Nifedipine alters serum theophylline levels in asthmatic patients with hypertension.

The effect of nifedipine on serum theophylline levels in 13 female hypertensive patients having asthma on theophylline therapy has been investigated. Administration of a slow release theophylline product in a dose of 200 mg bidaily for 15 days provided a steady-state through serum theophylline concentration (11.2 +/- 2.7 micrograms/ml). After this period, 10 mg bidaily nifedipine was added to therapy and trough serum theophylline levels, pulmonary function tests and blood pressure measurements have been performed following 15 and 45 days of simultaneous use of theophylline and nifedipine. No change has been observed in serum theophylline level after 15 days of simultaneous use, however after 45 days, serum theophylline level was significantly lower (7.3 +/- 1 micrograms/ml). There were no changes in clinical responsiveness of either of these drugs.

Hydatid disease of the liver: current surgical management.

We present our experiences with 44 cases of hydatid disease of the liver. Two-thirds were females, and the average age was 44. The most common symptom was abdominal pain (63.6%), and the most common sign was abdominal mass (48%). Eosinophilia, positive Weinberg and Cassoni tests, ultrasonography and CT scanning were the major tools for diagnosis. There were 67 cysts, 49 (73%) located at the right lobe, 18 (27%) located at the left lobe. Twelve (18%) were complicated. The most common complication was intrabiliary rupture. After evacuation of the cyst, we managed the cyst cavity with one or more of the following procedures: omentoplasty, tube drainage, capitonnage, partial cystectomy, cystectomy and scolicidal agent injection. Infected cases were drained, and choledochotomy and internal or external drainage were performed for intrabiliary ruptured cases. Tube drainage and omentoplasty did not increase mortality. Average hospital stay was 11 days. There was no operative mortality.

[Herpes simplex virus type 2 antibody levels in sera from various age groups using ELISA and IFAT].

We have investigated HSV-2, IgG antibody levels in 180 sera which were obtained from 60 girl students between 18-20 years old, 60 pregnant women between 19-39 years old and 60 young men between 20-22 years old, by using ELISA (Enzyme Linked Immunosorbent Assay) and IFAT (Indirect Fluorescent Antibody Technique). All of the sera obtained from girl students and pregnant women were found negative for HSV-2, IgG antibody by ELISA. However 2 of 60 young men's sera were positive. Totally 2 of 180 sera showed positivity for HSV-2. IgG antibodies (% 1.11). Whilst all of the girl students' sera were negative with IFAT but 4 of 60 pregnant women's sera and 3 of 60 young men's sera were positive with the same test. So, the positivity rate was % 3.89 with IFAT. These findings suggest that HSV-2 infection is not so common in our population.

[Identification of pathogenic Escherichia coli strains in various clinical material and their antibiotic susceptibility].

In this study 200 E. coli strains were isolated and identified from 95 fecal, 96 urinary and 9 vaginal specimens. The pathogenic species isolated from feces were 61.05% of all identified E. coli strains. This percentage was 26.04% for urinary specimens. The distribution of E. coli serotypes was determined. Mostly E. coli O111 K58 B4 strain was isolated from the pathological materials such as 20% in feces, 12.5% in urine, 11.11% in vaginal secretions. The sex distribution of isolated E. coli strains from urine was 77.08% for female patients and 22.92% male patients. We can say that E. coli species which have been isolated and identified from pathological materials are susceptible to seftriaxon 73.5% and sefotaxime 72.5%, but they have been readily gaining resistance against gentamicin and nalidixic acid. Broad spectrum antibiotics and chemotherapeutic agents are widely used in the treatment of E. coli infections. However medical therapy doesn't always produce good result. Because E. coli strains have been gaining resistance to chemotherapeutic agents and antibiotics. So, it is suggested that the most effective antibiotic should be determined by antibiogram and used.

[Rubella virus IgG and IgM antibody levels in 17-20 year old female students using ELISA and fluorescent antibody tests].

