[Venous and arterial digitalised video subtraction angiography (DVSA). Initial experience with 120 examinations (author's transl)].
Digitalised video subtraction angiography permits the demonstration of arteries in the trunk by means of intravenous injection. Initial experience with 120 examinations has demonstrated its usefulness for showing the arteries in the neck, the aorta, the renal arteries, pelvic arteries and arteries in the thigh in between 80 and 90% of cases. Use of the technique during arterial arteriography avoids the need for photographic subtraction and permits a reduction of contrast volume and flow rate. Further clinical experience will be necessary in order to determine the value of the technique.

[Arterial vascular occlusion in penicillin allergy (author's transl)].
A nodular exanthema of the skin was observed in an 8-year-old girl. She had been treated with penicillin a few days ago because of a gastrointestinal infection. Continuation of penicillin treatment led to occlusion of larger arteries with gangrene of the forefoot. This was accompanied by septic temperatures and superinfected skin lesions. Angiologic and angiographic investigations showed a right-sided femoro-popliteal occlusion and occlusions of the arteries of the lower leg. On the left side there was spasm of the superficial femoral artery. After cessation of penicillin and high-dose steroid therapy rapid restitution occurred. Arterial spasms regressed, the occlusions remained persistent, but there was adequate collateral blood supply and ischaemic acral necroses healed. An allergic reaction of the intermediary type involving major arteries can be considered causative underlying pathology.
Observations on the structure of mechanically stretched chromatin fibrils.
During the spreading of human chromosomes prepared from cultured human lymphocytes, the peripheral fibers attaching the chromosome to the support film are mechanically stretched. The stretching reveals structural elements of the fibers; some of these features have been described earlier, for extracted chromatin. The molecule of DNA is bared through stretching and binds labeled actinomycin D in consequence. One of the prominent features of stretched fibers is the irregularity of resulting structures. This paper further demonstrates the excellent resolution obtainable by electron impact evaporation of tungsten.
[Early clinical experience with nuclear magnetic resonance (author's transl)].
Previous work on adults with carcinomas, benign polyps, postoperative changes or inflammatory or radiation induced swellings in the facial skeleton has shown that it is possible to demonstrate the local anatomy, and particularly the orbits and sinuses without subjecting the patients to radiation. Differentiation between fat, oedema and malignant tissue appears to be possible, as has been demonstrated by our early cases. Early attempts at examining the trunk could only be carried out in infants. These suggest that in the thorax and abdomen differentiation of anatomical structures is not possible. The value of nuclear magnetic resonance in clinical diagnosis is still uncertain. Its advantage depends on the ability to differentiate tissues because of their varying water content. It raises the possibility of differentiating soft tissues by a non-invasive method and without using radiation. Further investigations are necessary in order to evaluate the clinical usefulness of the new technique. The physical basis of the method, compared with conventional radiology, permits only guarded optimism.
Percutaneous sclerotherapy of varicocele.
The results of selective phlebography of the spermatic vein are reported in 580 infertile men with abnormal sperm morphology and suspected varicoceles. Percutaneous transvenous retrograde sclerotherapy of the internal spermatic vein was performed in 260 of the patients. Follow-up phlebography in 109 patients 3 to 6 months after treatment revealed a persistent varicocele in only 3% of them.
[Embolization of renal artery -- an alternative in nephrectomy in the treatment of patients with dialysis and malignant hypertension (author's transl)].
Embolization of renal arteries (coil embolization by Gianturco) was performed in four patients with malignant renal hypertension as an alternative to bilateral nephrectomy. Three patients with successful renal transplantation developed intractable hypertension without arterial stenosis of the transplant. One patient treated with hemodialysis showed malignant hypertension refractory to volume depletion and antihypertensive therapy. In all cases hypertension could be improved and thus medical therapy be reduced.
[Computer-assisted tomography, B-scan sonography and cerebral angiography in obliterations of the carotid artery (author's transl)].
The real-time-B-scan is a new and essential method for diagnosing carotid stenoses. Whereas computer-assisted tomography shows only a minor sensitivity for detection of obliterations of cervical arteries, B-scan sonography has high sensitivity and specificity. B-scans should be increasingly performed both after uncertain clinical and computer tomographic findings prior to angiography of extracranial cerebral vessels. A more stringent indication for angiography and thus avoidance of investigational and irradiation risk can be achieved. Wide use of B-scan sonography may, through early recognition of carotid obliterations, lead to prevention of cerebral insults as patients can undergo vascular surgery at an early stage.
[Percutaneous transluminal catheter dilatation of kidney artery stenoses in patients with renovascular hypertension].
14 patients with severe hypertension showed unilateral (7) or bilateral (6) stenoses of renal artery due to arteriosclerosis (12) or fibromuscular hyperplasia (7); one stenosis appeared after renal transplantation. Successful application of percutaneous transluminal angioplasty of renal artery was performed in 14 stenoses. 4 of 10 patients showed normal values of blood-pressure, in 4 patients blood-pressure was improved, in 2 patients hypertension remained unchanged. The patients were observed 1 to 23 months after dilatation; antihypertensive drugs were necessary in all cases, but could be reduced. The effectiveness of the procedure was demonstrated by renal arteriogram and determination of pre- and poststenotic blood-pressure values.

Selective sclerotherapy of the internal spermatic vein in patients with varicoceles.
Percutaneous occlusion therapy of insufficient internal spermatic veins was performed successfully as an outpatient procedure in 155 patients with varicoceles. The administration of the sclerosing agent (Varicocid) through a catheter selectively introduced into the spermatic vein caused therapeutic occlusion. Follow-up studies, both clinical and phlebographic, confirmed the effectiveness of the treatment.
Percutaneous dilatation and recanalization of iliac and femoral arteries.
Five-year follow-up studies in patients with superficial femoral arterial occlusions and iliac artery stenoses have demonstrated a high success rate with percutaneous transluminal angioplasty performed by the Dotter technique. The special balloon catheters developed by Grüntzig, a standardized accessory pharmaceutical regimen, and more exact indications have further increased the rate of long-term success; the patency rates now approach 70% after three years. Clinical follow-up studies by Doppler ultrasound are useful for assessing the hemodynamic effects of the procedure.
