Anti-adenoviral effect of anti-HIV agents in vitro in serotypes inducing keratoconjunctivitis.

BACKGROUND: Around one million people are affected by adenoviral keratoconjunctivitis a year in Japan, and it is recognized as one of the major pathogens of ophthalmological nosocomial infection worldwide. Although cidofovir can be used systemically for immunocompromised patients with disseminated adenoviral infection, no specific anti-adenoviral agent has been established for the treatment of adenoviral infection. We evaluated the anti-adenoviral effect of anti-HIV (human immunodeficiency virus) agents in this study. METHODS: Five anti-HIV agents (zalcitabine, stavudine, nevirapine, indinavir and amprenavir) were subjected to in vitro evaluation. A549 cells were used for viral cell culture, and adenovirus serotypes 3, 4, 8, 19 and 37 were used. After calculating CC(50) (50% cytotoxic concentration) of each agent by MTS (3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium) method, we cultured adenovirus with the agents for seven days and quantitatively measured extracted adenoviral DNA by real-time PCR. RESULTS: Among the anti-HIV drugs, zalcitabine and stavudine, both nucleoside reverse transcriptase inhibitors, showed significant anti-adenoviral activity. In contrast, nevirapine, a non-nucleoside reverse transcriptase inhibitor, and indinavir and amprenavir, which are both protease inhibitors, were ineffective against adenovirus. CONCLUSIONS: These results indicate that zalcitabine and stavudine are possible candidates for the local and systemic treatment of adenoviral infection, and the anti-adenoviral effect might depend on the pharmacological properties of anti-HIV agents. The chemical properties on the clinical safety for systemic and local application need to be determined in order to for these drugs to be accepted for the treatment of adenovirus in clinical settings.

Novel OPA1 mutations identified in Japanese pedigrees with optic atrophy.

PURPOSE: To determine whether mutations in the OPA1 gene were present in two Japanese families with optic atrophy. METHODS: Thirty exons and their boundaries of the OPA1 gene were amplified by PCR with genomic DNA as templates and directly sequenced. The detected sequence changes were confirmed to be mutations by examining whether they were present in normal control individuals. A splicing mutation was characterized by RT-PCR of total RNA of leukocytes obtained from patients and one normal individual. The mutant transcripts resulting from the splicing mutation were further confirmed and quantified by sequencing and identifying the denatured RT-PCR products by polyacrylamide electrophoresis. RESULTS: One novel splicing mutation of c.871-1G>T and one novel insertion mutation of c.579_580insTT (p.R194fsX228) were identified from two familial cases, respectively. Both mutations segregated within the family heterozygously and were not found in the 189 control individuals examined. Two mutant transcripts resulted from the splicing mutation were identified through amplified OPA1 cDNA prepared from the RNA of leukocytes of the patients. One had a 21 bp deletion at the beginning of the exon 9 leading to a 7 amino acid in-frame deletion of the protein. The expression level of this mutant transcript was similar to the transcript from the wild type allele of the patient. The other mutation was a 114 bp deletion, leading to a 38 amino acid in-frame deletion that skipped all of exon 9, and the expression of this mutant transcript was much lower than the 21 bp deletion. CONCLUSIONS: The predicted consequence of both mutations is the loss of GTPase activity. Our findings further establish the involvement of OPA1 mutation in Japanese patients with optic atrophy and serve as supportive evidence that haploinsufficiency of the OPA1 gene is the cause of the optic atrophy.

Comparison of visual function after epiretinal membrane removal by 20-gauge and 25-gauge vitrectomy.

PURPOSE: To compare the efficacy and safety of 25-gauge vitrectomy with 20-gauge vitrectomy for macular pucker. DESIGN: Randomized clinical study. METHODS: Sixty-seven eyes (67 patients) with macular pucker were prospectively and randomly assigned to undergo 20-gauge vitrectomy or 25-gauge vitrectomy as a control group. Main outcome measurements consisted of best-corrected visual acuity (BCVA), mean induced astigmatism, total operation time, and postoperative complications. RESULTS: The mean BCVA at baseline and at one month and six months after surgery was 20/78, 20/62, and 20/42, respectively, in the 20-gauge group, and 20/85, 20/45, and 20/40 in the 25-gauge group. A significant difference in change in the logarithm of the minimal angle of resolution of BCVA between 20-gauge and 25-gauge was found at one month (-0.10 vs -0.269, P < .001), but not at six months (-0.327 vs -0.276, P > .01). CONCLUSIONS: If faster visual improvement is desired, 25-gauge vitrectomy may be preferable to 20-gauge vitrectomy for macular pucker.

The early efficacy of topical levocabastine in patients with allergic conjunctivitis.

