Induction of drug metabolizing enzymes by sulfinpyrazone.

A previous interaction study of sulfinpyrazone (Anturano) suggested that it induced microsomal drug metabolizing enzymes in the liver. To verify this finding the effect of sulfinpyrazone 800 mg per day for four weeks was investigated in ten healthy volunteers. Both the therapeutic actions of sulfinpyrazone, the uricosuric and the antiaggregating effects, were demonstrated (p less than 0.05). The influence on the microsomal drug metabolizing system in the liver was demonstrated by an increase in serum-gamma-glutamyl transpeptidase from 15.1 to 23.3 U/l (p greater than 0.05), a significant increase in the urinary excretion of d-glucaric acid (29.6 to 77.9 microMol/24 h, p less than 0.05) and an increase in antipyrine clearance from 50.3 ml/min to 83.9 ml/min (p less than 0.05). The possibility of enhancement of drug metabolism during treatment with sulfinpyrazone in combination with other drugs should be kept in mind.

[Lesions of the epiphyses - classification - therapy - prognosis (author's transl)].

Between 1970 and 1980 53 children with epiphyseal injuries were treated. Analysing these 53 cases we found that metaphyseal lesions of the epiphyseal plate (i.e. epiphyseolysis, separation of the epiphysis with triangular metaphyseal fragment) heal completely, even in those cases in whom an open reduction was necessary. Conservatively treated epiphyseal injuries (i.e. fracture of the epiphysis, crushing of the epiphyseal plate) showed bad results. Contrary to these results 80% of open reduced injuries showed good results. We can conclude that epiphyseal injuries do not necessarily end in disturbances of growth or malformation if surgical correction of the injury is done as early as possible and if the injured extremity is immobilised at least 3 to 5 weeks. Destruction of the epiphyseal vascular system can only be avoided by surgical reposition of the epiphyseal injury.

[Pharmacotherapeutical action on arteriosclerotic vascular diseases with benzarone (author's transl)].

We report the results of a pilot study to evaluate the therapeutical efficiency of Benzarone in a time span of four weeks in 11 male patients with intermittent claudication. Numerous experimental data indicate that there was a statistically significant increase of painless walking distance and a statistically significant decrease of plasma uric acid, cholesterol and triglycerides. Coagulation analyses furthermore show an increase of the factor VIII activity and the plasminogen levels. For the first time, positive results were shown with Benzarone in patients with intermittent claudication.

[The effects of nicotine on the circulation and metabolism after cigarette smoking with reference to the plasma levels of nicotine and COHb (author's transl)].

The effects after smoking cigarettes with different nicotine content (1.5 mg and 0.08 mg nicotine/cigarette) were investigated in 6 healthy habitual smokers. Carboxyhemoglobin (COHb) and plasma nicotine levels were determined at the same time as circulatory parameters, heart rate and blood pressure together with metabolic parameters like blood sugar, lactate, free fatty acids and plasma dopamine-beta-hydroxylase (DBH) and plasma cortisol. With the exception of COHb the parameters investigated were shown to be dependent on the nicotine levels. For blood pressure, heart rate and lactate the peaks appeared simultaneously with the maximum nicotine levels, whereas for DBH, cortisol, blood sugar and the free fatty acids there was a delayed reaction in comparison with the nicotine levels. The parameters investigated are not affected by COHb with levels up to 5.6 +/- 0.5%. These results show that after cigarette smoking, nicotine causes considerable changes in the circulation and metabolism.

[The scintigraphic diagnosis and follow-up of injuries to the epiphyseal plates (author's transl)].

Injuries to the epiphysel plates without involvement of the epiphyses or metaphyses, such as crush fractures or pure epiphysiolysis may be difficult to diagnose radiologically. Thirteen bone scans after damage to the growth plate have been performed on eight children. These indicate that these scans are able to diagnose lesions of the epiphyseal plates at an early stage and with certainty. The scintigrams also provide information concerning the healing process of the plate; they indicate when healing has been completed and when the extremity can be used for weight-bearing again. Radiation exposure of the children during scintigraphy with 99mTc-polyphosphate is within acceptable limits.

[Computer tomographic characteristics of space-occupying processes in the anterior mediastinum and their differential diagnosis (author's transl)].

Histological diagnosis of space-occupying lesions in the anterior mediastinum is rarely possible using conventional radiological diagnoses; computer tomography in our opinion often makes this possible. Our experience is based on 47 tumours which have been investigated histologically. In the anterior upper or middle mediastinum there were thymic tumours (thymona, thymus hyperplasia or persistence of the thymus), retrosternal thyroids, malignant lymphomas, teratoid cysts, one seminoma and one aneurysm. In the anterior inferior mediastinum, we found pericardial cysts, lipomas and one Morgagni hernia. The computer tomographic features of these various lesions are described. Some tumours can be defined histologically by computer tomography alone, whereas others (e.g. solitary lymphoma, benign thymoma) make it necessary to consider the clinical features in addition. Computer tomography is particularly valuable in the diagnosis of persistence of the thymus, since standard radiography in two planes fails to show the lesion.

A rapid and sensitive method for the determination of phenazone (antipyrine) using gas-liquid-chromatography with nitrogen detection.

A simple and rapid method for the quantitative determination of antipyrine, using gas-liquid-chromatography with nitrogen detection, is described. Only one extraction step is needed, the recovery is 91.2% and the precision varies between 3.14--1.96%. The lower limit of quantitative assay reached 0.1 mg/l plasma. During routine handling the method was easy, quick and cheap.