In this study Rubella virus antibody levels were investigated by using ELISA and IFAT in 94 sera obtained from girl students in Gülhane Military Medical Academy nursing college. We have propagated Rubella virus in BHK-21 cell line for production Rubella virus antigen in order to use IFAT. 81 sera IgG (86.2%) and 29 sera IgM (30.9%) were found positive in ELISA. However 76 sera (80.9%) IgG and 27 sera (28.7%) were positive in IFAT. On the other hand we have obtained suspected results in 8 sera (8.5%) with 1/10 dilution in IgG ELISA and in 3 sera (3.2%) with the same dilution in IgM ELISA. One of 29 sera which are ELISA IgM positive have established 80 IU/ml RF. We can say that ELISA is more reliable sensitive and practical than IFAT. It may be considered 29 student who have Rubella IgM antibody may be infected or reinfected by Rubella virus with in the last six months. On the other hand they may also infected with Parvovirus or some other viruses. We believe that the causes of false positive Rubella IgM antibody results should be investigated well enough.

Sonographic and MRI findings in prepubertal adnexal hemorrhagic cyst with torsion.

Adnexal torsion is rare before menarche. We report the case of a 10-year-old girl with persistent left lower quadrant pain proven by surgery to be caused by adnexal torsion due to a hemorrhagic cyst. Sonography showed a well-defined, complex, predominantly solid mass with some sound through-transmission and a small amount of fluid. The left ovary could not be distinguished from the mass; the right ovary appeared normal. Doppler sonography demonstrated no blood flow within the mass. MRI revealed a circumferential region of high signal intensity in the periphery of the mass and multiple hyperintense foci in the left ovary.

An unusual presentation of immunodeficiency with hyper-IgM.

Hyper-IgM syndrome is a rare immunodeficiency disease characterized by markedly decreased serum IgG, IgA, and IgE levels but normal or elevated IgM levels. The most common clinical signs are infections, cirrhosis, arthritis, malignancies, and mucosal ulcers. Approximately two-thirds of patients have chronic neutropenia associated with oral and perirectal ulcers. We report a 14-month-old girl with hyper-IgM syndrome who has recurrent cutaneous ulcers restricted to the diaper area.

Clinical, andrological and genetic characteristics of patients with congenital bilateral absence of vas deferens (CBAVD).

Congenital bilateral absence of the vas deferens (CBAVD) is a form of infertility with an autosomal recessive genetic background in otherwise healthy males. In this study, we examined the clinical and cystic fibrosis transmembrane-conductance regulator (CFTR) gene mutations in sixty patients with bilateral absence of vas deferens that applied to andrology clinic due to male factor infertility. Urogenital anomalies of vas deferens, seminal vesicle and epididymis were detected in our patient group. CFTR gene mutations, which are known to be frequent among cystic fibrosis patients, could not be detected in our patient group with that high frequency. Delta F508 mutations were detected in only 6% of patients. IVS8 polyT alleles were positive in 68% of patients. No 1677delTA mutations and M470V variants were detected in our patient group. However, sperm retrieval is almost always possible from CBAVD patients; secondary pathologies may also result defective spermatogenesis.

Neuronal organization in area 17 of cat visual cortex.