BACKGROUND: We investigated the early efficacy of topical levocabastine, an H(1) histamine-receptor antagonist, in improving the clinical symptoms of allergic conjunctivitis. METHODS: Thirty-six patients with allergic conjunctivitis were enrolled. One drop of levocabastine was instilled in one eye and one drop of artificial tears in the contralateral eye. Clinical examinations were performed before, and 15 and 30 minutes after instillation. Symptoms of itching and signs of injection were assessed at each time point. RESULTS: Both levocabastine and artificial tears resulted in a statistically significant reduction in ocular itching. However, levocabastine was significantly more effective. CONCLUSIONS: Although artificial tears had a positive effect in reducing symptoms of allergic conjunctivitis, by the washing out of allergens, levocabastine was more effective than artificial tears in controlling acute symptoms of allergic conjunctivitis, demonstrating that the selective H1 histamine-receptor antagonist action of levocabastine is rapidly effective in a clinical setting.

[Trial of isolation of sick house syndrome and unclassified multiple chemical sensitivities--Definition of sick house syndrome, and symptoms].

PURPOSE: This study was designed to clarify the definition of sick house syndrome (SHS). METHODS: SHS was defined based on the disease related to habitation as follows. 1. The cause of the onset of a disease relates to house. 2. Symptoms appear within house. 3. Symptoms will be less serious or disappear if patient away from house. 4. If patient goes into house, symptoms will appear repeatedly. When it corresponded to all above, it was defined to SHS, and it classified as MCS (multiple chemical sensitivities) without above conditions. Even if SHS is isolated from similar disease completely, characteristic symptoms of MCS are hard to be detected because MCS are combination of two or more diseases. Based on this working hypothesis, the logistic regression by setting MCS as reference was performed so that characteristic symptoms of SHS show odds ratios with exceeding one. RESULTS: The odds ratios with more than two of characteristic symptoms in SHS were "nausea or vomiting" "Troublesome in everything" and the causative substances to which symptoms get worse was "The smell of a perfume and cosmetics". Characteristic symptoms of an allergy disease were detected by comparison with the allergic conjunctivitis, allergic rhinitis, and bronchial asthma, respectively. CONCLUSION: These results showed that the classification method was appropriate. This definition is not fundamentally differed from the definition of the sick-building syndrome of WHO.

[Simulation of airbag impact on eyes after trabeculectomy by finite element analysis method].

PURPOSE: A finite element computer model of the human eye after trabeculectomy was used in an experiment of simulated airbag ocular injury. METHODS: A half-layer-incised scleral flap was created on the limbus and the strength of its adhesion to the outer sclera was set at 30%, 50% and 100%. The simulations were performed at a workstation using the finite element analysis program PAM CRASH (Nihon ESI, Tokyo, Japan), and the airbag was set to hit the surface of the post-trabeculectomy eye at various velocities in two directions, straight to the corneal center or straight to the scleral flap. RESULTS: In the case of airbag impact on the corneal center, the scleral flap was unlikely to rupture except when the airbag impact velocity was 40 m/sec. In the case of airbag impact on the scleral flap, at the lowest impact velocity of 20 m/sec, partial scleral flap rupture was likely to occur only at the lower adhesion strength, and scleral laceration extending to the posterior sclera was observed at impact velocities of over 30 m/sec. CONCLUSIONS: These simulation results suggest that current airbags may induce globe rupture in eyes after trabeculectomy treatment.

[New medical treatment for viral conjunctivitis].

Around one million people in Japan are suffering from adenoviral conjunctivitis every year and it is recognized as one of the major pathogens of nosocomial infection. Several complications, such as corneal erosion and conjunctival pseudomembrane, are observed in some of the cases and corneal sube- pithelial opacity may bring visual impairment. Moreover, no specific anti-adenoviral agent has been discovered and an effective treatment has not been established for adenoviral infection. We have researched new medical treatment for viral conjunctivitis based on recent findings in adenoviral conjunctivitis. Firstly, anti-adenoviral activity was evaluated in vitro in agents which could possibly act as anti-adenoviral drugs. Twelve candidates, such as zalcitabin, interferon beta, etc., were selected among antiviral drugs, adenoviral receptor inhibitors, natural products and anti-inflammatory drugs. Remarkable anti-adenoviral effect was observed in zalcitabin, sanilbudine, interferon beta and anti-osteopontin peptide. Two anti-human immunodeficiency virus (HIV) drugs with anti-adenoviral activity, zalcitabin and sanilbudine, are nucleoside reverse transcriptase inhibitors, but, in contrast, non-nucleoside reverse transcriptase inhibitors and protease inhibitors were ineffective against adenovirus. Interferon beta and anti-osteopontin peptide displayed anti-adenoviral effects by absorption inhibition. Secondly, side effects caused by possible anti-adenoviral eye drops, including cidofovir whose development as eye drops against eyeball and ocular adnexa had been suspended, were analyzed in a white rabbit model. In animals given cidofovir locally, significant narrowing of lacrimal canaliculus, redness of eyelid and conjunctival injection was observed, but obstruction of the lacrimal duct was not found. Although zalcitabin and sanilbudine eye drops induced eyelid redness, no change was observed in the lacrimal route and conjunctiva. In animals treated by cidofovir, inflammation histologically suggesting mainly allergic change was observed. These results indicate that these four drugs are possible candidate for safe eye drops against adenoviral conjunctivitis. These four agents are divided into two categories, inhibitors of adenoviral replication, zalcitabin and sanilbudine; and suppressors of adenoviral infection, interferon beta and anti-osteopontin peptide. It is expected that eye drops for specific treatment of adenoviral conjunctivitis are going to be available in the near future following investigation of therapeutic effect in adenoviral infected animals and clinical trials in humans.

