[Diagnostic value of atrial natriuretic peptide in hypertension and heart insufficiency].
Serum ANP levels were measured by radioreceptor assay in 40 patients with various forms of secondary hypertension and 6 patients with heart failure. In addition, serum ANP was determined in 4 patients with renal artery stenosis before and after dilatation, as well as in 5 anephric patients before and after haemodialysis. Our results showed elevated serum ANP level in most patients with various forms of secondary hypertension and chronic heart failure. A distinction between these two groups and a control group of healthy individuals was not possible due to the wide range and occasional normal levels in the first two groups. ANP levels in patients with renal stenosis decreased after dilatation but there was no correlation with the success of this procedure. A positive correlation between ANP and plasma renin level was detectable in patients with renal artery stenosis, but was also elevated in anephric patients with absent renin production. In summary, our results show that measurements of serum-ANP are of little significance in the diagnosis of hypertension and chronic cardiac failure.
Microsurgical treatment of lens capsule perforations. Part I: Experimental research.
Although perforating injuries of the lens capsule typically lead to traumatic cataract in humans and rats, capsular healing is observed regularly in injured rabbits and dogs. This is most likely because protein exudation in rabbits and dogs is much greater than that in humans and rats. Using isolated white Wistar rat lenses, rat lenses in vivo, and isolated human lenses, we attempted both to replace lens protein as well as develop techniques for microsurgically closing the capsule by applying a fibrinogen-based tissue adhesive. This application was effective in preventing traumatic cataract. Use of a fibrinogen-glued capsule graft yielded similarly good (but not better) results.

[Development of bacterial flora in leg ulcer].
In the present study of the microbiological flora of 284 ulcera crurum we found 452 various bacterial strains. 174 (61.3%) of the 284 ulcera crurum were found in women, 110 (38.7%) in men with a predominance in the age group between 70-79 years. Staphylococcus aureus were the most frequent bacteria, with an isolation rate of 22,4%. Furthermore, gram-negative bacteria like pseudomonas (14.6%), proteus (10.2%) were isolated. Besides these other species of the group of enterobacteriaceae must be taken into consideration--especially Enterobacter and Klebsiella. In comparison to the results of bacterial isolation rates reported in the literature of the last 15 years, it is obvious that staph. aureus is most common in the bacterial invasion of ulcera crurum . An increase of isolation rates of pseudomonas and proteus and other gram-negative germs could not be found during the last 10 years. The variety of bacterial species on leg ulcers and the frequent occurrence of mixed flora give evidence of the importance of microbiological diagnostic and antibacterial therapy.
[Effects of rifamycin SV and a neomycin-bacitracin combination on the healing of microbial eczema].
21 patients suffering from microbial eczema were locally treated with antibiotics over a period of 14 days. As antibiotics we chose rifamycin SV and a combination of neomycin and bacitracin. S. aureus--most frequently traceable on microbial eczema--is usually sensitive to rifamycin SV as well as neomycin and bacitracin in combination.
[Oral 13-cis-retinoic acid therapy in adenoma sebaceum symmetricum and the most severe forms of acne and rosacea].
Twenty-one patients with severe acne (acne papulopustulosa, acne conglobata, acne cystica), six patients with severe rosacea (rosacea paulopustulosa, rosacea conglobata, rhinophyma), and three patients with tuberous sclerosis were treated with 13-cis-retinoic acid for 6-48 weeks. Most patients had been previously treated with dermabrasion, antibiotics or metronidazole. Dependent on the severeness of the pathological symptoms 13-cis-retinoic acid was administered at a dose of 0.2 to 0.5 mg/kg body weight and was then reduced every 4 weeks. We confirm the sebostatic and antiinflammatory effect of the 13-cis-retinoic acid and long-lasting remissions. Side effects had not been serious.
[Z-shaped wound closure of circular defects by wedge flaps].
This article deals with several possibilities of closing round wound defects. We especially refer to the wedge-shaped shifting flap technique that has been developed at the Department of Dermatology of the University of Marburg. This method can be applied, if roundish defects have a diameter of 1.0 up to 2.8 cm.

