Continent cecoileal conduit: preliminary report.

A continent cecoileal conduit to improve rehabilitation after external urinary diversion is described. The isolated cecum functions as a reservoir. The continence mechanism is constructed by intussuscepting the terminal ileum into the isolated cecum to form a nipple, preventing leakage of urine. The kidneys are protected from cecoureterorenal reflux by ureterocecostomies with submucous tunnels. We have used this technique on 4 patients, 2 of whom need no external urinary pouch since the urostoma is absolutely continent. One patient became incontinent 3 months postoperatively and is wearing a conventional bag and 1 patient died. There is no refluex into the kidneys. The upper urinary tract, which is dilated slightly 4 weeks postoperatively, tends to become normal after a few months.

The physical properties of polyglycolic acid sutures (Dexon) in sterile and infected urine.

In a series of in vitro tests the time-dependent alteration of the resistance to breaking and of the extensibility of catgut and of polyglycolic acid sutures (Dexon) was examined. The threads were incubated in sterile and infected urine up to 10 days at 36 C. In the sterile urine environment Dexon dissolved on the 6th day and in the infected urine on the 3rd day. Catgut did not dissolve at all in the sterile urine and in the infected urine it dissolved only on the 8th day. In view of these results the use of Dexon in the urine environment may cause some concern, especially in an infection exists.

Controlled prevention of post-operative thrombosis in urological diseases with depot heparin.

The frequency of venous thrombosis after urological operation was investigated by means of iodine fibrinogen in 65 patients chosen at random. 34 patients were treated with depot heparin (Calciparin). Out of 31 patients, 18 (58%) presented an increase of the fibrinogen radioactivity. Moreover, these 18 untreated patients presented clinical signs of thrombosis. On the other hand, the fibrinogen radioactivity was increased in 4 patients (12%) of the Calciparin group. Comparison of the results of the control group and the experimental group is quite significant. The advantage of the prevention of thrombosis is more obvious in longer lasting operations.
