripheral blood samples from the patients with allergic conjunctival disease. Conjunctival samples and PBMC were stimulated with phorbol myristate acetate and calcium ionophore. Frequencies of cytokine-producing T cells were analyzed by flow cytometry based on the intracellular cytokine staining. RESULTS: The frequency of IL-4-producing conjunctival T cells in patients with vernal keratoconjunctivitis (VKC) and atopic keratoconjunctivitis (AKC) were significantly higher than that in patients with allergic conjunctivitis (AC). The frequencies of IL-4-producing conjunctival CD4+ T cells of the patients with AC, AKC, or VKC were significantly higher than those of peripheral blood CD4+ T cells of the same disease. Further, increased frequency of P-selectin ligand expressing IL-4-producing T cells was observed in conjunctiva compared to that in PBMC of the patients with allergic conjunctival diseases. The frequencies of IL-4-producing peripheral blood T cells in the patients with ACD were significantly higher than that in normal controls. CONCLUSION: These results suggest that IL-4-producing T cells that have infiltrated into the conjunctiva play an important role in the immunopathogenesis of allergic conjunctival diseases, and P-selectin ligand is involved in the entry of T cells into allergic inflammatory site.