Multicoated contact lens for bimanual vitreous surgery without endoillumination.

We developed a new multicoated contact lens for use with the bimanual vitrectomy technique. The new lens is made of quartz, and its multicoated surface has properties that decrease light reflection and increase light absorption. The lens is placed on the cornea, and the coaxial light from the operating microscope alone enables surgeons to obtain a clear image of the retinal surface. This new vitrectomy contact lens is very useful for the treatment of eyes with proliferative membranes and allows surgeons to perform vitrectomy by using both hands.

Two patients with severe corneal disease in KID syndrome.

PURPOSE: To report two independent Japanese patients with keratitis, ichthyosis, and deafness (KID) syndrome and severe corneal disorder. DESIGN: Observational case reports. METHODS: Clinical observation of a 5-year-old boy (Patient 1) and a 64-year-old man (Patient 2) with KID syndrome, presenting prominent corneal diseases. Molecular genetic assessment of the GJB2 gene encoding connexin-26 was performed. RESULTS: Patient 1 had bilateral diffuse superficial punctuate keratopathy with severe corneal neovascularization. He had a missense mutation of the GJB2 gene. Patient 2 had bilateral corneal stromal keratitis and right corneal ulceration with rupture of the Descemet membrane. He did not have any pathologic mutation of the GJB2 gene. The area of palisades of Vogt was diminished and tear production reduced in both patients. Topical eye drops, and corticosteroid or antibiotics, respectively, relieved them effectively. CONCLUSION: The impaired ocular surface regulating system might be a cause of corneal disease in KID syndrome and it can be treated by eye drops.

Simulation of air-bag impact on an eye with transsclerally fixated posterior chamber intraocular lens using finite element analysis.

PURPOSE: To determine the physical and mechanical conditions of an impacting air bag that would rupture an eye with a transsclerally fixated posterior chamber intraocular lens (IOL). SETTING: Numerical simulation study on a computer. METHODS: Simulations in a model human eye were performed with a computer using the finite element analysis program PAM-CRASH (Nihon ESI). The air bag was set to impact the surface of an eye with a transsclerally fixated posterior chamber IOL at various velocities. The tensile force limit of a 10-1 polypropylene suture was assumed to be 0.16 N, which is specified in the U.S. Pharmacopeia XXII. RESULTS: At the lowest velocity of 20.0 m/s, 10-0 polypropylene sutures were not likely to break. Sutures fixating the IOL might break and a corneoscleral incision was likely to open after 0.3 second at the medium impacting velocity (30 m/s). Suture rupture was very likely at the highest velocity (40 m/s) since the tensile force on the sutures continuously exceeded the breaking force after the impact. CONCLUSIONS: In an eye with a transsclerally fixated posterior chamber IOL, severe ocular trauma can be caused by an air bag at high velocity. Small individuals such as elderly women are at greater risk for air-bag ocular injury. Further research on modifying air-bag design and deployment is important to minimize the risk for ocular injury.

Simulation of airbag impact on eyes after photorefractive keratectomy by finite element analysis method.

BACKGROUND: A simulation model of the human eye which we have developed was applied to simulated airbag ocular injury, to determine the physical and mechanical conditions of the impacting airbag that would cause globe rupture in a post-photorefractive keratectomy (PRK) eye. METHODS: Simulations were performed with a computer using the finite element analysis program PAM-CRASH()(Nihon ESI, Tokyo, Japan). The airbag was set to impact on the surface of post-PRK eyes-D3, D6, D10, and D15-and an intact eye at various impact velocities. Strain on the cornea and sclera exceeding 18.0% and 6.8%, respectively, was assumed to indicate the possibility of rupture of each tissue. RESULTS: In contrast to the intact eye, in post-PRK eyes, at the lowest velocity of 20 m/s, some of the element reached the strain threshold in D15. At the medium velocity of 30 m/s, limited corneal rupture was observed in all situations. At the high velocity, 40 m/s, scleral laceration was found in eyes with all diopters, and apparent corneal rupture was observed in D10 and D15, indicating that globe rupture was very likely to occur. CONCLUSION: These results suggest that severe ocular trauma can be caused in post-PRK eyes by airbags at high impact velocities. Preoperative discussion with candidates for laser refractive surgery regarding the potential for severe ocular injury if the normal integrity of the eye is compromised by surgery may be appropriate. Research on modification of airbag design and deployment to minimize the risk of ocular injury is important.