Uptake and killing of Leishmania mexicana amazonensis amastigotes by human skin fibroblasts.
This paper describes the in vitro infection of human established fibroblast lines by Leishmania mexicana amazonensis amastigotes. Intracellular parasites were located within vacuoles. The proportion of infected cells reached a peak of about 50% on Days 2 or 3, and decreased to almost 0 on Days 6 or 8. Transmission electron microscopy was used to document different stages of the degeneration of L. m. amazonensis amastigotes on Days 4 and 5. The proportion of infected fibroblasts decreased in both the control and the irradiated cells, indicating that dilution of the parasites by cell multiplication was not the main factor in the observed decrease of the infection with time. This conclusion was also supported by studies with fibroblasts prelabelled with 3H thymidine. In these studies, total cell associated radioactivity as well as the total and TCA precipitable radioactivity of the culture medium were similar for infected and non-infected fibroblasts during the course of infection. The abortive infection of fibroblasts by L. m. amazonensis provides a potentially useful model for the study of the relationship between host cells and intracellular parasites.
Diagnosis of cystic fibrosis homozygotes and heterozygotes from plasma and fibroblast cultures. A three-generation family study.
The diagnosis of cystic fibrosis (CF) homozygotes and heterozygotes and of individuals without the CF gene, based on differences in the thermal stability of acid phosphatase and alpha-mannosidase, is reported. The residual activities at 36.5 degrees C and 41.3 degrees C were below 10% of the activity in unheated samples for homozygotes, 40-50% for heterozygotes and above 90% for normals. The intracellular alkaline phosphatase and extracellular beta-hexosaminidase activities after treatment with heparin and gammaglobulin were 500% and 200%, respectively, of the activities without this treatment in CF homozygotes, whereas for heterozygotes and normals the values were the same after treatment as before. Pedigrees of four CF families, covering 2-3 generations, are presented. The possible use of these tests as diagnostic tools is further discussed.
Enzyme activity profiles in mouse teratocarcinomas. A quantitative ultramicroscale analysis.
Nine tumour lines with different developmental capacities were derived from spontaneous as well as from one induced teratocarcinoma: three teratocarcinoma-derived rhabdomyosarcomas TDR 602, TDR 694, and TDR 114; two teratocarcinoma-derived neuroblastomas TDN 2151 and TDN 2283; two teratocarcinoma-derived endodermal tumours TDE 274 and TDE 113; one multipotential teratocarcinoma OTT 2289, and one undifferentiated teratocarcinoma OTT 2158. Quantitative analyses of ten catabolic enzymes, i.e. alkaline and acid phosphatase, alpha- and beta-galactosidase, alpha- and beta-glucosidase, alpha-mannosidase, alpha-fucosidase, beta-glucuronidase, and hexosaminidase were carried out at the 20-cell level, and specific enzyme activity profiles were established for each of the tumour lines studied. These profiles may be used for the biochemical identification of a tumour type at the single cell level in addition to morphological and biological criteria.

Alkaline phosphatase and acid lysosomal hydrolases in pancreatic juice and fibroblast cell cultures of patients with chronic calcifying pancreatitis.
Nine lysosomal enzymes and alkaline phosphatase have been assayed in human pancreatic juice from controls and patients with chronic calcifying pancreatitis. Specific activities were evaluated by a nonparametric test (Wilcoxon) with a probability of 2 P less than or equal to 0.5. The values of acid phosphatase, alpha-glucosidase, beta-glucosidase and alpha-galactosidase are significantly higher in pathological juices; the values of alpha-mannosidase and beta-glucuronidase are also increased in the same patients but at the limit of significance. Alkaline phosphatase, beta-hexosaminidase and alpha-fucosidase follows the same trend but the values are not statistically significant between the two groups of patients. Studies on skin cultures of four patients with chronic calcifying pancreatitis demonstrate that the increased specific activities of lysosomal enzymes in the pathological juices do not correspond to a leakage of these enzymes into the extracellular space as described for cystic fibrosis.
Heat inactivation of plasma hydrolases as a means of discriminating between the genotypes in cystic fibrosis: the experience in Israel.
The ability to discriminate between cystic fibrosis (CF) patients, carriers and control subjects, by determining the degree of hear inactivation of certain plasma enzymes has recently been reported. Material from Israel on 39 patients, 68 carriers and 65 control subjects, was tested under the originally described conditions with concordant results and distinct separation of the three genotypes. No carriers were detected in the 65 control subjects, perhaps because of the dilutional effect of those ethnic groups in Israel among whom the frequency of the CF gene is known to be rare. Biochemically, no evidence of heterogeneity of CF was found among the various ethnic groups.

Detection of cystic fibrosis homozygotes and heterozygotes with plasma.
Evidence is accumulating which indicates that the basic defect in cystic fibrosis interferes with the formation of recognition markers on hydrolytic enzymes. As a result these hydrolases leak into the extracellular space and some of them display abnormal thermolabilities. By heat inactivation of plasma alpha-mannosidase at 41 degrees C and plasma phosphatase at 36.5 degrees C, both enzymes being assayed at pH 5.4, it is possible to differentiate normal people from cystic fibrosis carriers and patients.
