High alkaline phosphatase activity in isoproterenol stimulated fibroblast cultures from patients with numerically unbalanced chromosomal aberrations.

Treatment of cultured fibroblasts from patients with unbalanced chromosomal aberrations with a mixture of isoproterenol, theophylline and ascorbic acid resulted after 48 hours in an at least three-fold increase of alkaline phosphatase activity on a per cell basis, whereas cells from normal healthy individuals did not show this dramatic response. Cells were studied from patients with trisomy 21 (14 cases), trisomy 18 (3 cases), trisomy 13 (1 case), pentasomy X (1 case), Turner syndrome (2 cases), and Klinefelter syndrome (1 case), and no exception was noted. The mechanism of this phenomenon is not clear, but it is speculated that increased cyclic-AMP levels caused by the action of isoproterenol on adenylcyclase may account for excessive reactions of unbalanced cells as compared to normal cells. This simple biochemical diagnostic procedure might become useful in screening programs for unbalanced chromosomal abberations.

Reliable detection of cystic fibrosis in skin-derived fibroblast cultures.

The specific induction of alkaline phosphatase with Tamm-Horsfall glycoprotein, isoproterenol, and theophylline in skin-derived fibroblast cultures from patients with cystic fibrosis permits one to reliably discriminate between cystic fibrosis patients on the one hand, and heterozygotes and normals on the other. It was found the fibroblast-like and intermediary types of amniotic fluid-derived cells behave essentially like skin-derived fibroblasts. These findings imply that if different amniotic fluid cell types can be reliably separated, prenatal diagnosis of cystic fibrosis should become feasable in the near future.

Reliable diagnosis of the major type of cystic fibrosis with fibroblast cultures. A double blind study.

A double blind study has been carried out to demonstrate that the most common type of cystic fibrosis (CF) can be reliably diagnosed with skin-derived fibroblast cultures. Alkaline phosphatase (AIP) activity has been measured in 6 normal controls, 12 CF-heterozygotes and 6 CF-homozygotes before and after stimulation with Tamm-Horsfall-glycoprotein (THP), isoproterenol and theophyline ("THP-induction test"). The mean AIP-activities after THP-induction were 8.8, 12.7 and 34.6 for the three different genotypes respectively. There was no overlap between the values of CF-homozygotes on the one hand, and the values of CF-heterozygotes and normal controls on the other hand. All 24 specimens were correctly diagnosed in the present double blind study, indicating the very high degree of reliability of the THP-induction test in the detection of the predominant type of cystic fibrosis with fibroblast cultures. Normal controls and CF-heterozygotes could not be discriminated on an individual basis, but as a group the CF-heterozygotes displayed higher AIP-values.

Quantitative ultramicro-scale immunoenzymic method for measuring Ig antigenic determinants in single cells.

The Fluoro Ultra Micro Enzyme Linked Immuno Assay (FUMELIA) allows one routinely and quantitatively to measure a few thousand antigenic determinants on single cells. Highly purified Escherichia coli beta-galactosidase has been coupled to specific antibodies. By use of the Parafilm microcuvette techniuqe, the activity of the antibody-conjugated beta-galactosidase is assayed with a conventional spectrophotofluorometer. Attempts were undertaken to sensitize FUMELIA even further, so as to be able to detect a very few antigenic sites. It seems that even in its present state of development FUMELIA is more sensitive for the quantitation of cell-associated antigens than are techniques in which radiolabeled reagents are used. The potential of FUMELIA is illustrated by the quantitative measurement of membrane-bound immunoglobulins on single lymphocytes. It could be shown that T-cells as well as C-cells can synthesize Ig antigenic determinants. Thus it seems likely that T-cell receptors will, after all, be found to be immunoglobulins.

[Result of an antifibrinolytic treatment using tranexamic acid for the reduction of blood-loss during and after tonsillectomy].

Blood loss during and after tonsillectomy was assessed in 80 patients, 40 of whom were treated with tranexamic acid. A statistically significant reduction of blood loss (28%) was observed in this group during operation. Later bleedings occurred only in 27.5% (control: 67.5%) and stopped after 2 h (mean of control: 5.6h). Bleedings in the tranexamic acid group were mild and scarcely required other hemostyptic treatment, while in the control group some patients had to be treated with etamsylate or fibrogen for a longer period to stop bleeding. Tranexamic acid appears to be an effective and well-tolerated agent for reduction of blood loss caused by local hyperfibrinolysis.

Polycythemia vera treated with -32p and myleran: development of chronic granulocytic leukemia with chromosomal abnormalities in one patient.

Chronic granulocytic leukemia developed in a 59-year-old woman who had previously received a total of 21 mCi -32P for polycythemia vera. She was treated with Myleran (busulphan) for her chronic granulocytic leukemia. Cytogenic studied revealed deletion of chromosomes No. 8 and 12, and translocation between 1 and 8. The patient also developed a severe antoimmune hemolytic anemia, for which she received prednisone treatment. She died with a perforated stomach ulcer.