An antibody to microtubule-associated protein 2 (MAP2) has been used to examine the arrangements of neurons in striate cortex of the cat. It is found that the apical dendrites of medium-sized and large pyramidal cells in layer V group together to form clusters that have a center-to-center spacing of about 56 microns. As these clusters ascend, the apical dendrites of pyramidal cells in layer II/III are added to them. The thinner apical dendrites of the smaller pyramidal cells in layer VIa also form groups that are referred to as bundles. These bundles ascend into layer IV independent of the clusters, and their arrangement suggests that the bundles are formed so that the apical dendrites of the layer VIa pyramids can pass between the groups of cell bodies of the layer V neurons. It is proposed that the clusters formed from the apical dendrites of the layer V and layer II/III pyramidal cells represent the axes of vertical modules of pyramidal cells, which represent the basic neuronal aggregates within area 17 of cat visual cortex. And it is suggested that these modules can be recruited in various combinations, most obviously by the excitation provided by the thalamic inputs, to form the functional columns, such as the ones that are concerned with eye preference and orientation. Based upon the distribution of the dendritic clusters, the pyramidal cell modules would have diameters of 56 microns, and since they are considered to extend through the depth of the cortex, each one would contain some 203 neurons. The striate cortex of one hemisphere contains 160,000 of these modules, which is about the same as the number of X-cells projecting to one hemisphere from the dorsal lateral geniculate nucleus and twice the number of X- or beta-ganglion cells in the retina. The form of the pyramidal cell modules in cat striate cortex is compared to those present in monkey striate cortex, in which the similar modules are about 10 times more numerous, but only 31 microns in diameter (Peters and Sethares, 1991a).

A case with multiple gynecological malignancies.

A patient with cervical non-Hodgkin lymphoma was treated with chemotherapy. Fourteen months after the diagnosis of the lymphoma, an endometrial adenocarcinoma was detected as a secondary malignant tumor. The patient was treated with surgery followed by radiotherapy. Approximately 7 years after the diagnosis of endometrial cancer, vaginal invasive squamous cell carcinoma was diagnosed as the third primary malignancy, and a second-line palliative radiotherapy was applied. Seven months after the last radiotherapy, postradiational sarcoma in the vagina was diagnosed. Congenital and acquired immune system disorders, viral oncogenes, and various human leukocyte antigen (HLA) types were investigated. Total blood count and lymphocyte subset analysis were performed, and CD4+ lymphopenia was detected. Serologic tests were carried out for human immunodeficiency virus, hepatitis B virus, human papillomavirus, Epstein-Barr virus, and herpes simplex virus infection. Epstein-Barr virus viral capsid antigen IgG was found positive. Low-risk human papillomavirus panel was detected by Hybrid Capture method in the cervical smear. The HLA investigation revealed HLA-A2, HLA-A3, HLA-B57, HLA-B35, HLA-B4, HLA-B6, HLA-DR3, HLA-DR1, HLA-DR51, HLA-DR52, HLA-DQ6(1), and HLA-DQ7(3). The patient died because of the disease.

Factor V (His 1299 Arg) in young Turkish patients with cerebral infarct.

Inherited gene defects related to the coagulation system have been reported as risk factors for stroke. Recently, a genetic component in the factor V (FV) gene that contributes to activated protein C resistance both in the presence and absence of FV 1691 G-->A was reported. This highly conserved FV gene haplotype was marked as R2 polymorphism, an A to G alteration at position 4070 in exon 13 that predicts the His 1299 Arg substitutions. The aim of this study was to evaluate the role of this mutation in Turkish children with ischemic infarct. The case-control study included 48 patients with cerebral infarction; all were 18 years of age or younger (range: 10 months to 18 years). Ten (20.8%) of the 48 patients were found to carry the FV 1299 His-->Arg mutation, one being homozygous. One patient who had a combination of FV 1691 G-->A and protein C deficiency also carried the FV 4070A mutation. A homozygous FV 1299A patient had a prothrombin (PT) 20210A mutation in the heterozygous state. The cerebral infarct risk for FV 1299 was found to be 2.4 (95% confidence interval 0.9-6.8) for all groups. When underlying conditions were excluded, the incidence of FV 1299 was found to be 8/35 (22.8%), but the risk was almost the same. When two other common thrombophilic mutations (i.e. FV 1691 G-->A and PT 20210 G-->A) were excluded, the incidence of FV 4070 mutation increased to 7/21 (33.3%). The risk also increased to 3.9 (95% confidence interval 1.2-12.3).