Absorption of short wavelengths of endoillumination in indocyanine green solution: implications for internal limiting membrane removal.

PURPOSE: To measure the absorption coefficients of short wavelengths, irradiated by a light source used for vitreous surgery, in indocyanine green (ICG) solution and balanced salt solution (BSS), and to determine the implications of these absorption coefficients on retinal phototoxicity caused by ICG-assisted internal limiting membrane removal. METHODS: The absorption coefficients of short wavelengths irradiated by a commercially available light source used for vitreous surgery were measured in ICG solution and BSS using a dual-beam spectroradiometer. RESULTS: The absorption coefficient of wavelengths irradiated by the endoillumination light source in ICG solution was almost the same as that obtained in BSS. The absorbance coefficients of the ICG solutions were 0.18 cm(-1) at 400 nm and 0.03 cm(-1) at 450 nm. In BSS, the absorption coefficients were 0.17 cm(-1) at 400 nm and 0.015 cm(-1) at 450 nm. No significant difference in absorbance was seen between 400 nm and 450 nm ( P>0.05). CONCLUSIONS: The absorption of wavelengths is not greater in ICG solution than in BSS. ICG solution during intravitreal use probably does not enhance retinal photochemical injury during ICG-assisted internal limiting membrane removal.

Measured visual acuity of fellow eyes as a prognostic factor in macular hole surgery.

PURPOSE: To investigate the effect of the vision of the fellow eye on the visual acuity of eyes after successful macular hole surgery. DESIGN: Prospective consecutive series. METHODS: A consecutive series of eyes with successful macular hole closure were studied and assigned to one of two groups according to the visual acuity of the fellow eye; a group with visual acuity less than 20/200 and a group with visual acuity of 20/200 or better. Preoperative and postoperative visual acuity in the two groups was measured based on the logarithm of the minimal angle of resolution (LogMAR), and postoperative visual acuity was also determined by using the multiple-letter visual acuity chart, which permitted measurement of visual acuity at an extrafoveal point. RESULTS: Group 1 (<20/200) consisted of 19 eyes and group 2 (>20/200) consisted of 51 eyes. LogMAR visual acuity at 6 months postoperatively was significantly better in group 1 than in group 2 (0.21 vs 0.41, P <.01). The logMAR change 6 months after surgery was significantly greater in group 1 than in group 2 (0.49 vs 0.23, P <.01). There was no significant difference between two groups in logMAR visual acuity at 6 months postoperatively determined with the multiple-letter visual acuity chart (0.20 vs 0.29, P >.05). CONCLUSIONS: Visual recovery after successful macular hole surgery is inversely correlated with vision in the fellow eye. Learning to use eccentric fixation may contribute to visual improvement after macular hole surgery.

Tear osteopontin levels in patients with allergic conjunctival diseases.

BACKGROUND: Recent reports have revealed the importance of several cytokines in chronic conjunctival allergic diseases (ACD) such as vernal keratoconjunctivitis (VKC) and atopic keratoconjunctivitis (AKC). Osteopontin (OPN) is a noncollagenous adhesive matrix protein that is expressed by activated macrophages. There has been considerable interest in the potential role of OPN in monocyte infiltration at sites of inflammation. We measured OPN level in tears using ELISA, to determine whether the level of this cytokine is elevated in ACD. METHODS: The level of OPN in tears was measured by ELISA using samples from patients with VKC, AKC or allergic conjunctivitis (AC) and from normal subjects. The level of OPN in tears was compared with the clinical severity of ACD and serum level of total IgE. RESULTS: The level of OPN in tears in AKC patients was significantly higher than that in AC and normal controls. Tear level of OPN in patients with VKC was also significantly elevated compared to those with AC and to normal controls. The clinical severity of ACD correlated significantly with the level of OPN. However, no correlation was observed between tear OPN level and serum level of total IgE. CONCLUSIONS: These results indicate that OPN plays an important pathophysiological role in severe ocular allergic conditions and that an elevated level of OPN in tear fluid reflects the local clinical status of ocular allergy, which may be an example of tissue remodeling.
